The ratio of serum acylcarnitines to free carnitines (AC/FC) reflects impaired cardiomyocyte β-oxidization. The effect of heart failure (HF) treatment on AC/FC remained unclear. This pilot study retrospectively examined treatment-induced AC/FC changes in patients with HF with preserved ejection fraction (HFpEF) and HF with reduced ejection fraction (HFrEF) in 148 consecutive inpatients undergoing echocardiography and carnitine measurement at admission and discharge.

Background: This study aimed to compare the efficacy and safety of baricitinib in patients with rheumatoid arthritis (RA) receiving different doses based on renal function.
Methods: We conducted a retrospective study within the JAK Study Group, involving 23 facilities in Fukuoka Prefecture, examining patients treated with baricitinib for RA. Patients were categorized into two dose groups: 4 mg with normal/mild renal dysfunction and 2 mg with moderate renal dysfunction.

Background: Although arthroscopic Bankart repair for shoulder instability after a traumatic shoulder dislocation is a standard procedure, return-to-play rates for overhead-throwing athletes with shoulder instability on the throwing side have been reported to be lower than those in non-overhead athletes or in non-throwing shoulders. A recent technical report showed that the dynamic anterior glenohumeral capsular ligament tensioning (DAGHT) procedure in abducted external rotation, which mimics the late cocking phase of the throwing motion during surgery, may provide optimal tensioning of the anterior glenohumeral ligament. This study compared clinical outcomes and return to play after arthroscopic Bankart repair between DAGHT and a conventional procedure for overhead-throwing athletes with traumatic dislocation on the throwing side.

With the increase in life expectancy, the number of elderly individuals undergoing hemiarthroplasty and total hip arthroplasty has risen, leading to a higher incidence of implant-related fractures. Diagnosing fractures, especially occult and non-displaced ones, can be challenging even with advanced imaging techniques. This report describes the diagnostic challenges and surgical management of a rare combination of periprosthetic femoral fracture and an ipsilateral acetabular occult non-displaced fracture.

Although the most common surgical treatment for traumatic anterior shoulder instability is arthroscopic Bankart repair (ABR), which has shown good postoperative results, a potential risk of postoperative external rotation deficit exists. For overhead-throwing athletes, recovery of postoperative range of motion during abduction and external rotation is essential to return to preinjury performance levels. We consider that the key to returning to play after ABR on the dominant side in overhead-throwing athletes is to simultaneously gain anterior stability and mobility of the shoulder.

Background And Hypothesis: There are limited data on the long-term outcomes and risk factors for non-recovery after development of rituximab (RTX)-associated persistent hypogammaglobulinaemia among children with idiopathic nephrotic syndrome (NS).
Methods: A nationwide Japanese survey was conducted to determine the prognosis of patients with childhood-onset idiopathic NS who developed persistent hypogammaglobulinaemia after RTX administration. Specifically, predictors of IgG level recovery and risk factors for serious infection were examined.

Background: Although recent studies have revealed the importance of inflammation in the pathogenesis of aortic dissection (AD), little is known about the relationships among inflammatory cells in human AD tissue.
Methods And Results: We assessed the relationships among various immune cell types, including neutrophils, macrophages (M1 and M2), B cells, and helper T cells (Th1, Th2, Th17, Treg and Tfh ) in human AD tissue. AD tissues displayed abundant infiltration of immune cells.

Purpose: To evaluate the relationship between kinetic parameters of ultrafast dynamic contrast-enhanced (DCE) magnetic resonance imaging (MRI) and tumor-infiltrating lymphocytes (TILs) in breast cancer.
Patients And Methods: This retrospective study was approved by an institutional review board and included 76 women (median age: 60) with 76 surgically proven breast cancers who underwent DCE MRI including ultrafast sequence. Based on the TILs level, we classified the patients into the low-TILs (< 10%) group and the high-TILs (≥ 10%) group.

The consensus is that the indication for renal denervation (RDN) is resistant hypertension or "conditioned" uncontrolled hypertension, with high office and out-of-office blood pressure (BP) readings despite appropriate lifestyle modification and antihypertensive drug therapy. "Conditioned" uncontrolled hypertension is defined as having one of the following: 1) inability to up-titrate antihypertensive medication due to side effects, the presence of complications, or reduced quality of life.

Databases were searched up to 15 November 2023 to identify randomized, sham-controlled trials of RDN.
