[A case of recurrent breast cancer that responded to bevacizumab].

We report a case of a 59-year-old woman who was forced to undergo mastectomy of the right breast (Rt Bt) plus axillary lymph node (Ax) dissection for right breast cancer at another hospital. The pathological diagnosis was invasive ductal carcinoma( scirrhou[s sci], pT2N2M0, Stage IIIA, estrogen recepto[r ER[]+], progesterone recepto[r PgR[]+], human epidermal growth factor receptor-2[HER2][2+]). Although no recurrence was observed after postoperative adjuvant chemotherapy and endocrine therapy, skin metastasis on the left back and pleuritis carcinomatosa were detected at our hospital 9 years and 6 months after the operation.

[Protocol for the administration of modified FOLFOX6 (mFOLFOX6) in patients with unresectable/recurrent colorectal cancer].

In patients undergoing FOLFOX6 therapy for the treatment of unresectable/recurrent colorectal cancer, control of cumulative peripheral neuropathy is problematic. In our department, we stop using mFOLFOX6 as a primary therapy after 6 to 8 courses. Instead, we use mFOLFOX6 as a secondary therapy, and re-introduce mFOLFOX6 as a tertiary therapy; the patients undergoing this treatment protocol were included in Group A.

Cervical pregnancy: a report of four cases.

Various conservative treatments for cervical pregnancy have been reported. However, unlike tubal ectopic pregnancy, the treatment of cervical pregnancy has not been well established. For patients who desire fertility preservation, treatment with methotrexate chemotherapy carries a high success rate for preservation of the uterus.

[A case of early Takayasu's arteritis with initial symptoms of carotidynia and fever].

We report herein on a 15-year-old girl with Takayasu's arteritis (TA) in the early pre-pulseless phase who presented with left neck pain and fever. Laboratory examination revealed a high level of C-reactive protein (CRP) and an elevated erythrocyte sedimentation rate (ESR). Findings of ultrasonography, computed tomography (CT), and magnetic resonance imaging and angiography (MRI, MRA) demonstrated a thickening of the left carotid artery but neither stenosis nor dilation of the large vessels.

Extrathoracic subclavian venipuncture by using only the J-type guidewire for permanent pacemaker electrode placement.

Extrathoracic subclavian/axillar venipuncture is an accepted method for implanting pacemaker leads. Although several procedures have been reported, no standard method has been established yet. We evaluated the usefulness of a method in which only J-type guidewires are used.

[A case of urachal abscess accompanied by a stone].

A 64-year-old woman presented to our hospital with the chief complaints of abdominal pain and appetite loss, and she was admitted to the internal medicine department. Kidney, ureter and bladder X-ray revealed intrapelvic calcification near the bladder, and so, the patient consulted our department. Computed tomography and magnetic resonance imaging revealed an urachal abscess accompanied by a stone.

Kami-shoyo-san, Kampo (Japanese traditional medicine), is effective for climacteric syndrome, especially in hormone-replacement-therapy-resistant patients who strongly complain of psychological symptoms.

Aim: Hormone replacement therapy (HRT) for climacteric symptoms is effective for vasomotor symptoms, such as hot flushes and sweating, but not for various psychological symptoms, such as anxiety, depression, and irritability. In such cases, prescribing Kampo, traditional herbal medicine, is sometimes considered. However, the effectiveness of Kampo has not been clearly shown.

Visceral adipose tissue level, as estimated by the bioimpedance analysis method, is associated with impaired lung function.

Unlabelled: Aims/Introduction:  It has been reported that metabolic syndrome is associated with impaired lung function, and abdominal obesity is regarded as the most important determinant of this association. We evaluated the association between a component of metabolic syndrome, indices of body composition, including the total adipose tissue content, lean bodyweight and visceral adipose tissue content, as assessed by bioimpedance analysis, and lung function.

Materials And Methods:   A total of 516 participants responded to our questionnaire to determine the smoking status and history of past diseases.

[A case of a foreign body (sheath dilator) in the urethra and bladder].

An 84-year-old man presented with the chief complaints of fever and disturbance of consciousness at another hospital. A sheath dilator had been inserted into the urethra instead of an indwelling catheter at the hospital. Since he had previously been treated for myelodysplastic syndrome in our Department of Internal Medicine, he was transferred to our hospital.

[Long-term survival case of small-cell lung cancer with Lambert-Eaton myasthenic syndrome without anticancer therapy].

A 78-year-old man with complaints of appetite loss and weight loss visited our hospital in November 2006. Positron-emission tomography and computed tomography (PET/CT) showed swollen lymph nodes in the abdominal para-aorta, mediastinum and neck, with intense FDG accumulation. The pathological findings of the cervical lymph nodes revealed small-cell cancer.

Closed rupture of both flexor digitorum profundus and superficialis tendons of the small finger in zone II: case report.

We report a rare case of closed rupture of both flexor digitorum profundus (FDP) and flexor digitorum superficialis tendons in zone II in the small finger. We performed delayed, primary end-to-end suture of the FDP and excision of the flexor digitorum superficialis, because myostatic contracture of the FDP tendon was not severe and the FDP tendon remnants were not frayed.

A prospective, randomized, controlled, multicenter study of naftopidil for treatment of male lower urinary tract symptoms associated with benign prostatic hyperplasia: 75 mg once daily in the evening compared to 25 mg thrice daily.

Introduction: We examined the tolerability of dosage methods of naftopidil in the treatment of male lower urinary tract symptoms associated with benign prostatic hyperplasia (BPH/male LUTS).

Patients And Methods: A total of 80 patients with BPH/male LUTS who had an International Prostate Symptom Score (IPSS) >or=8 and IPSS quality of life (QoL) >or=2 were enrolled and randomly administered naftopidil for 8 weeks at either 75 mg once daily (OD) in the evening (group O; n = 41) or 25 mg thrice daily (TID) in the morning, afternoon and evening (group T; n = 39).

Results: IPSS total score, IPSS-QoL and BPH impact index (BII) were significantly improved for both groups at 8 weeks after starting treatment compared to baseline.

Case of three delayed complications of radiotherapy: bilateral vocal cord immobility, esophageal obstruction and ruptured pseudoaneurysm of carotid artery.

We report the first case of three delayed complications following irradiation for laryngeal carcinoma: bilateral vocal cord immobility, obstruction of esophagus and spontaneously ruptured pseudoaneurysm of common carotid artery. Medial fixation of bilateral vocal cords and stenosis of cervical esophagus were noted at 28 years after radiotherapy. Spontaneous rupture of a pseudoaneurysm bulging into the hypopharynx and obstruction of the esophagus occurred at 35 years after irradiation.

Effects of Japanese herbal medicine, Juzen-taiho-to, in otitis-prone children--a preliminary study.

Conclusion: Juzen-taiho-to (JTT, TJ-48), a Japanese herbal medicine that improves immune function, was found to be effective in otitis-prone in children.

Objective: To evaluate the efficacy of JTT against intractable and recurrent infections in immature immune systems, we administered JTT to otitis-prone infants and investigated clinical changes before and during JTT administration.

Subjects And Methods: Twenty-four otitis-prone infants were administered JTT at 0.

Cervical spondylolysis in a judo player: a case report and biomechanical analysis.

Study Design: A case report and a biomechanical study using a finite element method.

Objectives: To report a case with the cervical spondylolysis and to understand the biomechanics of the cervical spine with spondylolysis at C6. Cervical spondylolysis, although not a common spinal disorder, can occur in athletes.

[Five patients with intrapulmonary lymph nodes indistinguishable from small peripheral carcinoma of the lung].

We encountered 5 patients with subpleural intrapulmonary lymph nodes. In all 5 patients, computed tomographic (CT) scans demonstrated subpleural small nodules in the middle or lower lung lobes. Because lung cancer could not be ruled out, biopsies by video-assisted thoracic surgery (VATS) were performed, yielding a diagnosis of intrapulmonary lymph nodes in all 5 cases.

Chronic cholesterol crystal embolism with a spontaneous onset.

A 74-year-old man was referred to our hospital because of hypertension, blue toe syndrome and an elevation of serum creatinine from 0.8 to 1.4 mg/dl for eleven months.

Spontaneous hemothorax in neurofibromatosis treated with percutaneous embolization.

We evaluated the effectiveness of transcatheter arterial coil embolization therapy for the treatment of spontaneous hemothorax followed by aneurysm rupture in neurofibromatosis patients. Three patients were treated for massive hemothorax caused by arterial lesions associated with neurofibromatosis. Bleeding episodes were secondary to ascending cervical artery aneurysm and dissection of vertebral artery in 1 patient, and intercostal artery aneurysm with or without arteriovenous fistula in 2 patients.

Longitudinal changes of pulmonary function and bronchial responsiveness in cough-variant asthma treated with bronchodilators alone.

Cough due to cough-variant asthma (CVA) responds well to bronchodilators such as beta 2 adrenergic agonists. The aim of this study was to assess longitudinal changes of pulmonary function and bronchial responsiveness in CVA, which was treated with bronchodilators alone. Seventeen CVA patients recorded intensity and frequency of cough every day.

IgG4-associated prostatitis complicating autoimmune pancreatitis.

A 65-year-old man underwent transurethral resection of the prostate on a diagnosis of benign prostatic hypertrophy. Almost simultaneously, a diagnostic resection of minor salivary glands was performed. After the resections he suffered from obstructive jaundice.

Development of a semi-automated superimposing image verification system using a template matching algorithm in radiotherapy.

To analyze shifts in the isocenter of images, we developed a semi-automated superimposing image-verification system that is capable of automatically quantifying shifts in the isocenter through image analysis with a personal computer (PC). The accuracy and usefulness of this software were examined through a comparison of nine portal images with a simulation image and by comparing nine portal images with a DRR image, using a human pelvic phantom. The difference between the known magnitude of shift and the magnitude of shift detected with this method was analyzed as detection error.

[CT and ultrasonographic findings in patients with carotidynia].

We report 2 cases of clinical signs and symptoms consistent with idiopathic carotidynia, evaluating and following up the appearance of carotidynia on CT and ultrasonography. Subjects were a 22-year-old woman and an 82-year-old man reporting pain and mass on the left side of the neck. They had abnormal soft-tissue infiltration surrounding the symptomatic carotid artery centered at the distal common carotid and carotid bifurcation in CT and ultrasonography.
