Importance: Early discharge after colorectal surgery has been advocated. However, there is little research evaluating clinical and/or laboratory criteria to determine who can be safely discharged early.

Objective: To evaluate the diagnostic performance of a C-reactive protein (CRP) level combined with 4 clinical criteria in ruling out an anastomotic leak and therefore allowing an early discharge on postoperative day 2 or 3.

NLRC4 GOF Mutations, a Challenging Diagnosis from Neonatal Age to Adulthood.

J Clin Med

September 2021

Pediatric Nephrology, Rheumatology, Dermatology Unit, Hôpital Femme Mère Enfant, Hospices Civils de Lyon, 69677 Bron, France.

The NLRC4 inflammasome is part of the human immune innate system. Its activation leads to the cleavage of pro-inflammatory cytokines IL-1β and IL-18, promoting inflammation. gain-of-function (GOF) mutations have been associated with early-onset recurrent fever, recurrent macrophagic activation syndrome and enterocolitis.

WT1 overexpression is frequently identified in acute myeloid leukemia (AML) and has been reported to be a potential marker for monitoring measurable residual disease (MRD). We evaluated the use of postinduction WT1 MRD level as a prognostic factor, as well as the interaction between postinduction WT1 MRD response and the effect of allogeneic stem cell transplantation (allo-SCT) in the first complete remission (CR). In the ALFA-0702 trial, patients with AML, aged 18 to 59, had a prospective quantification of WT1 MRD.

Electronic address:

Article Synopsis
  • A study was conducted to investigate ovarian reserve and fertility in women with BRCA1/2 pathogenic variants compared to controls facing fertility preservation due to cancer treatments.
  • The study found a trend suggesting that BRCA1/2 carriers may experience a premature decline in ovarian reserve, but average levels of AMH and AFC were similar between the two groups.
  • Importantly, the risk of having low ovarian reserve indicators was significantly higher in BRCA1/2 carriers, highlighting the need for careful monitoring of their reproductive health.

Article Synopsis
  • The study aimed to evaluate how operating room (OR) teams perceive risks and adopt safety measures when conducting hyperthermic intraperitoneal chemotherapy (HIPEC) and pressurized intraperitoneal aerosol chemotherapy (PIPAC).
  • Conducted among OR teams from ten high-volume centers across six countries, the survey collected responses from various professionals, revealing that individual protection measure uptake was low at 51.4%.
  • While PIPAC had a higher perceived protection level, all groups, particularly cleaning staff and anesthesiologists, showed a desire for more information, highlighting the need for better training on safety protocols in these procedures.

Follicular lymphomas (FLs) with MYC rearrangements (MYC-R) and extra copies of MYC (MYC-EC) are rare and the prognosis impact is uncertain. We conducted a retrospective study including 321 FL patients, among whom 259 (81%) had no 8q24 alterations and 62 (19%) were assigned to 8qAlt. Forty-five cases were classified as MYC-EC and six as MYC-R.

IntroductionThe contribution of healthcare-associated infections (HAI) to mortality can be estimated using statistical methods, but mortality review (MR) is better suited for routine use in clinical settings. The European Centre for Disease Prevention and Control recently introduced MR into its HAI surveillance.AimWe evaluate validity and reproducibility of three MR measures.

January 2022

AP-HP, Hôpital Kremlin-Bicêtre, Department of Head and Neck Surgery, School of Medicine, Paris-Saclay University, 94070 Le Kremlin-Bicêtre, France.

Aim: Altered sense of smell and/or taste is a leading symptom of SARS-CoV-2 infection, but its prevalence at a population-level is unknown.

Methods: From a questionnaire addressed to a representative subset of the French general adult (≥18-year) population over a 6-week period during the first French lockdown (April 7 to May 19 2020), self-reported new cases of altered sense of smell and/or taste were collected.

Results: From 29,660 participants, new altered sense of smell and/or taste was 2.

Background: Cytotoxic chemotherapy (CC) is currently used in metastatic melanoma after patients have developed resistance to immune checkpoint inhibitors (ICI) and/or Mitogen-Activated Protein Kinase inhibitors (MAPKi). We sought to evaluate if a previous treatment by ICI or MAPKi influences clinical outcomes in patients treated by CC in metastatic melanoma.

Methods: Eighty-eight patients with a metastatic melanoma, treated by CC after a previous treatment by ICI or MAPKi between January 2009 and October 2019, were retrospectively analyzed.

Myeloid-derived suppressor cells (MDSC) are a heterogeneous population of immature myeloid cells with immunosuppressive properties. In cancer patients, the expression of lectin-type oxidized LDL receptor 1 (LOX-1) on granulocytic MDSC identifies a subset of MDSC that retains the most potent immunosuppressive properties. The main objective of the present work was to explore the presence of LOX-1+ MDSC in bacterial and viral sepsis.

Electronic address:

Food oral immunotherapy (OIT) is a promising treatment for persistent and severe food allergies (FAs) in children, but also for accelerating tolerance to cow's milk and cooked egg in young children. In the near future, pediatricians will increasingly encounter severely allergic children undergoing FA-OIT. FA-OIT consists in daily ingestion of increasing doses of the allergen during the up-dosing phase, and ingestion of a constant dose during the maintenance phase.

Longitudinal assessment of IFN-I activity and immune profile in critically ill COVID-19 patients with acute respiratory distress syndrome.

Crit Care

April 2021

Immunology Laboratory, Hôpital E. Herriot - Hospices Civils de Lyon, 5 place d'Arsonval, 69437, Lyon Cedex 03, France.

Article Synopsis
  • The study examines immune responses in 64 critically ill COVID-19 patients suffering from ARDS during their hospital stay, highlighting the lack of research on this topic amid the pandemic.
  • Results show persistently low lymphocyte counts and high cytokine levels, with a notable initial immune response followed by a decline over time, indicating ongoing immune dysfunction.
  • The findings suggest that ARDS severity is linked to immune changes at ICU admission, resembling patterns seen in immunosuppression during bacterial infections, with implications for patient outcomes and secondary infections.
