The management of postamputation pain remains a significant clinical challenge, with existing therapeutic approaches often yielding inconsistent outcomes. Neuromodulation techniques, particularly peripheral nerve stimulation (PNS), have emerged as promising interventions. However, the evidence supporting their effectiveness in treating phantom limb pain (PLP) and residual limb pain (RLP) remains limited.

Comprehensive characterization of the transcriptional landscape in Alzheimer's disease (AD) brains.

Sci Adv

January 2025

Department of Biostatistics and Health Data Science, School of Medicine, Indiana University, Indianapolis, IN 46202, USA.

Alzheimer's disease (AD) is the leading dementia among the elderly with complex origins. Despite extensive investigation into the AD-associated protein-coding genes, the involvement of noncoding RNAs (ncRNAs) and posttranscriptional modification (PTM) in AD pathogenesis remains unclear. Here, we comprehensively characterized the landscape of ncRNAs and PTM events in 1460 samples across six brain regions sourced from the Mount Sinai/JJ Peters VA Medical Center Brain Bank Study and Mayo cohorts, encompassing 33,321 long ncRNAs, 92,897 enhancer RNAs, 53,763 alternative polyadenylation events, and 900,221 A-to-I RNA editing events.

Preventative treatment of tuberous sclerosis complex with sirolimus: Phase I safety and efficacy results.

Ann Child Neurol Soc

June 2024

Division of Neurology, Department of Pediatrics, Cincinnati Children's Hospital Medical Center, University of Cincinnati College of Medicine, Cincinnati, OH, USA.

Objective: Tuberous sclerosis complex (TSC) results from overactivity of the mechanistic target of rapamycin (mTOR). Sirolimus and everolimus are mTOR inhibitors that treat most facets of TSC but are understudied in infants. We sought to understand the safety and potential efficacy of preventative sirolimus in infants with TSC.

While mostly de novo truncating variants in SCAF4 were recently identified in 18 individuals with variable neurodevelopmental phenotypes, knowledge on the molecular and clinical spectrum is still limited. We assembled data on 50 novel individuals with SCAF4 variants ascertained via GeneMatcher and personal communication. With detailed evaluation of clinical data, in silico predictions and structural modeling, we further characterized the molecular and clinical spectrum of the autosomal dominant SCAF4-associated neurodevelopmental disorder.

Article Synopsis
  • - Spinal cord stimulation (SCS) is a minimally invasive method used to treat severe neuropathic pain, but it's important to be aware of possible biologic complications during implantation.
  • - Although biologic complications like infection and nerve injury happen less often than device-related issues, they can have more serious effects on patients.
  • - Understanding these potential complications is essential for doctors to ensure patient safety and improve outcomes when using SCS for pain management.

Background: The relationship between inflammatory biomarkers (IB) and organ space surgical site infections (OS-SSIs) after emergency laparotomy (EL) is poorly understood.

Methods: Retrospective, single-center analysis of patients in the Pragmatic, Randomized Optimal Platelet and Plasma Ratios (PROPPR) trial who underwent EL and survived 48 h after admission was performed. IB levels of IL-6, IL-8, G-CSF, MCP-1, neutrophil to lymphocyte ratio, and platelet to lymphocyte ratio were analyzed.

Distinguishing from -related disease: qualitative analysis of examination and imaging features.

Ophthalmic Genet

November 2024

Retina Consultants of America, Retina Consultants of Texas, Houston, Texas, USA.

Introduction: and -related diseases are both phenotypically heterogeneous and clinically difficult to differentiate. There may be examination and imaging features that can aid in establishing a clinical diagnosis.

Methods: A single-center, retrospective, consecutive case series including patients with a molecular confirmation of pathologic variants in either the ABCA4 or PRPH2 were included.

Article Synopsis
  • - The study evaluates the effectiveness of the 5-minute Apgar score in predicting neonatal mortality specifically in infants with major birth defects, indicating its substantial predictive capability.
  • - Analyzing data from the Texas Birth Defect Registry, researchers found that low Apgar scores (less than 7) were strongly linked to higher mortality rates within the first 28 days of life, even among different defect categories and types of births (preterm and term).
  • - The findings suggest that the Apgar score can serve as a valuable tool for quick clinical assessments of newborns with birth defects, but further research is necessary to optimize its use in this group.

Article Synopsis
  • - The study explored the use of acellular dermal regeneration templates (ADRT) to heal myelomeningoceles in newborns, a condition that poses risks due to central nervous system infections and challenges in surgical repair.
  • - A review of seven cases revealed that many patients experienced complications from previous repair attempts, leading to the need for ADRT to enhance the healing process and protect vital structures.
  • - Results showed that after ADRT placement, four patients developed healthy granulation tissue within three weeks, and all wounds healed without infections, suggesting ADRT is a promising option for difficult myelomeningocele repairs.

Article Synopsis
  • - The study analyzed the OXYGEN trial, which compared high levels of oxygen (80% FiO2) vs. lower levels (30% FiO2) for preventing infections in patients with specific types of fractures and aimed to see if using an "as-treated" approach changed the results compared to the "intention-to-treat" method.
  • - Conducted at 29 trauma centers, 1,231 patients with tibial plateau, tibial pilon, or calcaneus fractures were randomly assigned to either oxygen treatment; adherence was evaluated using two specific criteria based on the percentage of surgery time at different oxygen levels.
  • - Results showed no significant differences in primary and deep infection rates, but the treatment group had fewer
