Article Synopsis
  • A study investigated whether stereotactic body radiotherapy (SBRT) is just as effective as standard radiotherapy for treating localized prostate cancer in terms of treatment failure rates.
  • The trial involved 874 men with early-stage prostate cancer, comparing SBRT (36.25 Gy in 5 sessions) to conventional radiotherapy (78 Gy in 39 sessions or 62 Gy in 20 sessions) over several weeks, focusing on the chance of being free from cancer progression after 5 years.
  • Results showed that SBRT was noninferior to standard treatment, with similar success rates in preventing cancer failure (95.8% for SBRT vs. 94.6% for control) but higher rates of genitour

Objective: To investigate the expression levels of prothrombin induced by vitamin K absence-II (PIVKA-II) and osteopontin (OPN) in patients with hepatocellular carcinoma (HCC) and cirrhosis, and to evaluate their potential as markers for cirrhosis severity.

Methods: This retrospective study included 84 patients with HCC and cirrhosis treated at the Liver Disease Center of the 904th Hospital from January 2021 to December 2023, forming the cirrhosis group. Fifty healthy individuals undergoing routine physical examinations during the same period comprised the control group.

  • Lateral elbow pain is common and often caused by lateral epicondylitis or radial tunnel syndrome, prompting this study to assess the effectiveness of neuromuscular ultrasound for diagnosis.
  • Conducted with 34 patients, the study measured how well ultrasound could distinguish between the two conditions, showing high sensitivity and specificity compared to physical examination methods.
  • Results indicated that neuromuscular ultrasound had a sensitivity of 92.6% and specificity of 80%, making it a valuable tool for clinicians to accurately diagnose these conditions in patients with lateral elbow pain.

On a Hunt for the "True" Septocutaneous Perforator: A Histology Cross-Section Study.

J Reconstr Microsurg

November 2024

Division of Plastic Surgery, St. Luke's Hospital, Sacred Heart Campus, Allentown, Pennsylvania.

Background:  Modern trends in reconstructive surgery involve the use of free perforator flaps to reduce the donor site morbidity. The course of perforator vessels has a great anatomic variability and demands detailed knowledge of the anatomical relationships and the variability of the course of the perforators. The numerous modifications to perforator nomenclature proposed by various authors resulted in confusion rather than simplification.

Article Synopsis
  • The study looked at whether a medical procedure called thrombectomy helps patients who have a large stroke as seen on a specific type of scan called a noncontrast CT within 24 hours of having the stroke.
  • It involved 300 patients from different hospitals who were split into two groups; one group received the thrombectomy treatment, while the other only received regular care.
  • The main goal was to see if those who had thrombectomy had better recovery after 90 days compared to those who didn’t, along with checking if there were any serious side effects.

  • Cognitive impairment is linked to diabetes, but how they affect each other and impact long-term cognitive outcomes, particularly in older adults with cerebrovascular disease, isn't well understood.
  • A study involving 654 participants analyzed brain MRIs and cognitive tests over 5 years, focusing on various cerebrovascular disease markers.
  • Results indicated that diabetes negatively impacts cognitive performance, particularly in individuals with high levels of white matter hyperintensities, rather than functioning independently or significantly affecting Alzheimer biomarkers.

Limited data exists on the safety and efficacy with the concomitant use of 4 factor prothrombic complex concentrate (4F-PCC) and andexanet alfa (AA). This case series describes 7 patients at our institution who received both 4F-PCC and AA for the management of life-threatening bleeding associated with apixaban or rivaroxaban. Four patients received AA due to worsening bleeding after 4F-PCC.

Purpose: To compare the infection rate of intraarticular platelet-rich plasma (PRP) knee injections between open and closed techniques in randomized controlled trials (RCTs) published in the last decade.

Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, PubMed, Scopus and Virtual Health Library were accessed in October 2022 using the terms 'platelet-rich plasma', 'PRP', 'knee' and 'tibiofemoral' alone and in combination with Boolean operators AND/OR. RCTs published during the last 10 years evaluating PRP intra-articular knee injections were considered eligible.

Background: Compared to normal high-density lipoprotein (HDL) cholesterol values, very high HDL cholesterol is associated with a higher incidence of mortality and atherosclerotic cardiovascular disease (ASCVD). As such, clinical risk stratification among persons with very high HDL cholesterol is challenging.

Objectives: Among persons with very high HDL cholesterol, the purpose was to determine the prevalence of coronary artery calcium (CAC) and compare the association between traditional risk factors vs CAC for all-cause mortality and ASCVD.

Cognitive decline is a frequent complaint in healthy controls and neurological patients, regardless of the underlying pathology. Whilst cognitive impairment can be easily diagnosed in the more advanced stages of neurodegenerative diseases, early detection can be challenging. This is mainly the consequence of the incomplete understanding of the underlying pathophysiological mechanisms.

Objective/background: Preference for extended-release, once-nightly sodium oxybate (ON-SXB, FT218) vs twice-nightly immediate-release (IR) oxybate was assessed in participants switching from IR oxybate to ON-SXB in an open-label/switch study, RESTORE (NCT04451668).

Patients/methods: Participants aged ≥16 years with narcolepsy who completed the phase 3 REST-ON trial, were oxybate-naive, or were on a stable IR oxybate dose (≥1 month) were eligible for RESTORE. For participants who switched from twice-nightly dosing to ON-SXB, initial doses were closest or equivalent to their previous nightly IR oxybate dose.
