Background: Collateral status (CS) plays a crucial role in infarct growth rate, risk of postthrombectomy hemorrhage, and overall clinical outcomes in patients with acute ischemic stroke (AIS) secondary to anterior circulation large-vessel occlusions (LVOs). Hypoperfusion intensity ratio has been previously validated as an indirect noninvasive pretreatment imaging biomarker of CS. In addition to imaging, derangements in admission laboratory findings can also influence outcomes in patients with AIS-LVO.

Article Synopsis
  • Acute ischemic stroke (AIS) patients' collateral status (CS) is a crucial factor in predicting favorable outcomes, with relative cerebral blood flow (rCBF) being a key measurement for evaluating CS.
  • A study analyzing 221 AIS patients found that factors like male gender, chronic kidney disease, higher stroke severity (NIH Stroke Scale score ≥12), and low systolic blood pressure (<140) are linked to poor collateral status (rCBF <38%).
  • The findings emphasize the importance of these baseline characteristics and lab values in determining the severity of collateral blood flow and potential patient outcomes in AIS due to large vessel occlusion.

Longitudinal echocardiography in pediatric patients with hypermobile Ehlers-Danlos syndrome.

Am J Med Genet A

December 2024

Division of Genetic and Genomic Medicine, Center for Cardiovascular Research, Nationwide Children's Hospital, Columbus, Ohio, USA.

Vascular Ehlers-Danlos, Marfan and Loeys-Dietz syndromes have increased risk of aortic dilation and dissection. Previous early studies showed hypermobile Ehlers-Danlos syndrome (hEDS) may also have increased risk, with echocardiography screening recommended; subsequent studies have not confirmed the risk or recommended echocardiography. This pediatric-based study assessed aortic dilation prevalence in those with hEDS by serial echocardiographic examinations and assessed family history for aortic dissections.

Dual Channel Endoscopic Mucosal Resection.

Clin Colon Rectal Surg

September 2024

Division of Colon and Rectal Surgery, Lahey Hospital and Medical Center, Burlington, Massachusetts.

Endoscopic mucosal resection (EMR) is the recommended technique for colon polypectomy for nonpedunculated lesions that are >20 mm in size not requiring excision. Dual-channel EMR (DC-EMR) uses an endoscope with two working channels to facilitate easier submucosal injection, snare resection, and clip closure of polypectomy defects. There is also promising early literature indicating that this endoscopic modality can reduce the overall learning curve present for single-channel colonoscopy EMR.
