Background: Suture-loaded long hollow needles are used in arthroscopic surgery to pass or retrieve sutures and in meniscal repair surgeries. However, manual suture loading can be time-consuming and challenging, especially with finer sutures. We present a novel technique using vacuum suction to simplify and expedite suture loading in hollow needles and similar devices.

Importance: The diagnosis of ramp lesions can be problematic, even with arthroscopy, due to their extreme posteromedial position. Consequently, they have been colloquially referred to as the "hidden lesions" of the knee. Undiagnosed and untreated injuries in this knee region may be associated with ongoing dynamic rotational laxity of the knee after anterior cruciate ligament reconstruction and an increased risk of anterior cruciate ligament graft failure.

Case: We present a 15-year-old adolescent girl with acute onset knee pain and a lesion in the posterior septum (PS) believed to be an infectious process on initial investigations. This was excised successfully using a combination of transseptal and transnotch portals and proven to be a case of heterotopic ossification (HO) within the PS with an excellent outcome within 4 weeks. Our case is unique for the pathology that occurred and the technique of excision.

Proximal fibular osteotomy has been proposed as a simple and inexpensive alternative to high-tibial osteotomy and unicondylar knee arthroplasty and may be useful for low-income populations that cannot afford expensive treatment methods. However, there is no consensus existing regarding the mechanism by which it acts nor the outcome of this procedure. While cyclops lesion, a fibrous nodule on the tibial side of the knee joint, is a well-known condition complicating anterior cruciate ligament, inverted cyclops lesion, a fibrous nodule on the femoral side of the knee, is a relatively less known condition. We report a case of inverted cyclops in a patient who presented with chronic knee stiffness eight years after supracondylar nailing of a femoral shaft fracture. There are only four reported cases of inverted cyclops in literature.
