Managing Patients with Head and Neck Cancer and Advanced Age or Comorbidities.

Semin Radiat Oncol

April 2025

Department of Radiation Medicine & Applied Sciences, University of California San Diego, La Jolla, CA; Gleiberman Head and Neck Cancer Center, La Jolla, CA. Electronic address:

The dominant treatment paradigm for locoregionally advanced head and neck squamous cell carcinoma (HNSCC) involves postoperative or definitive radiotherapy with concurrent cisplatin chemotherapy. Despite years of research investigating de-intensified treatment, cisplatin-based chemoradiotherapy remains the standard, yet it is associated with significant acute and chronic toxicity. However, due to shared risk factors, such as advanced age, and tobacco and alcohol use, patients with HNSCC frequently have comorbid illnesses that impact treatment tolerability, adding complexity to treatment-related decision-making.

High-density lipoproteins (HDLs) are heterogeneous particles with pleiotropic functions including anti-inflammatory and anti-infectious effects. In clinical studies, lower HDL-associated cholesterol (HDL-C) concentration has been associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, severity and mortality. A reduction in the number of HDL particles, particularly small ones has been observed with alterations in their protein and lipid composition impairing their functions.

In France, the practice of laboratory medicine has, over the past ten years or so, met with a certain amount of disenchantment from medical students. Despite its intricacy and importance to healthcare, the world of the clinical laboratory lacks visibility in medical studies. While other phenomena can explain this disenchantment, our belief in the importance of promoting greater visibility of laboratory medicine in medical studies led us to examine for this article the images and symbols associated with the profession, based on a semiotic analysis.
