The case we presented confirms how echocardiography is accurate in the diagnosis of even challenging coronary artery origin anomalies such as L-ACO-IS. Coronary artery origin evaluation should be an integral part of echocardiography examinations in infants, children, and young adults, especially those undergoing physical activity.
