Characteristics of Early-Onset vs Late-Onset Colorectal Cancer: A Review.

JAMA Surg

September 2021

Centre for Colorectal Disease, St Vincent's University Hospital, Dublin, Ireland.

Article Synopsis
  • The incidence of early-onset colorectal cancer (diagnosed in individuals under 50) is on the rise globally, with distinct clinical and molecular characteristics compared to late-onset cases.
  • Factors contributing to this increase may include diet, obesity, antibiotic use, and changes in gut microbiome, with most cases occurring sporadically rather than being genetically inherited.
  • There is a need for tailored preventive and therapeutic approaches for early-onset colorectal cancer, as younger patients often present with more severe disease stages but similar outcomes to older patients after treatment.

Background: Teduglutide, a semisynthetic analogue of glucagon-like peptide-2 (sGLP-2), increases intestinal absorption of fluids and nutrients, reducing the need for parenteral nutrition (PN). This report aims to describe our experience with sGLP-2 in a cohort of adult patients with short-bowel syndrome.

Methods: This is a prospective observational study on adult patients initially evaluated in our specialized intestinal rehabilitation program that received sGLP-2 from June 2014 to March 2020.

Background: A cardiac myxoma (CM) is the most common primary tumor of the heart. This case report highlights that for metastatic CMs, even after a successful total resection, there is a small but real risk of recurrence that can manifest as late as 10 years after initial tumor resection.

Case Description: We present the case of a 53-year-old woman who visited the emergency room after a 4-day worsening right-sided weakness.

Background: Bifurcation aneurysms can be treated with stent-assisted coiling using two stents in a Y-configuration. We aim to investigate the angiographic and clinical outcomes of Y-stent constructs for the treatment of intracranial aneurysms.

Methods: A systematic review of PubMed, Ovid MEDLINE, and Ovid EMBASE databases was conducted based on PRISMA guidelines.

Extracellular vesicles (EVs) are a heterogeneous population of vesicles composed of a lipid bilayer that carry a large repertoire of molecules including proteins, lipids, and nucleic acids. In this review, some guidelines for plasma-derived EVs isolation, characterization, and proteomic analysis, and the application of the above to cardiovascular disease (CVD) studies are provided. For EVs analysis, blood samples should be collected using a 21-gauge needle, preferably in citrate tubes, and plasma stored for up to 1 year at -80°, using a single freeze-thaw cycle.

Objective: To evaluate the value of CSF lactate (L) for the diagnosis of ventriculostomy related infections (VRI), and compare it with other CSF markers.

Methods: Prospective study of neurocritical patients admitted to Maciel Hospital and Clinicas Hospital ICUs in which an external ventricular drain (EVD) was inserted. In patients with clinical suspicion of VRI, a CSF sample was obtained through the EVD for CSF culture and markers analysis (glucose, protein, lactate and leukocytes).

Objective: To evaluate the diagnostic value of CSF lactate (L(CSF)) for the diagnosis of bacterial meningitis (BM) following neurosurgery, and compare it with other CSF markers.

Methods: Prospective study of consecutive neurosurgical postoperative patients admitted to the Intensive Care Unit (ICU) at Maciel Hospital. Patients with clinical suspicion of BM were categorised, according to preset criteria, into 3 groups: (1) proven BM; (2) probable BM, and (3) excluded BM.

Five cases of gallbladder adenocarcinoma treated at Surgical Department 2, Maciel Hospital, Montevideo, Uruguay, in a period ranging from 1985 to 1989 and follow-up to date are reported. All cases were first diagnosed by microscopic examination of the gallbladder, which had been removed for presumed benign disease; there were no intramucosal carcinomas, and 60% of the cases were classified as Nevin's stage II (submucosal and muscularis). Based on the pattern of spread (mainly direct hepatic infiltration, as well as venous and lymphatic involvement), our radical surgical treatment of gallbladder carcinoma includes cholecystectomy, extended lymphatic clearance (along the porta hepatis or R1, pre- and retroduodenopancreatic and hepatic artery R2, and celiac axis R3), and medial hepatectomy (segments, IV, V, and VIII according to Couinaud).

Reconstructive surgery in chronic venous obstruction of the lower limbs.

J Cardiovasc Surg (Torino)

May 1991

Service of Vascular Surgery, Maciel Hospital, Montevideo, Uruguay.

Forty-one patients with chronic venous insufficiency due to venous trunk obstruction of the lower limbs underwent 43 flow reconstruction procedures consisting of either free veno-venous bypass or venous transposition. The indicating symptoms for these procedures were (1) progressive chronic edema, (2) claudication while walking, or (3) trophic lesions. Doppler ultrasound and plethysmographic techniques were employed in the diagnosis while phlebography was considered the most useful and most essential technique.
