Traditional postero-medial ankle approach for Bartonícek type III in Volkmann Fractures: Is it useful?

Foot Ankle Surg

December 2024

Faculty of Medicine, University of Barcelona, Spain; Clínica Nostra Senyora del Remei, Barcelona, Spain; Unitat d`Anatomia i Embriologia Humana, Spain.

Introduction: There is evidence that the presence of a posterior malleolus fracture (PMF) worsens the prognosis of ankle fractures, making conservative treatment a poor choice. PMFs include a heterogeneous group of bone injury patterns that are sometimes associated to medial malleolus extensions, as well as fibula fractures or syndesmotic damage. This requires the surgeon to be well-versed in anatomy to choose the appropriate surgical approach.

Exp Clin Transplant

September 2024

>From the Nephrology, Dialysis and Transplantation Service, Las Higueras Hospital, Talcahuano, Chile; and the Departamento de Medicina Interna, Facultad de Medicina, Universidad de Concepción, Concepción, Chile.

Article Synopsis
  • Anticoagulant-related nephropathy is a condition where excessive anticoagulant levels lead to blood in urine and can result in acute kidney injury and possibly chronic kidney disease.
  • The case study highlights a 63-year-old patient who developed kidney problems after starting anticoagulant medication two months post-kidney transplant.
  • The patient experienced graft dysfunction alongside elevated international normalized ratio (INR) levels, indicating a potential overdose of the anticoagulant.

Arthroscopic anatomic lateral ligament reconstruction of the ankle joint has proven to be a safe option in the treatment of chronic ankle instability (CAI), with good functional results as well as allowing simultaneous management of associated lesions. We described an arthroscopic technique for anatomic reconstruction of the anterior talofibular ligament and calcaneofibular ligament using only 2 arthroscopic portals. This surgical technique to treat CAI is technically less demanding than other described techniques that use 3 or 4 arthroscopic portals.

Tissue regeneration is often impaired in patients with metabolic disorders such as diabetes mellitus and obesity, exhibiting reduced wound repair and limited regeneration capacity. We and others have demonstrated that wound healing under normal metabolic conditions is potentiated by the secretome of human endothelial cell-differentiated mesenchymal stem cells (hMSC-EC). However, it is unknown whether this effect is sustained under hyperglycemic conditions.

A key component of treatment for all people with advanced kidney disease is supportive care, which aims to improve quality of life and can be provided alongside therapies intended to prolong life, such as dialysis. This article addresses the key considerations of supportive care as part of integrated end-stage kidney disease care, with particular attention paid to programs in low- and middle-income countries. Supportive care should be an integrated component of care for patients with advanced chronic kidney disease, patients receiving kidney replacement therapy (KRT), and patients receiving non-KRT conservative care.

Prevention and early detection of kidney diseases in adults and children should be a priority for any government health department. This is particularly pertinent in the low-middle-income countries, mostly in Asia, Africa, Latin America, and the Caribbean, where up to 7 million people die because of lack of end-stage kidney disease treatment. The nephrology workforce (nurses, technicians, and doctors) is limited in these countries and expanding the size and expertise of the workforce is essential to permit expansion of treatment for both chronic kidney disease and end-stage kidney disease.
