Different prognostic effect of lymph node metastasis between remnant gastric cancers and primary proximal gastric cancers.

J Gastrointest Surg

October 2024

Department of Digestive Surgery (Gastric and Esophageal Surgery Division), Kyoto Prefectural University of Medicine, Kawaramachihirokoji, Kamigyo-ku, Kyoto-shi, Kyoto, Japan.

Background: Although the dissected lymph node number in remnant gastric cancer (RGC) may be smaller than in primary proximal gastric cancer (PGC), altered lymphatic flow provides different metastatic patterns in lymph nodes, which could potentially give rise to prognostic differences between RGC and PGC with nodal metastasis.

Methods: Between 1993 and 2020, 2546 consecutive patients with gastric cancer underwent gastrectomy. Of these, 53 patients with RGC and 381 patients with PGC with pathologic TNM stage I-III gastric cancer underwent curative gastrectomy.

Article Synopsis
  • The study evaluated the effectiveness of preoperative pancreatic juice cytology (PJC) in diagnosing intraductal papillary mucinous neoplasm (IPMN) of the pancreas to inform treatment strategies.
  • Out of 1,130 patients, 852 underwent PJC, showing an overall accuracy of 55% for distinguishing cancerous from noncancerous lesions, with varying rates based on IPMN type.
  • Despite low sensitivity, the high positive predictive value suggests PJC may still be useful in uncertain surgical cases but is generally not recommended as a reliable diagnostic tool.

Article Synopsis
  • - The study evaluated the effectiveness of texture and color enhancement imaging (TXI) compared to narrow band imaging (NBI) for detecting polyps in the right side of the colon during colonoscopy in patients aged 40 and above, involving 381 participants across three institutions.
  • - Researchers conducted a 30-second additional observation (Add-30-s) using either TXI or NBI after initial viewing with white light to compare detection rates of adenomas and sessile serrated lesions.
  • - Results showed that TXI was noninferior to NBI, with similar detection rates of polyps between both methods, indicating that TXI can be a viable alternative to NBI for polyp detection in colon examinations.

Background/objectives: The detection of malignancy is a major concern in the management of intraductal papillary mucinous neoplasm (IPMN). The height of the mural nodule (MN), estimated using endoscopic ultrasound (EUS) and computed tomography (CT), has been considered crucial for predicting malignant IPMN. Currently, whether surveillance using CT or EUS alone is sufficient for detecting MNs remains unclear.
