Schwannoma is a benign tumor of the peripheral nerve sheath and is a unique clinical entity when localized to a lower limb. Growing as a painless nodule, it might be misdiagnosed by many medical professionals as another benign soft tissue skin condition, such as lipoma, myxoma, or ganglion cyst. Definitive diagnosis of peripheral schwannoma is made by biopsy and histopathologic evaluation, followed by surgical excision, which is the definitive treatment of the tumor.

A 71-year-old man who had hip abductor insufficiency due to a chronic injury to the right superior gluteal nerve injury after lipoma resection presented to our outpatient clinic 1.5 years postoperatively with persistent pain, atrophy of the gluteus medius muscle, and Trendelenburg sign with a corresponding limp. A magnetic resonance imaging scan and neurophysiological diagnostics confirmed a chronic lesion of the superior gluteal nerve with completed reinnervation and absent pathological spontaneous activity, excluding neurosurgical options to restore hip abduction.

Intravenous lipomas (IVLs) of the head and neck are uncommon benign tumors that develop within the venous walls, often detected incidentally during imaging for unrelated issues. While usually asymptomatic, these IVLs can cause congestive venous symptoms like swelling, paresthesia or pain in the head and neck and upper limbs, or even venous thromboembolism. The precise diagnosis of IVLs is predominantly achieved through computed tomography (CT) and magnetic resonance imaging (MRI), with CT being the most frequently used method.

Tracheal lipomas, though exceedingly rare among benign tracheal masses, present unique diagnostic and management challenges due to their unusual clinical course. This case report documents the presentation and surgical management of a 56-year-old male with a tracheal lipoma, a first documented case in Puerto Rico. The patient presented with progressive dyspnea and stridor, initially misdiagnosed and treated as asthma exacerbations.

Their electronic health records were reviewed for demographics, as well as neurosurgical, urologic, rehabilitation, and orthopedic interventions.

Lipomas, the most common soft-tissue mesenchymal neoplasms in adults, are characterized by the proliferation of mature white adipocytes without cytologic atypia. Lipomas are rarely observed in the head and neck region. Lipomatous hypertrophy of the interatrial septum (LHIS) is a rare but benign cardiac tumor that can be found on cardiac imaging such as echocardiogram, or during surgery or an autopsy. Cardiac MRI is the best imaging modality to determine the borders of the tumor and its extension into the intraventricular septum and ventricular free wall. Patients require close monitoring because LHIS may cause right or left outflow tract obstruction or superior vena cava obstruction, requiring cardiac surgical intervention.

Lipomas are superficial tumors that occur primarily in the subcutaneous region and very rarely occur deeply within or between the muscle layers.
