Integrating large language models (LLMs) into health care holds substantial potential to enhance clinical workflows and care delivery. However, LLMs also pose serious risks if integration is not thoughtfully executed, with complex challenges spanning accuracy, accessibility, privacy, and regulation. Proprietary commercial LLMs (eg, GPT-4 [OpenAI], Claude 3 Sonnet and Claude 3 Opus [Anthropic], Gemini [Google]) have received much attention from researchers in the medical domain, including radiology.

Wildland Firefighters Suffer Increasing Risk of Job-Related Death.

J Burn Care Res

January 2025

Department of Burn Surgery, Medical University of South Carolina, 171 Ashley Avenue, Charleston SC 29425, USA. MD.

Wildland firefighting is a niche specialization in the fire service - inherently dangerous with unique risks. Over the past decade, fatalities amongst all firefighters have decreased; however, wildland firefighter fatalities have increased. This subject has only been described in the grey literature, and a paucity of medical literature exists.

Objective: To evaluate the association between tumour size and the growth rate (GR) of small renal masses (SRMs) in patients managed by active surveillance (AS).

Materials And Methods: We queried the prospective, multi-institutional Delayed Intervention and Surveillance for Small Renal Masses (DISSRM) registry for patients on AS with an imaging interval of ≥6 months, identifying 456 patients. We tracked tumour size over time; a GR >0.

Background: Acute ischemic stroke (AIS) from distal medium vessel occlusion (DMVO) presents unique treatment challenges. Mechanical thrombectomy (MT) is emerging as a viable option for these patients, yet the role of pre-stroke aspirin treatment is unclear. This study evaluates the impact of pre-stroke low-dose aspirin on outcomes in DMVO patients undergoing MT.

Glycocalyx disruption, endothelial dysfunction and vascular remodeling as underlying mechanisms and treatment targets of chronic venous disease.

December 2024

Vascular Surgery Research Laboratories, Division of Vascular and Endovascular Surgery, Brigham and Women's Hospital, Harvard Medical School, Boston, MA, USA -

The glycocalyx is an essential structural and functional component of endothelial cells. Extensive hemodynamic changes cause endothelial glycocalyx disruption and vascular dysfunction, leading to multiple arterial and venous disorders. Chronic venous disease (CVD) is a common disorder of the lower extremities with major health and socio-economic implications, but complex pathophysiology.

This systematic review and meta-analysis aimed to evaluate the effectiveness of psychological interventions compared to standard rehabilitation in patients with knee osteoarthritis (OA). Given the recognized influence of psychological factors in OA management, the review assessed their impact on pain, functional disability, and self-efficacy. A comprehensive search was performed across multiple databases, including PubMed, PsycINFO, CINAHL, the Cochrane Central Register of Controlled Trials, Web of Science, OpenGrey, and ClinicalTrials.

Laboratory of Organic and Medicinal Chemistry, Department of Chemistry, Central University of Punjab, Bathinda, India.

Alzheimer's disease is one of the most complex neurological disorders and millions of people are suffering from this disease all over the world. In the past two decades acetylcholinesterase (AChE) has been the most explored pathological hallmark. The generation of potent AChE inhibitors has grown as a rapid pathological tool for the efficacious treatment of the disease.

Background: Cancer pain is a prevalent and persistent issue, and while there have been some observations of the possible benefits of acupuncture in managing cancer pain, there is still debate regarding its safety and effectiveness. This study aims to compare the efficacy and safety of different acupuncture modalities in the treatment of cancer pain through a network meta-analysis.

Methods: Between the time each database was created and June 3, 2024, eight databases were queried: PubMed, Cochrane, Embase, Web of Science, CNKI, Wanfang, VIP, and China Biomedicine.

Although many parents worry that their child will be the target of racial profiling, there is a dearth of literature on how parental worries about children facing racism are linked to racial socialization (RS) practices and youth internalizing symptoms. Additionally, it is unclear how RS content relative to competency may uniquely influence whether and how parental worries influence youth internalizing outcomes. Using data from 203 Black parents (M = 44.

Background: The association, if any, between the transmitral mean pressure gradient (TMPG) after mitral transcatheter edge-to-edge repair and 1-year mortality is controversial in patients undergoing mitral transcatheter edge-to-edge repair with the MitraClip system. We sought to estimate the association between intraoperatively measured residual mitral regurgitation (rMR) and TMPG and 1-year mortality among patients undergoing mitral transcatheter edge-to-edge repair to facilitate decisions on additional devices.

Methods: In patients with severe secondary (functional) MR, we analyzed registry data using generalized estimating equations.

Background: Severe burns induce volume shifts via capillary leaks, eventually requiring massive fluid resuscitation and promoting tissue edema. Albumin may help to mitigate the edema, thereby improving perfusion. This study shows that sublingual microcirculation measurements can quantify both tissue perfusion and edema.

Background: The effect of the modality of hydrocortisone administration on clinical outcomes in patients with septic shock remains uncertain. This systematic review and meta-analysis evaluate the impact of intermittent bolus and continuous infusion of hydrocortisone on these outcomes.

Methods: We searched the PubMed, Embase databases, and Cochrane Library for randomized controlled trials (RCTs) and cohort studies published from inception to January 1, 2023.

Background: The BCG vaccine induces trained immunity, an epigenetic-mediated increase in innate immune responsiveness. Therefore, this clinical trial evaluated if BCG-induced trained immunity could decrease coronavirus disease 2019 (COVID-19)-related frequency or severity.

Methods: A double-blind, placebo-controlled clinical trial of healthcare workers randomized participants to vaccination with BCG TICE or placebo (saline).

Protein synthesis is by far the most energetically costly cellular process in rapidly dividing cells. Quantifying translating ribosomes in individual cells and their average mRNA transit rate is arduous. Quantitating assembled ribosomes in individual cells requires electron microscopy and does not indicate ribosome translation status.

Introduction: This research is focused on early detection of Alzheimer's disease (AD) using a multiscale feature fusion framework, combining biomarkers from memory, vision, and speech regions extracted from magnetic resonance imaging and positron emission tomography images.

Methods: Using 2D gray level co-occurrence matrix (2D-GLCM) texture features, volume, standardized uptake value ratios (SUVR), and obesity from different neuroimaging modalities, the study applies various classifiers, demonstrating a feature importance analysis in each region of interest. The research employs four classifiers, namely linear support vector machine, linear discriminant analysis, logistic regression (LR), and logistic regression with stochastic gradient descent (LRSGD) classifiers, to determine feature importance, leading to subsequent validation using a probabilistic neural network classifier.

Unlabelled: Parkinson's Disease (PD) is a neurodegenerative disorder that primarily affects persons aged 65 and older. It leads to a decline in motor function as a result of the buildup of abnormal protein deposits called Lewy bodies in the brain. Existing therapies exhibit restricted effectiveness and undesirable side effects.

Supercritical CO modified by polar solvents can extract a wide variety of polar and non-polar chemical components compared to conventional methods. The current study aims to extract Rivas (Rheum ribes) flower using the ethanol modified supercritical CO (SCO-EOH) method; analyze its chemical compounds and bioactivity, encapsulate the extract in maltodextrin, gum-Arabic (GA), and their combination (GA + MD) using the spray drying method and investigate the differences among microparticles using Principal Component Analysis (PCA). The Rivas extract obtained by the SCO-EOH method was a rich source of unsaturated fatty acids (mainly linoleic acid: 57.
