Introduction: The association between air pollution and cardiovascular diseases is well established. However, fewer studies focused on the relationship between air pollution and peripheral artery disease (PAD), notwithstanding that not only it is a predictor of CVD mortality but also that incidence is globally rising, particularly in low-middle income countries.

Objectives: The aim of this study is to estimate the association between long-term exposure to air pollutants and the incidence of PAD in the Rome Longitudinal Study (RLS) during 2011-2019.

Alterations in subcortical brain regions are linked to motor and non-motor symptoms in Parkinson's disease (PD). However, associations between clinical expression and regional morphological abnormalities of the basal ganglia, thalamus, amygdala and hippocampus are not well established. We analyzed 3D T1-weighted brain MRI and clinical data from 2525 individuals with PD and 1326 controls from 22 global sources in the ENIGMA-PD consortium.

Article Synopsis
  • The study aimed to compare the effects of personalized positive end-expiratory pressure (PEEP) versus a standard low PEEP/FiO2 table on patients with acute respiratory distress syndrome (ARDS) undergoing pressure support ventilation (PSV).
  • Researchers conducted a cross-over randomized trial with 30 ARDS patients, measuring how different PEEP settings affected lung mechanics and patient comfort.
  • Results showed that the personalized PEEP significantly improved lung function, reduced respiratory effort, and did not lead to lung overdistension, making it a promising approach for treating ARDS during ventilation.

Therapeutic Gene Editing for Hemoglobinopathies.

Mediterr J Hematol Infect Dis

Department of Clinical Sciences and Community, University of Milan, IRCCS Ca' Granda Foundation Maggiore Policlinico Hospital, Milan, Italy.

In the last ten years, a consistent number of clinical studies have evaluated different gene approaches for the treatment of patients with sickle cell disease (SCD) and transfusion-dependent β-thalassemia (TDT). Initial studies of gene therapy for hemoglobinopathies involved the use of lentiviral vectors to add functional copies of the gene encoding β-globin in defective CD34 cells; more recently, gene editing techniques have been used involving either CRISPR-Cas9, transcription activation-like effector protein nuclease, zinc finger nuclease, and base editing to either induce fetal hemoglobin production at therapeutic levels or to genetically repair the underlying molecular defect causing the disease. Here, we review recent gene editing studies that have started the development of a new era in the treatment of hemoglobinopathies and, in general, monoallelic hereditary diseases.

Article Synopsis
  • - The study aimed to evaluate how different surface treatments (machined surface, laser-treated, and laser with hydroxyapatite) affect bone tissue behavior using a biomimetic approach on rabbit tibiae.
  • - Methods included implantation of 30 implants in 20 rabbits, where various analyses like SEM, biomechanical tests, and histometric measurements were utilized to assess bone formation around the implants.
  • - Results indicated that laser-treated implants, especially those with hydroxyapatite, showed significantly better bonding and integration with bone tissue compared to machined surfaces, demonstrating that these treatments enhance bone deposition on implants.

Introduction: Ineffective erythropoiesis and subsequent anemia as well as primary and secondary (transfusional) iron overload are key drivers for morbidity and mortality outcomes in patients with β-thalassemia.

Areas Covered: In this review, we highlight evidence from observational studies evaluating the association between measures of anemia and iron overload versus outcomes in both non-transfusion-dependent and transfusion-dependent forms of β-thalassemia.

Expert Opinion: Several prognostic thresholds have been identified with implications for patient management.

Objective: Selected populations of patients with chronic hepatitis B (CHB) may benefit from a combined use of pegylated interferon-alpha (pegIFN-α) and nucleos(t)ides (NUCs). The aim of our study was to assess the immunomodulatory effect of pegIFN-α on T and natural killer (NK) cell responses in NUC-suppressed patients to identify cellular and/or serological parameters to predict better T cell-restoring effect and better control of infection in response to pegIFN-α for a tailored application of IFN-α add-on.

Design: 53 HBeAg-negative NUC-treated patients with CHB were randomised at a 1:1 ratio to receive pegIFN-α-2a for 48 weeks, or to continue NUC therapy and then followed up for at least 6 months maintaining NUCs.

Physiologic Effects of Extracorporeal Membrane Oxygenation in Patients with Severe Acute Respiratory Distress Syndrome.

Am J Respir Crit Care Med

September 2024

Department of Anesthesia, Critical Care and Emergency, Institute for Scientific Research and Care Foundation Ca' Granda, Maggiore Policlinico Hospital, Milan, Italy.

Article Synopsis
  • - Blood flow rate during venovenous ECMO impacts mixed venous oxygenation (Sv), which can influence pulmonary circulation and right heart function in patients with severe acute respiratory distress syndrome (ARDS).
  • - A study evaluated the physiological effects of varying ECMO blood flow levels on Sv in 20 ARDS patients, revealing that higher blood flow rates led to decreased cardiac output, pulmonary artery pressure, and right ventricular workload.
  • - The results showed an inverse relationship between cardiac output and mixed venous oxygenation, indicating that as ECMO blood flow increased, overall cardiac efficiency and workload on the right heart decreased without affecting mismatching measures significantly.

α-Thalassemia is an inherited blood disorder characterized by decreased synthesis of α-globin chains that results in an imbalance of α and β globin and thus varying degrees of ineffective erythropoiesis, decreased red blood cell (RBC) survival, chronic hemolytic anemia, and subsequent comorbidities. Clinical presentation varies depending on the genotype, ranging from a silent or mild carrier state to severe, transfusion-dependent or lethal disease. Management of patients with α-thalassemia is primarily supportive, addressing either symptoms (eg, RBC transfusions for anemia), complications of the disease, or its transfusion-dependence (eg, chelation therapy for iron overload).

Adherence to the Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) diet and exposure to selenium species: A cross-sectional study.

Nutr Res

February 2024

CREAGEN - Environmental, Genetic and Nutritional Epidemiology Research Center, Department of Biomedical, Metabolic and Neural Sciences, University of Modena and Reggio Emilia, Modena, Italy; Department of Epidemiology, Boston University School of Public Health, Boston, MA, USA. Electronic address:

Selenium is a trace element found in many chemical forms. Selenium and its species have nutritional and toxicologic properties, some of which may play a role in the etiology of neurological disease. We hypothesized that adherence to the Mediterranean-Dietary Approach to Stop Hypertension Intervention for Neurodegenerative Delay (MIND) diet could influence intake and endogenous concentrations of selenium and selenium species, thus contributing to the beneficial effects of this dietary pattern.
