Sex Differences in Obstructive Sleep Apnea.

Clin Chest Med

September 2021

Harvard-Brigham and Women's Hospital, Fellowship in Pulmonary and Critical Care Medicine, Brigham and Women's Hospital, Harvard Medical School, 75 Francis Street PBB CA-3, Boston, MA 02115, USA. Electronic address:

Obstructive sleep apnea (OSA) for many years has been thought to be a disease of men, but research performed more recently has revealed women are at significant risk for OSA as well as the morbidity associated with leaving it untreated. There are estimates that up to 90% of women with severe sleep apnea are not being diagnosed and that if diagnosed, they are less likely to be treated. This article will explore the sex differences in OSA, specifically addressing areas of prevalence, phenotypes, diagnostic criteria, and treatment.

Background: Gastrointestinal bleeding (GIB) is a major concern in patients hospitalized with acute coronary syndrome (ACS) due to the common use of both antiplatelet medications and anticoagulants. Studies evaluating the safety of gastrointestinal endoscopy (GIE) in ACS patients with GIB are limited by their relatively small size, and the focus has generally been on upper GIB and esophago-gastroduod-enoscopy (EGD) only.

Aim: To evaluate the safety profile and the hospitalization outcomes of undergoing GIE in patients with ACS and concomitant GIB using the national database for hospitalized patients in the United States.

Adapting STEMI care for the COVID-19 pandemic: The case for low-risk STEMI triage and early discharge.

Catheter Cardiovasc Interv

April 2021

The Carl and Edyth Lindner Center for Research and Development, The Christ Hospital, Cincinnati, Ohio, USA.

The coronavirus pandemic has resulted in the need for rapid assessment of resource utilization within our hospital systems. Specifically, the overwhelming need for intensive care unit (ICU) beds within epicenters of the pandemic has created a need for consideration as to how acute coronary syndrome cases, and specifically ST-elevation myocardial infarction (STEMI) patients, are managed postprocedure. While most patients in the United States continue to be managed in coronary care units after primary percutaneous coronary intervention, there is a robust literature regarding the ability to triage STEMI patients safely and efficiently with low-risk features to non-ICU beds.

Division of Infectious Diseases, Department of Medicine, Loyola University Medical Center and Stritch School of Medicine, Maywood, Illinois.

Mucormycosis is an infection caused by a group of filamentous molds within the order Mucorales. Infections may result from ingestion of contaminated food, inhalation of spores into the nares or lungs, or inoculation into disrupted skin or wounds. In developed countries, mucormycosis occurs primarily in severely immunocompromised hosts (e.

Purpose: Probiotics may reduce risk of urinary tract infection by preventing colonization of uropathogens. We aimed to determine the change in the ratio between uropathogens:Lactobacillus (U/L) within the lower urinary tract in response to oral probiotic.

Methods: This was a double-blinded randomized controlled trial of healthy pre-menopausal female volunteers.

Background: Randomized trial data support the superiority of coronary artery bypass grafting (CABG) surgery over percutaneous coronary intervention (PCI) in diabetic patients with multivessel coronary artery disease (MV-CAD). However, whether this benefit is seen in a real-world population among subjects with stable ischemic heart disease (SIHD) and acute coronary syndromes (ACS) is unknown.

Objectives: The main objective of this study was to assess the generalizability of the FREEDOM (Future REvascularization Evaluation in Patients with Diabetes Mellitus: Optimal Management of Multi-vessel Disease) trial in real-world practice among patients with diabetes mellitus and MV-CAD in residents of British Columbia, Canada.

Challenges in the diagnosis and treatment of neurosarcoidosis.

Ann Med

September 2016

a Internal Medicine Unit , Guastalla Hospital, AUSL Reggio Emilia , Reggio Emilia , Italy.

The diagnosis and treatment of neurosarcoidosis can be very challenging for several reasons. It affects clinically 5%-10% of sarcoidosis patients, but can be found in up to 25% of autopsies. These data reveal that a high percentage of asymptomatic or misdiagnosed cases can be missed at an initial diagnostic approach.

Traumatic distal bronchial interruption is unusual. A 42-year-old male with history of blunt injury to his chest and abdomen 20 years previously presented with recurrent pneumonia of the right lung. Patient's middle and lower lobes were partially atelectatic and the right intermediate bronchus was found completely dehisced at the time of surgical exploration.

MIB-1 Proliferation Index in Ductal Carcinoma In Situ of the Breast: Relationship to the Expression of the Apoptosis-Regulating Proteins bcl-2 and p53.

Breast J

November 2000

Department of Pathology, Loyola University Medical Center and Stritch School of Medicine, Maywood, Illinois, and Department of Pathology, Beth Israel Deaconess Medical Center and Harvard Medical School, Boston, Massachusetts.

Tumor growth is the net result of cell proliferation and cell death. To investigate the relationship between these phenomena in ductal carcinoma in situ (DCIS), we studied proliferation index (PI) in DCIS in relation to the expression of two proteins involved in the regulation of cell death, bcl-2 and p53. Thirty-nine consecutive cases of DCIS were studied.

Detection and Significance of Occult Axillary Metastatic Disease in Breast Cancer Patients.

Breast J

July 1999

Department of Pathology, Loyola University Medical Center and Stritch School of Medicine, Maywood, Illinois; Department of Pathology, Beth Israel Deaconess Medical Center and Harvard Medical School, Boston, Massachusetts; Breast Cancer Program, Lombardi Cancer Center, Georgetown University Medical Center, Washington, DC.

After clinical staging, the single most important prognostic factor for patients with newly diagnosed primary breast cancer is the presence or absence of detectable metastases to axillary lymph nodes when examined by conventional light microscopy. More sensitive methods of determination of lymph node status, such as evaluation of serial sections, immunohistochemical staining, and use of molecular biological assays increase the rate of detection of micrometastases. Although the feasibility of enhanced detection of occult axillary metastatic disease is well established, the prognostic significance of such detection is only recently starting to emerge.

Background: Axillary lymph node dissection for evaluation of the presence or absence of metastatic disease is the single most important prognostic factor for patients with newly diagnosed primary breast cancer. Recently, sentinel lymph node (SLN) biopsy is being investigated as an alternative to the evaluation of the entire axilla. We evaluated whether the application of multilevel sectioning and immunohistochemistry in SLNs will increase the accuracy of detection of metastatic deposits.
