Clinical and radiographic outcomes after subduro-peritoneal shunt insertion in adults.

Clin Neurol Neurosurg

December 2024

Department of Neurosurgery, Charité - University Hospital Berlin, Charité Campus Benjamin Franklin, Hindenburgdamm 30, Berlin 12203, Germany.

Background: Subdural collections consist amongst others of cerebrospinal, inflammatory, haemorrhagic or infective fluid. While these accumulations can be treated with conservative or surgical measures, such as burr hole evacuation or craniotomy, clinicians may resort to implantation of a subduro-peritoneal shunt, due to their high recurrence rates. While this treatment option is widely used in the pediatric population, its efficacy and safety in adults is scarcely reported.

Adherence to the Mediterranean diet (MeD) has been associated with a reduced incidence of peripheral arterial disease (PAD)/lower extremity arterial disease (LEAD) in observational trials and in a randomized trial. In secondary prevention, a lower hazard ratio for composite major adverse cardiovascular events has been associated with better adherence to MeD in a relatively small, randomized trial. This has not been confirmed in a sub-analysis of a large interventional trial of dual antithrombotic treatment.

Outcomes of Intraoperative Frozen Section Examination of Surgical Resection Margins of the Penis in Penile Cancer.

Clin Genitourin Cancer

October 2024

Institute of Andrology, University College London Hospitals NHS Foundation Trust, London, UK; Division of Surgery and Interventional Science, University College London, London, UK. Electronic address:

Article Synopsis
  • Frozen section examination (FSE) of tumor margins during penile-preserving surgery (PPS) is crucial for determining how much tissue to excise in penile cancer patients.
  • A study analyzed 137 cases of penile squamous cell carcinoma (SCC) using FSE and found a sensitivity of 66.7% and specificity of 100%, with 86.1% showing negative margins.
  • The results suggest that FSE is a reliable tool for assessing margins in PPS, helping reduce unnecessary tissue removal, although the study's retrospective nature poses some limitations.

Article Synopsis
  • Catheter ablation (PVI) is an effective treatment for atrial fibrillation (AF), but it has a relapse rate of 15-50%, especially in hypertensive patients.
  • This meta-analysis included 637 patients from 8 trials, showing that adding renal denervation (RDN) to PVI significantly reduces AF recurrence and lowers blood pressure.
  • The combination of RDN and PVI does not negatively impact kidney function, suggesting RDN is a valuable adjunctive treatment for AF in patients with resistant hypertension.

Article Synopsis
  • - The study focuses on the impact of Hepatocyte Nuclear Factor 1-beta (HNF1B) gene variants and chromosome 17q12 deletion (17q12del) on kidney disease progression, particularly chronic kidney disease (CKD), in a large cohort of 521 patients.
  • - Findings reveal that patients with the 17q12del experience a significant delay in the progression to CKD stage 3 compared to those with other HNF1B variants, with specific mutations in the DNA-binding domains correlating with even better outcomes.
  • - Additionally, the 17q12del is linked to lower magnesium levels (hypomagnesemia) and higher likelihood of elevated uric acid levels (hyperuric

Non-operative management of metacarpal II-IV fractures: A retrospective study from a tertiary hand unit.

J Plast Reconstr Aesthet Surg

October 2024

University College London Medical School, 74 Huntley St, London WC1E 6DE, United Kingdom; Department of Plastic and Reconstructive Surgery and Hand Therapy Unit, Royal Free Hospital, Pond St, London NW3 2QG, United Kingdom.

Metacarpal fractures produce significant hand disability, and non-operative management of these fractures can produce satisfactory functional outcomes with few complications. However, most studies assessing non-operative outcomes of metacarpal fractures revolves around metacarpals I and V, which possess different anatomy. Therefore, further investigation into outcomes after non-operative treatment of metacarpals II-IV is required to inform management decisions and allow modification of the rehabilitation protocols to specific fracture patterns.

Background: Gestational diabetes mellitus (GDM) is a known risk factor for stillbirth (Rosenstein et al., 2012) [1]. Delayed villous maturation (DVM), predominantly seen in term placentas in pregnancies complicated with glucose dysmetabolism, may in part be a consequence of excessive maternal glucose leading to release of fetal insulin and other growth factors that promote excessive placental growth at the expense of villous maturation (Redline, 2012) [2].

Transthoracic echocardiography (TTE) is the most commonly used imaging modality to diagnose left ventricular thrombus (LVT), however, cardiac magnetic resonance (CMR) remains the gold standard investigation. A comparison of the diagnostic performance between two modalities is needed to inform guidelines on a diagnostic approach towards LVT. We performed a systematic review and meta-analysis to investigate the diagnostic performance of three methods of TTE (non-contrast, contrast, and apical wall motion scoring) for the detection of LVT compared to CMR as a reference test.
