This case report describes a 10-year-old spayed female domestic medium-hair cat with chronic upper respiratory signs related to a remnant of the left globe in the orbit after incomplete enucleation, causing a draining tract. A CT scan, complete blood (cell) count, biochemical analysis, rhinoscopy, biopsy, culture, histopathologic analysis, and surgical revision of enucleation were done. Complete blood (cell) count was unremarkable.

Aims: Although molecular tests developed for a growing list of oncogenic alterations have significantly aided in the classification of head and neck carcinomas, tumours in which prototypical histologic and immunophenotypic features are lacking or only partially developed continue to pose diagnostic challenges. Searching for known diagnostic and therapeutic targets by clinical next-generation sequencing (NGS) assays can often lead to new discoveries.

Methods And Results: We present our institutional experience in applying targeted RNA NGS in 36 head and neck carcinomas that were morphologically difficult to classify between 2016 and 2023.

Article Synopsis
  • * Genomic profiling of ACC shows low tumor mutational burden, a lack of actionable targets, and negative PD-L1, leading to recommendations for a specific combination of anti-PD-1 and anti-CTLA-4 immunotherapy.
  • * A successful complete response was noted in a patient with treatment-resistant locally recurrent lacrimal gland ACC through personalized immunotherapy combined with other therapeutic strategies, highlighting the importance of considering patient-specific factors in immune-cold cancers.

Non-invasive miRNAs for early detection and diagnosis of lacrimal adenoid cystic carcinoma.

Discov Oncol

October 2024

Department of Ophthalmology, Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine, No.639 Zhizaoju Road, Shanghai, 200025, People's Republic of China.

Lacrimal adenoid cystic carcinoma (LACC) is one of the most common malignant epithelial tumors of the lacrimal gland, characterized by high rates of local recurrence, distant metastases, and tumor-related mortality. This malignancy impairs the lacrimal gland's ability to secret tears, thereby disrupting the normal function of the ocular surface. Due to ineffective diagnostic approaches and the complex anatomical location of the lacrimal gland within the orbit, many LACC patients are often diagnosed at the later stages.

Aim: To evaluate the outcomes of eye-sparing surgery for lacrimal gland adenoid cystic carcinoma and the impact on tumor recurrence and orbital integrity.

Methods: The study enrolled four patients with recurrent lacrimal gland adenoid cystic carcinoma. The outcome focused on the relevance of the integrity of the lateral orbital wall to the occurrence of extraorbital metastasis in the local recurrence of lacrimal gland adenoid cystic carcinoma.

Article Synopsis
  • Surgical excision of advanced lacrimal gland carcinoma often requires removing the orbital contents and reconstructing the anterior skull base with tissue that can withstand radiotherapy; this process involves complex considerations regarding patient quality of life and aesthetics.
  • In three cases, a rectus abdominis flap was utilized for reconstruction after en bloc resection, chosen for its reliable structure and blood flow to fill the surgical cavity.
  • The flap successfully survived with no complications, but showed notable shrinkage after six months, leading to the recommendation for using a slightly larger flap volume initially to account for future reductions.

Article Synopsis
  • Immunoglobulin-G4 (IgG4)-related disease is a rare fibro-inflammatory condition that can impact multiple organs simultaneously or at different times, often presenting with tumor-like organ growth.
  • Key affected organs include the lacrimal glands, salivary glands, pancreas, and more, with distinctive histopathological features like dense IgG4-positive plasma cells and storiform fibrosis.
  • The disease has an estimated incidence of less than 1 in 100,000 people globally, and a specific case of IgG4-related vasculitis was diagnosed through lung fine needle aspiration biopsy, highlighting its rarity in medical literature.

Article Synopsis
  • - The study aimed to assess how diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) can differentiate between inflammation and malignant lymphoma of the lacrimal gland.
  • - Researchers analyzed 28 patients over a 10-year period, finding that 61% had inflammation (dacryoadenitis) while 21% had lymphoma, with ADC values being significantly lower for lymphoma cases compared to inflammation.
  • - The results indicate that DWI can be a valuable imaging tool to help distinguish between these two conditions in the lacrimal gland.

Article Synopsis
  • Patients with immunodeficiency are at a higher risk of developing certain cancers, particularly mature lymphoid neoplasms and lymphoproliferative disorders.
  • A 50-year-old woman on immunosuppressive therapy for dermatomyositis presented with swollen lacrimal glands, weight loss, and night sweats, leading to an elective biopsy.
  • Post-surgery, she experienced acute abdominal issues that revealed multiple organ lesions and ultimately led to a diagnosis of polymorphic B-lymphoproliferative disorder resembling lymphomatoid granulomatosis, a rare condition linked to immunosuppression.

  • - The cyst was found on the medial side of the eyelid, causing ectropion but showing no signs of inflammation; surgical incision and drainage reduced its size.
  • - Imaging studies indicated cystic dilation and obstruction in the canaliculus, highlighting that canalicular cysts should be considered when similar patients present with masses in the eye area.

Respiratory epithelial cysts are uncommon lesions that can be congenital or acquired cystic malformations. Beyond the respiratory system, the location of the respiratory epithelial cyst has been described in the central nervous system, orbits, and the maxillofacial region. The common etiology is believed to be sequestration or entrapment of the epithelial cells of the paranasal sinus.
