Should dry spinal tuberculosis be managed differently than wet spinal tuberculosis?

Background Context: On radiopathological examination of spinal tuberculosis (TB), two predominant forms are known: dry and wet types. Wet TB, as the name suggests, has abscess formation as its predominant presenting feature and is the exudative form; dry TB includes caseation and sequestration with minimal exudate. Dry TB often exhibits poorer recovery patterns than the wet counterparts, which can be possibly ascribed to vasculitis, ischemia, or tubercular myelitis, rather than isolated mechanical compression.

A man in his early 50s presented to the emergency department (ED) with sudden onset of palpitation and presyncope. The 12-lead electrocardiogram (ECG) recorded in ED showed monomorphic ventricular tachycardia requiring cardioversion in view of haemodynamic instability. The patient was subsequently detected to have an anomalous left coronary artery origin from the pulmonary artery.

Minimal Access Retromandibular Approach to Fractures of Vertical Segment of the Mandible: A Versatile Technique.

 Fractures of the condylar region of the mandible, although fairly common, also generate the most debate regarding management-both closed and open treatment options have been recommended and shown to yield good results. We present our experience with a minimal access retromandibular approach to fractures of the vertical mandible.  This is a retrospective study of all patients who underwent open reduction and internal fixation for fractures of the vertical mandible (condyle, subcondylar region, ramus) using a retromandibular approach at a tertiary care hospital in a metropolitan city in India between January 2022 and July 2023.

Background Young-onset type 2 diabetes mellitus (T2DM), defined as a diagnosis before the age of 45, is an increasingly common and aggressive form of diabetes. This population is at a heightened risk of developing complications earlier in life due to longer disease duration and often suboptimal glycemic control. Complications such as diabetic neuropathy, retinopathy, and nephropathy are significant concerns, leading to reduced quality of life and increased morbidity.

Interesting Case of Anterior Communicating Artery Aneurysm Turns Out to Be a Ruptured Fenestrated A1 Segment Aneurysm With Concomitant Multiple Aneurysms: A Case Report and Literature Review.

The luminal separation of the vessel into two distinct, parallel channels that reunite distantly is known as fenestration. There is a correlation between the development of an aneurysm and the proximal portion of fenestration, although fenestrations are typically regarded as a variant of normal anatomy. We report an interesting case of an anterior communicating artery (a-comm) aneurysm, which turns out to be a ruptured fenestrated A1 segment aneurysm after digital subtraction angiography with concomitant multiple aneurysms treated by clipping along with case report and literature review.

Background: Feedback on the diagnostic process has been proposed as a method of improving clinical reasoning and reducing diagnostic errors. Barriers to the delivery and receipt of feedback include time constraints and negative reactions. Given the shift toward asynchronous, digital communication, it is possible that electronic feedback ("e-feedback") could overcome these barriers.

Presumed Perinatal Ischemic Left Middle Cerebral Artery Stroke With Cerebral Palsy, Developmental Delay, and Epilepsy: A Case Report.

The perinatal period is a high risk for ischemic events to occur leading to lifelong morbidity. Various patterns of ischemic injury to the fetal and neonatal brain have been studied depending on gestational age as well as the degree of hypoxia/ischemia. We present a case of presumed perinatal ischemic left middle cerebral artery stroke diagnosed by magnetic resonance imaging (MRI) in a child with global developmental delay, cerebral palsy, and epilepsy.

Aspergilloma Superinfection in the Spinal Canal of an 18-Year-Old Man: A Case Report.

Case: An 18-year-old immunocompetent man presented with symptomatic lumbar canal stenosis, discharging sinuses, and cold abscess for 3 years treated with multiple incision drainage procedures with cottonoid packing antibiotic therapy. Radiographic imaging showed a soft tissue mass in the canal causing bony destruction. Postoperative histopathological examination showed an Aspergillus fungal ball.

Introduction: Adverse drug reactions (ADRs), including cutaneous adverse drug reactions (CADRs), are significant global health concerns with India among the top affected countries. CADRs represent a significant concern in healthcare, impacting the skin, its appendages and mucous membranes ranging from mild rashes to severe, life-threatening conditions like Stevens-Johnson syndrome and toxic epidermal necrolysis. Self-medication with over-the-counter drugs is a notable public health issue linked to CADRs.

Utility of HACOR Score in Patients with Acute Exacerbation of Post-tuberculosis Obstructive Airway Disease: A Retrospective Observational Study.

Objective: In this study, we tried to analyze the utility of the HACOR score in the acute exacerbation of post-tuberculosis obstructive airway disease (post-TB-OAD).

Materials And Methods: The HACOR score for patients in acute exacerbation of post-TB-OAD who needed noninvasive ventilation (NIV) support was calculated at 1, 12, 24, and 48 hours. The history of NIV success or failure was noted.

Background And Objectives: Indian ethnic and educational diversities necessitate obtaining normative cognitive data in different populations. We aimed to evaluate cognitive scores using a Marathi translation of the Kolkata cognitive battery (KCB) and to study the association of KCB components with depression and sociodemographic variables.

Budd-Chiari syndrome associated with congenital afibrinogenaemia reversed after orthotopic liver transplant.

A woman in her mid-20s, a known case of congenital afibrinogenaemia, presented with abdominal pain and distension. She was diagnosed with decompensated liver cirrhosis due to Budd-Chiari syndrome. She underwent deceased donor liver transplantation.

Introduction: The coexistence of insomnia and obstructive sleep apnea (OSA) poses a complex and challenging clinical scenario, commonly referred to as comorbid insomnia and OSA (COMISA). The bidirectional relationship between these two sleep disorders is explored, illuminating how the presence of one can exacerbate the severity and manifestations of the other. We under took this study to understand the prevalence of COMISA in Indians which is never studied.

Hemophagocytic Lymphohistiocytosis: A Rare Complication of Acute Hepatitis E Infection.

Hemophagocytic lymphohistiocytosis (HLH) is an aggressive hematological disorder caused by uncontrolled activation of cytotoxic T-cells (CTL), natural killer (NK) cells, and macrophages leading to hyperinflammation and cytokine storm. The clinical course is characterized by high-grade fever, cytopenia, and multiorgan dysfunction. HLH is classified as either primary/familial or secondary, the latter being most often triggered by infections, malignancies, and autoimmune disorders.
