Urothelial carcinoma may occur anywhere in the urinary tract including the pendulous urethra. To prevent urethral stricture after resection and monopolor fulguration we describe the use of the holmium laser to fulgurate recurrent pTa UC from the urethra. The surgical approach was staged and provided excellent long term results for management of superficial UC.

Chronic postoperative pain has been associated with mesh repair in meta-analysis of clinical trials. We compared the incidence of early complications, recurrence, and chronic pain syndrome in anatomic and mesh repairs in 200 patients. We defined chronic pain syndrome as pain in the inguinal area more than 3 months after inguinal hernia repair, patient referral to pain management, or necessity of a secondary procedure for pain control.

A small pool of estrogen receptors (ERalpha and -beta) localize at the plasma membrane and rapidly signal to affect cellular physiology. Although nuclear ERs function mainly as homodimers, it is unknown whether membrane-localized ER exists or functions with similar requirements. We report that the endogenous ER isoforms at the plasma membrane of breast cancer or endothelial cells exist predominantly as homodimers in the presence of 17beta-estradiol (E2).

Central serotonin (5-hydroxytryptamine, 5-HT) function has a role in a range of genetically influenced psychiatric diagnoses and behaviors. Several human 5-HT receptor polymorphisms are 'candidate alleles', altering in vitro function, and potentially affecting behavior and drug response. The 5-HT(2A) His452Tyr polymorphism alters signal transduction, and has been associated with diminished efficacy of clozapine in schizophrenia.

Transforming growth factor-beta1 (TGF-beta1) is a growth factor that is implicated in fibrosis of many organs. Interactions between the estrogen receptor (ER) and the epidermal growth factor receptor (EGFR) contribute to the biological effects of these binding protein families. EGFR stimulates DNA synthesis and gene transcription in the uterus, related in part to estrogen-independent activation of the nuclear ER. This results from signal transduction enacted by the plasma membrane tyrosine kinase growth factor receptor, leading to 1) phosphorylation and activation of the nuclear ER, and 2) phosphorylation of coregulator proteins.

We hypothesized that the response of the genioglossus to negative pressure during wakefulness should be intact in obstructive sleep apnea (OSA) patients despite published evidence showing impairment of the response of palatal muscles (Mortimore IL and Douglas NJ. The objective of this retrospective, consecutive, case series design study was to determine the number of unselected patients with obstructive sleep apnea (OSA) who deteriorated after uvulopalatopharyngoplasty (UPPP). Sixteen of 27 patients at the Sleep Clinic at Veterans Affairs Medical Center who underwent UPPP for OSA and who completed both a pre- and postpolysomnogram were studied. After comparing the apnea-hypopnea index (AHI) before and after UPPP, three groups of patients were identified: deteriorators, unchanged, and improvers.

Efficacious surgical treatment of anterior glenohumeral instability often requires repair of the anteroinferior capsulolabral structures, including the glenoid origin of the anterior band of the inferior glenohumeral ligament. Rupture in this location, the Bankart lesion, may be accompanied by nonrecoverable stretching of the anterior band. Certain brain-computer interface (BCI) methods use intrinsic signals from the motor cortex to control neuroprosthetic devices. The organization of the motor pathways in those populations likely to use neuroprosthetic devices, therefore, needs to be determined; there is evidence that following disease or injury the representation of the body in the motor cortex may change. In this study, functional MRI measures of somatotopy following spinal cord injury (SCI) showed evidence of changes in limb representations in the motor cortex.

We report a rare complication related to the insertion of Kirschner wires for stabilization of an acromioclavicular separation. Five years after placement of the Kirschner wires, the patient presented with hemoptysis. On review of chest radiographs, a fractured wire was found to have migrated from the acromioclavicular joint, through the hemithorax and into the trachea.

Five human anatomic specimen knees were used to determine the effect of patellar component position on patellofemoral kinematics, contact pressures, and contact areas after total knee arthroplasty using a polyethylene, domed patellar component. Each patellar component was positioned at the anatomic center of the resected patellar surface and then repositioned 5 mm proximally, distally, medially, and laterally. Patellar tilt was greatest with medial positioning of the patellar component and least with central and lateral positioning.

We evaluated the effect of simulated scapular protraction on anterior glenohumeral translation and in situ inferior glenohumeral ligament strain. Five fresh-frozen cadaveric glenohumeral joints were tested in the position of apprehension while simulating scapular protraction using a custom translation testing device and a differential variable reluctance transducer. The results showed a statistically significant decrease in anterior glenohumeral translation as the scapula was increasingly rotated to simulate 20 degrees of scapular protraction.

Objective: To determine rates of survival, long-term patency, and recurrent variceal hemorrhage among patients with alcoholic cirrhosis treated by partial portacaval shunt.
Design: Single-institution cohort follow-up study of 72 consecutive patients who underwent small-diameter portacaval H-graft shunt with collateral ablation during a 10-year period (1981 through 1990). Subjects were enrolled and followed up for up to 15 years.

Patellofemoral joint kinematics, contact areas, and contact pressures were measured concomitantly before and after total knee arthroplasty in 10 fresh frozen human cadaver knees using an Instron machine, a custom patellofemoral joint testing jig, axial bone markers, a continuous video digitizing system, and Fuji pressure sensitive film. The implant used in this study was the Kirschner Performance Knee System with an all polyethylene, domed patellar component.
