Cerebral palsy (CP) often results in severe hip issues, disrupting musculoskeletal development and mobility due to problems such as dislocations and contractures, aggravated by spasticity and heightened muscular tone. While total hip arthroplasty (THA) is required in CP patients, the procedure carries high risks due to concerns about dislocation and wear. This study explores a method of intraoperative navigation to precisely execute preoperative strategies for spinopelvic alignment and optimal cup placement.

Purpose: Currently, various techniques are available to mark and selectively remove initially suspicious axillary lymph nodes (target lymph nodes, TLNs) in breast cancer patients receiving neoadjuvant chemotherapy (NACT). To date, limited data are available on whether the use of magnetic seeds (MS) is suitable for localizing TLNs. This study aimed to investigate the feasibility of MS in patients undergoing target lymph node biopsy (TLNB) or targeted axillary dissection (TAD) after NACT.

Silicone implants or tissue expanders placed under the pectoralis major (PM) muscle are often used for breast reconstruction. However, the disruption of PM insertions, which is often an inevitable part of the surgical procedure, is known to cause PM morbidity and, subsequently, problems with the use of the ipsilateral arm. In this systematic review, we present current knowledge regarding the effect of submuscular silicone-based breast reconstruction on the function of PM and the ipsilateral arm.

The syndrome of inappropriate antidiuresis (SIAD), the commonest cause of hyponatraemia, is associated with significant morbidity and mortality. Tolvaptan, an oral vasopressin V2-receptor antagonist, leads through aquaresis to an increase in serum sodium concentration and is the only medication licenced in Europe for the treatment of euvolaemic hyponatraemia. Randomised controlled trials have shown that tolvaptan is highly efficacious in correcting SIAD-related hyponatraemia.

Ketone bodies and the heart.

Arch Med Sci Atheroscler Dis

December 2021

Christeas Hall Research Laboratory, Medical School, National and Kapodistrian University of Athens, Athens, Greece.

Ketone bodies are low chain organic substances with four carbon atoms, with β-hydroxybutyric acid and acetone being the main ketone bodies in blood circulation. Under physiological conditions their levels are low while during conditions of oxidative stress, such as exercise, fasting state and acute illness, ketone body levels are increased. Recent findings have shown that in patients with heart failure their plasma concentration is increased.

False Negativity of Targeted Axillary Dissection in Breast Cancer.

Breast Care (Basel)

October 2021

1st Department of Surgery, National and Kapodistrian University of Athens, "Laiko" University Hospital, Athens, Greece.

Article Synopsis
  • Targeted axillary dissection (TAD) offers a potential alternative for staging node-positive breast cancer patients after neoadjuvant chemotherapy, but concerns remain regarding its false-negative rate (FNR) and the risk of underestimating remaining disease.
  • Studies indicate that the FNR of TAD is typically under 10%, though this figure is based on small sample sizes and lower N status tends to correlate with lower FNR.
  • Factors improving TAD reliability include identifying sentinel lymph nodes during surgery, and while various methods for marking positive nodes exist, no clear superiority has been established; practical considerations like cost and local regulations must guide choice of technique.
