Appalachia - Bridging the Opioid Epidemic Amid the Fentanyl Crisis.

Linacre Q

November 2024

Doctor Bioethics and Health Policy, Loyola University of Chicago, Chicago, IL, USA.

Although the opioid epidemic and fentanyl crisis are nationwide problems of immense proportions, calming the storm raging across the isolating geo-economic framework of Appalachia demonstrates the need for courageous, culturally effective programs that have the power to address and overcome the grave situation of addiction in this region. In the Appalachian region, unjust social structures are embedded in the communities' socio-economic conditions. As this reality poses even greater barriers to addressing the opioid epidemic in these communities, the restoration of social relationships within a community becomes vital to the development of a broader approach to social functioning and human flourishing.

Research institutions often lack policies addressing the risks and benefits of enrolling "invested parties" such as investigators, research staff, and patient, caregiver, and community representatives (groups most affected by a disease or intervention) in studies where they have direct involvement. Invested parties may have both strong motivations to study the condition or intervention and to participate as study subjects. More guidance is needed to promote appropriate access to research participation and mitigate potential risks.

Defining the analytical complexity of decision problems under uncertainty based on their pivotal properties.

PeerJ Comput Sci

July 2024

Epidemiology and Biostatistics, University of Illinois at Chicago, Chicago, Illinois, United States of America.

Background: Uncertainty poses a pervasive challenge in decision analysis and risk management. When the problem is poorly understood, probabilistic estimation exhibits high variability and bias. Analysts then utilize various strategies to find satisficing solutions, and these strategies can sometimes adequately address even highly complex problems.

PROGnostic RolE of strain measurements in stress cardiac MRI in predicting major adverse cardiac events.

Int J Cardiol

October 2024

Department of Perioperative Cardiology and Cardiovascular Imaging, Centro Cardiologico Monzino IRCCS, Milan, Italy; Department of Biomedical, Surgical and Dental Sciences, University of Milan, Milan, Italy. Electronic address:

Article Synopsis
  • The study aimed to determine if feature-tracking strain measurements during stress cardiac MRI could help predict long-term risks of major adverse cardiac events (MACE) in patients with coronary artery disease (CAD) without needing contrast agents.
  • After analyzing 729 patients over about 6 years, the presence of late gadolinium enhancement (LGE) and stress global circumferential strain (GCS) were found to be independent indicators of MACE, occurring in 9.6% of the patients.
  • Ultimately, the research demonstrated that a contrast-free stress cardiac MRI with stress GCS could provide similar prognostic information as traditional myocardial perfusion assessments, highlighting its importance for patients with severe renal issues.

Basing intubation of acutely hypoxemic patients on physiologic principles.

Ann Intensive Care

June 2024

Department of Physiology and Pharmacology, Center for Molecular Medicine, Department of Clinical Neuroscience, Karolinska Institutet, Stockholm, Sweden.

The decision to intubate a patient with acute hypoxemic respiratory failure who is not in apparent respiratory distress is one of the most difficult clinical decisions faced by intensivists. A conservative approach exposes patients to the dangers of hypoxemia, while a liberal approach exposes them to the dangers of inserting an endotracheal tube and invasive mechanical ventilation. To assist intensivists in this decision, investigators have used various thresholds of peripheral or arterial oxygen saturation, partial pressure of oxygen, partial pressure of oxygen-to-fraction of inspired oxygen ratio, and arterial oxygen content.

In Vitro Fertilization, State Wrongful Death Statutes and State Fetal Homicide Statutes: The Reaction to LePage v. Center for Reproductive Medicine.

Issues Law Med

May 2024

The author is an attorney in private practice in Illinois who specializes in state and federal constitutional appellate litigation and legislative consulting. Mr. Linton, the former General Counsel for Americans United for Life, has submitted amicus curiae briefs in several Supreme Court abortion cases, including Planned Parenthood v. Casey (1992) and Dobbs v. Jackson Women's Health Organization (2022). Mr. Linton has published more than two dozen law review articles on a variety of topics, as well as the only comprehensive analysis of abortion as a state constitutional right, ABORTION UNDER STATE CONSTITUTIONS (3d ed. 2020) (Carolina Academic Press). Mr. Linton received his law degree from Loyola University of Chicago.

The Alabama Supreme Court recently held, in , that the parents of human embryos that were negligently destroyed at a fertility clinic could bring an action for damages under the State's wrongful death statute. Although the Alabama legislature promptly enacted a law essentially overturning the state supreme court's decision, concerns have been raised that the court's decision might influence courts in other States to interpret their wrongful death statutes, or possibly even their fetal homicide statutes, to apply in similar circumstances, thereby threatening the availability of fertilization (IVF) technology. This article addresses those concerns.

DEep LearnIng-based QuaNtification of epicardial adipose tissue predicts MACE in patients undergoing stress CMR.

Atherosclerosis

October 2024

Department of Perioperative Cardiology and Cardiovascular Imaging, Centro Cardiologico Monzino IRCCS, Milan, Italy; Department of Biomedical, Surgical and Dental Sciences, University of Milan, Milan, Italy. Electronic address:

Article Synopsis
  • This study evaluated the significance of epicardial adipose tissue (EAT) volume in predicting major cardiovascular events (MACE) in patients undergoing stress cardiac MRI.
  • A total of 730 patients were divided into two groups to develop and validate a risk assessment model that incorporates EAT volume alongside other factors like left ventricular ejection fraction and stress perfusion defects.
  • The results indicated that including EAT volume significantly improves the prediction of MACE, suggesting that automated measurements of EAT can enhance existing cardiac risk assessments.
