A case of seminal vesicle-rectal fistula is reported. A 74-year-old Japanese man was admitted to our hospital due to repeated right scrotal swelling and high fever with pneumaturia. A diagnosis of the right acute epididymitis was made.

A case of solid variant type of pancreatic serous cystadenoma mimicking islet cell tumor.

We report a case of the solid variant type of pancreatic serous cystadenoma in which the very small size of the cyst on ultrasonography and computed tomography made distinguishing this tumor from other hypervascular solid tumors difficult. Recognizing the imaging features of high signal intensity on heavily T2-weighted image on MR may be helpful for correctly diagnosing the solid variant type of pancreatic serous cystadenoma.

[Patient absorbed dose in coronary angiography determined by the flat panel digital detector X-ray system].

The INNOVA 2000, an all-digital cardiovascular X-ray system with flat panel detector, is equipped with a monitoring function that makes it possible to track a patient's absorbed dose by displaying the real-time presumed absorbed dose. We verified this dose monitoring system and evaluated how it is affected by various parameters. We also compared the INNOVA 2000 to a conventional machine, the Advantx LC.

Leiomyomatosis peritonealis disseminata with malignant change in a man.

Leiomyomatosis peritonealis disseminata (LPD) is a rare clinicopathological entity typically observed in women of reproductive age. We report a case of LPD with malignant change in a man. A 77-year-old man presented with a mass measuring 10 cm in diameter at the terminal ileum and numerous peritoneal small nodules that were revealed by abdominal computed tomography.

Primary diffuse tracheobronchial amyloidosis: radiologic findings.

Reports detailing the radiologic features of tracheobronchial amyloidosis are scant and typically describe only the computed tomography findings of this disease. No reports have described the magnetic resonance (MR) findings of tracheobronchial amyloidosis. With this case, the authors describe the radiologic findings in a patient with diffuse tracheobronchial amyloidosis, focusing particularly on the MR and bone scintigraphy findings in this disease.

Arterial hyperintensity on fast fluid-attenuated inversion recovery images: a subtle finding for hyperacute stroke undetected by diffusion-weighted MR imaging.

Background And Purpose: Diffusion-weighted MR imaging is generally acknowledged to be more sensitive in detecting acute stroke than is conventional MR imaging. Our purpose in the present study was to evaluate the utility of fast fluid-attenuated inversion recovery (FLAIR) MR imaging compared with that of diffusion-weighted MR imaging for the diagnosis of hyperacute stroke.

Methods: We reviewed patient records and cerebral MR images from all patients in a 13-month period from whom diffusion-weighted and fast-FLAIR imaging were obtained within 6 hours after symptom onset (n = 11).

Effect of fluvoxamine on the pharmacokinetics of mexiletine in healthy Japanese men.

Background And Objectives: Fluvoxamine, a selective serotonin reuptake inhibitor, is known to inhibit several hepatic cytochrome P450 (CYP) isozymes, in particular CYP1A2. Mexiletine is mainly catalyzed by CYP2D6 and partially catalyzed by CYP1A2. Our objective was to study the potential pharmacokinetic interaction between fluvoxamine and mexiletine.

Cases: A 71-year-old male (case 1) and a 79-year-old female (case 2) presented with asymptomatic gross hematuria.

"Ivy sign" on fluid-attenuated inversion-recovery images in childhood moyamoya disease.

We report on MR studies of a 15-year-old girl with moyamoya disease in whom diffuse leptomeningeal enhancement ("ivy sign") was revealed by fluid-attenuated inversion-recovery (FLAIR) and contrast-enhanced imaging. We speculate that the mechanism behind this enhancement is caused by a retrograde slow flow of engorged pial vasculature via leptomeningeal anastomosis. Nevertheless, it remains unknown whether the precise source of a high signal on FLAIR images is attributable to pial vessels themselves or congested thickening of the leptomeninges or both.

Efficacy of extracorporeal ultrafiltration of ascitic fluid as a treatment of refractory ascites.

Background: Refractory ascites is recognized in patients with various conditions. Although intravenous reinjection of ascitic fluid after its filtration and concentration (IRA) is an effective method of treating this condition, many associated side-effects have been reported. We performed extracorporeal ultrafiltration of ascitic fluid (EUA) to demonstrate the efficacy and advantages of this method of treating refractory ascites.

Infective endocarditis-induced crescentic glomerulonephritis dramatically improved by plasmapheresis.

A 50-year-old woman was referred to our hospital because of skin purpura, anemia, high fever, and acute renal insufficiency. Five years ago, she had been diagnosed as having ventricular septal defect without any complications. A blood culture drawn during the hospitalization grew Streptococcus viridans.

Objective: To investigate the effect of omeprazole on the pharmacokinetics of mexiletine.

Methods: Nine healthy male Japanese volunteers participated in a crossover study. On day 1, the subjects received mexiletine 200 mg.

Evolution of coronary vascular tone in vasospastic angina.

In general, anginal symptoms diminish with time in patients with vasospastic angina. We assessed changes in coronary vascular tone (CVT) in patients with vasospastic angina over a 4-year period to evaluate the time course of spastic activity. Vasospastic angina was evaluated in 39 patients in whom occlusive spasm was evoked by selective intracoronary injection of ergonovine maleate (ERG-S) 48 h after stopping all coronary vasodilator drugs.

A 34-year-old Japanese male had leg pain, edema of the legs, hypohidrosis, whorl-like opacities of the bilateral cornea, bilateral subcapsular cataracts, and chest discomfort on exercise. He had no characteristic angiokeratomas but did have telangiectases. The electrocardiogram revealed high voltage.

Regression of coronary atherosclerosis by combined LDL-apheresis and lipid-lowering drug therapy in patients with familial hypercholesterolemia: a multicenter study. The LARS Investigators.

The purpose of the LDL-Apheresis Regression Study (LARS) group, which included 13 institutions in Japan, was to investigate the effects on coronary atherosclerosis of LDL-apheresis combined with cholesterol-lowering drugs. Changes in coronary artery stenosis were assessed angiographically in 37 patients with familial hypercholesterolemia (7 homozygotes and 25 heterozygotes) and hypercholesterolemia which had not been defined as familial hypercholesterolemia (5 patients) by visual judgement and computer analysis. Definite regression was observed in 14 cases, including 4 homozygotes and 10 heterozygotes and others.

A case of penile tumor: combination of leiomyosarcoma and squamous cell carcinoma.

Leiomyosarcoma of the penis is a very rare malignant tumor. We describe a patient with a combination of leiomyosarcoma and squamous cell carcinoma, apparently the first case in the world, and review previous cases of leiomyosarcoma.

Carcinosarcoma of the bladder diverticulum: a case report and review of literature.

A 56-year-old man with carcinosarcoma of the bladder diverticulum is reported. Histologically, the tumor contained two elements: carcinomatous and sarcomatous components with a transitional phase between them. The related literature is also reviewed.
