Objective: To compare nationwide outcomes of robotic liver resection (RLR) with laparoscopic liver resection (LLR).

Background: Minimally invasive liver resection is increasingly performed using the robotic approach as this could help overcome inherent technical limitations of laparoscopy. It is unknown if this translates to improved patient outcomes.

Background: In clinical practice, patient self-monitoring is crucial in achieving therapeutic goals in various diseases. In heart failure (HF), it is particularly important due to the increasing role of urine composition. Therefore, we proposed this study to assess the accuracy of urine chloride () assessment via strip test in relation to chloride and sodium (uNa) measurements in a gold-standard laboratory method.

Article Synopsis
  • A new dual chamber leadless pacemaker system utilizes a temperature-based sensor to monitor heart rate response during exercise, focusing on its effectiveness in both the right ventricle and right atrium.
  • An exercise test was conducted 6 weeks post-implant on eligible subjects to evaluate the pacing response of the ventricular and atrial devices, measuring if the rate increased proportionately with metabolic workload.
  • Results showed that both the ventricular and atrial devices had mean rate-response slopes (0.93 and 0.91, respectively) within the acceptable range, indicating that the temperature-based sensor effectively adjusts pacing to meet increased metabolic demands.

Population-Based Cohort Study on Treatment and Overall Survival of Patients Clinically Diagnosed With T1 Ampullary Cancer.

Ann Surg

October 2024

Department of Surgery and School of Nutrition and Translational Research in Metabolism (NUTRIM), Maastricht University, Maastricht, the Netherlands.

Article Synopsis
  • The study aimed to assess treatment outcomes, overall survival (OS), and factors influencing OS in patients with T1 ampullary cancer, a rare type of gastrointestinal cancer.
  • Out of 244 patients with clinical T1 ampullary cancer, 75% underwent surgery, but a significant number were found to have more advanced disease upon further examination.
  • The results indicated a stark difference in survival based on disease classification, with poor prognostic factors including higher pathological N classification and poorly differentiated tumors, emphasizing the need for aggressive treatment like pancreatoduodenectomy for T1 cases.

Surgical Applications for Bone Marrow Aspirate Concentrate.

Arthroscopy

September 2024

Sports Medicine, Department of Orthopedic Surgery, Massachusetts General Hospital, Boston, Massachusetts, U.S.A.

Bone marrow aspirate concentrate (BMAC) is an autologous orthobiologic agent that may be of benefit in specific surgical scenarios. Composed of elements isolated from bone marrow, including mesenchymal stromal cells, bone marrow-derived platelets, red and white blood cells, and hematopoietic precursors, BMAC has gained appeal for its potential to slow the progression of chondral degeneration, improve function, and provide symptomatic relief. BMAC is typically prepared during the final stages of a surgical procedure, beginning with bone marrow aspirate harvested from the iliac crest, distal femur, body of the ilium, or proximal humerus and then centrifuged to yield concentrated marrow cells.

Article Synopsis
  • - Recent advancements in synthesizing organofluorine compounds have primarily focused on traditional methods, but this study introduces a novel enzymatic approach using iron-catalyzed radical transfer for creating C-F bonds.
  • - The researchers repurposed an enzyme called HppE to effectively catalyze a specific type of fluorination to produce chiral benzylic fluoride products, achieving high efficiency and selectivity.
  • - The study's findings suggest that the rate-determining step is related to activating the N-F bond, and the effectiveness of the process increases with the presence of sodium azide, improving the reaction's spatial dynamics.

Nat Immunol

October 2024

Pathology Advanced Translational Research Unit (PATRU), Department of Pathology and Laboratory Medicine, Emory University School of Medicine, Atlanta, GA, USA.

Article Synopsis
  • The study investigates the effects of blocking interleukin-10 (IL-10) and PD-1 on controlling HIV/SIV viral rebound after stopping antiretroviral therapy (ART).
  • In a trial with rhesus macaques, a combination treatment of anti-IL-10 and anti-PD-1 led to sustained control of viral levels in 9 out of 10 monkeys for over 24 weeks post-treatment interruption.
  • Outcomes indicated that specific immune responses, such as increased memory T cells and changes in cytokine levels, could predict successful viral control after ART cessation.

Affilin proteins, artificial binding proteins based on the ubiquitin scaffold, have been generated by directed protein evolution to yield de-novo variants that bind the extra-domain B (EDB) of oncofetal fibronectin, an established marker of tumor neovasculature. The crystal structures of two EDB-specific Affilin variants reveal a striking structural plasticity of the ubiquitin scaffold, characterised by β-strand slippage, leading to different negative register shifts of the β5 strands. This process recruits amino acid residues from β5 towards the N-terminus to an adjacent loop region and subsequent residues into β5, respectively, remodeling the binding interface and leading to target specificity and affinity.

Background: The subcutaneous implantable cardioverter-defibrillator (ICD) is associated with fewer lead-related complications than a transvenous ICD; however, the subcutaneous ICD cannot provide bradycardia and antitachycardia pacing. Whether a modular pacing-defibrillator system comprising a leadless pacemaker in wireless communication with a subcutaneous ICD to provide antitachycardia and bradycardia pacing is safe remains unknown.

Methods: We conducted a multinational, single-group study that enrolled patients at risk for sudden death from ventricular arrhythmias and followed them for 6 months after implantation of a modular pacemaker-defibrillator system.

March 2024

Department of Medical and Surgical Sciences, IRCCS Azienda Ospedaliero, Universitaria di Bologna, 40138 Bologna, Italy.

Metformin is a highly effective medication for managing type 2 diabetes mellitus. Recent studies have shown that it has significant therapeutic benefits in various organ systems, particularly the liver. Although the effects of metformin on metabolic dysfunction-associated steatotic liver disease and metabolic dysfunction-associated steatohepatitis are still being debated, it has positive effects on cirrhosis and anti-tumoral properties, which can help prevent the development of hepatocellular carcinoma.

Article Synopsis
  • A study was conducted to compare the perioperative outcomes of robotic liver surgery (RLS) and laparoscopic liver surgery (LLS) across various healthcare settings from 2009 to 2021.
  • The results showed that RLS had better outcomes in terms of "textbook outcomes," lower blood loss, fewer complications, and shorter operative times compared to LLS after matching patient groups for bias.
  • Despite the higher costs generally associated with robotic surgery, this study suggests that RLS may offer specific clinical advantages over LLS in minimally invasive liver procedures.
