Giant adrenal myelolipoma: report of a case.

Adrenal myelolipomas are rare benign tumors composed of mature adipose tissue and hematopoietic elements that resemble bone marrow. They are usually asymptomatic, and most cases are incidentally found at radiological examination or autopsy. Symptoms such as abdominal pain and increasing girth occur only when the tumor grows large.

CD34 positive stromal cells in gastric adenocarcinomas.

Aims: To investigate the role of CD34 positive stromal cells, namely dendritic interstitial cells, in gastric carcinomas, the distribution of CD34 positive stromal cells in gastric adenocarcinomas (GCs), with special reference to two histological types (diffuse (D-type) and intestinal (I-type)), was examined.

Methods: In total, 55 surgically resected GCs (15 D-type and 40 I-type) and their normal tissues were examined. To distinguish CD34 positive stromal cells from vascular endothelial cells and to recognise the tumour border, immunostaining for CD34, CD31, and low molecular weight cytokeratins was performed.

Cyclin E mRNA overexpression in epithelial ovarian cancers: inverse correlation with p53 protein accumulation.

Objective: We investigated the relationship between cyclin E mRNA overexpression and p53 protein accumulation in epithelial ovarian cancers.

Methods: mRNA was isolated and cDNA was prepared from 36 epithelial ovarian tumors (three adenomas, three low malignant potential tumors, and 30 carcinomas), and six normal ovaries. The cyclin E mRNA expression levels relative to an internal control, beta-tubulin, were determined by semiquantitative polymerase chain reaction (PCR).

A new method of laparoscopic cholecystectomy using three trocars combined with suture retraction of gallbladder.

Background And Purpose: Since the establishment of laparoscopic cholecystectomy (LC) for benign gallbladder lesions, the four-trocar method has been the standard procedure. However, the fourth trocar generally is used just for fundic retraction of the gallbladder. We have developed a three-trocar method for LC and performed it in 132 patients.

[Inflammatory pseudotumor of the lung with infiltration identified radiographically and hemoptysis].

The patient was a 53-year-old man whose chief complaint was a dry cough and hemoptysis. Chest X-ray films and computed tomographic scans revealed a hazy shadow with unclear margins in the left middle lobe. A diagnosis of inflammatory pseudotumor was made because transbronchial lung biopsy specimens showed proliferation of plasma cells and lymphocytes.

Reliable and cost-effective system for surgical stent graft insertion.

For intraoperative stented graft implantation, we use a half-inch translucent soft polyvinyl tube as a sheath and an obturator from a two-stage venous cannula as a pushing rod. Ten centimeters of any kind of graft can be used for the stent graft itself. The stent we used was a self-expandable Gianturco double Z stent and was sutured inside the graft.

Detection of IgA-binding sites on human immunodeficiency virus type-1 envelope glycoproteins, Gp120 and Gp41.

IgA has been supposed to play an important role in the prevention of HIV-1 infection. In this study, IgA-binding sites on gp120 and gp41 of HIV-1 envelope glycoproteins were analyzed using ELISA and overlapping synthetic peptides covering all of the gp120 and gp41 sites. IgA antibodies in plasma and saliva mainly bound to six and five sites on gp120 and gp41, respectively.

The results of radiation therapy for glottic carcinoma: prognostic significance of tumor size in laryngoscopic findings.

Seventy-five patients with carcinoma of the true vocal cord were treated with curative intent by radiotherapy at Kure National Hospital between 1977 and 1997. All tumors were classified according to stage (T1, T2 and T3), degree of differentiation (well-differentiated, moderately differentiated, poorly differentiated and unknown), and size (very small, small and large). All patients were treated using a cobalt-60 unit to a total dose that ranged from 56 Gy to 64 Gy (mean, 60 Gy).

Cytokines facilitate chemotactic motility of breast carcinoma cells.

Background: Both growth and motility of various tumor cells have been shown to be influenced by surrounding cells such as lymphocytes, histiocytes and fibroblasts through various cytokines, growth factors and extracellular matrices. The role of cytokines and extracellular matrices produced by lymphocytes, histiocytes and fibroblasts on migration and invasion of breast carcinoma cells has not been fully investigated

Methods: We investigated the effect of hepatocyte growth factor (HGF), interleukin (IL)-6, IL-8, IL-11, soluble type IV collagen and soluble laminin on the migration of 3 human breast carcinoma cell lines, MDA-MB-231, MCF-7 and T-47D, using a cell culture insert and a biocoat matrigel invasion chamber to assess migration across a matrigel-coated polyethylene telephtalate membrane.

Results: HGF, IL-6, IL-11 and IL-8 induced significant migration of MDA-MB-231 cells depending on the dose of each cytokine.

Myofibroblasts at the tumor border of invasive gastric carcinomas: with special reference to histological type and tumor depth.

In order to elucidate the role of alpha-smooth muscle actin (ASMA)-positive stromal cells, namely myofibroblasts (MFs), in invasive growth of gastric carcinomas (GCs), we examined the number of MFs at the deep border (DB) of GCs. In total 78 invasive GCs (48 intestinal type GCs and 30 diffuse type GCs) were examined immunohistochemically, and analyzed from the view point of tumor depth and histological type. In the intestinal type GCs examined, both the GCs confined to the submucosa and muscularis propria had none or a small number of MFs at the tumor border facing the submucosa (smDB) and muscularis propria (mpDB), respectively; whereas the GCs invading the subserosa had a moderate or large number of MFs at the tumor border facing the subserosa (ssDB) (p<0.

Combined latissimus dorsi musculocutaneous flap and free superior gluteal artery skin flap.

We used a combined superior gluteal artery skin flap and latissimus dorsi musculocutaneous flap to repair a massive tissue defect of the upper limb. This combined flap has several advantages in that the length can be more than 60 cm, it can be used as an auxilliary muscle (latissimus dorsi) and it can also be used as a sensory flap. This combined flap is useful for covering massive skin defects.

Differential display of ethanol-induced gene in N18TG2 cells.

Background: Recent advances in molecular biology, such as polymerase chain reaction (PCR) differential display, have enabled the screening of mRNAs transcriptionally regulated by chronic ethanol treatment. Screening of gene expression after ethanol exposure will be the most needed for new biological insights into alcoholism.

Methods: We used PCR differential display to detect differentially expressed RNAs in N18TG2 cells treated for 4 days with physiologic concentrations of ethanol (25 mM).

Chemokine receptor-usage of clinical HIV-1 isolates obtained from patients with HIV-1 infection in late clinical stages using PHA-blast.

In the present sudy, chemokine receptor-usage of primary HIV-1 isolates was examined using U87-CD4 cells expressing chemokine receptors CCR3, CCR5 and CXCR4. HIV-1 was isolated from the peripheral blood mononuclear cells (PBMC) and/or plasma of eight HIV-1-infected individuals in late CDC-II and CDC-IV clinical stages using PHA-blast prepared from the PBMC of healthy blood donors. The primary HIV-1 isolates from patients in late CDC-II stage rarely infected monocyte-derived macrophages in the present study, whereas most isolates from patients in the CDC-IV stage infected the macrophages.

Hyperimmunoglobulin E syndrome associated with nephrotic syndrome.

A 21-year-old man was admitted to Kure National Hospital with nephrotic syndrome in September 1996. He had suffered from an intractable pruritic skin rash and recurrent subcutaneous abscesses caused by the hyperimmunoglobulin E syndrome since the age of 18 months. Renal biopsy gave a diagnosis of membranoproliferative glomerulonephritis.

The aim of this study was to determine whether barium enema reduction (BER) is safe and effective in patients with a long duration of intussusception. Over the last 17 years, 104 patients were admitted to our hospital with a diagnosis of intussusception. All except 1 with peritonitis underwent BER primarily.

Long-term follow-up of a patient with hepatocellular carcinoma associated with triple hepatitis virus (HBV, HDV, HCV) infection.

A 69-year-old Japanese man with hepatocellular carcinoma (HCC) associated with triple hepatitis viruses [hepatitis B virus (HBV), hepatitis C virus (HCV), and hepatitis D virus (HDV)] infection is reported. The patient had a past history of intravenous drug abuse and a tattoo on his back. A liver biopsy, performed in November 1989, showed HCC associated with cirrhosis.

Breast reconstruction by means of innervated rectus abdominis myocutaneous flap.

We performed immediate breast reconstruction in 22 patients using rectus abdominis myocutaneous flap accompanied by neurorrhaphy in the past 2 years. In the neurorrhaphy, the 11th intercostal nerve, which controls the sensation of the myocutaneous flap, was anastomosed to the lateral cutaneous branch of the 4th intercostal nerve, which controls the sensation of the breast.
