During bidirectional cavo-pulmonary anastomosis (bidirectional Glenn; BDG), the thymic tissue is often excised to facilitate the exposure of the superior vena cava and its junction with the innominate vein. Subsequently, it is discarded. Since the last two decades, the lead author (ST) has pursued anchoring the excised thymus in its position by suturing it to the opposite unexcised thymic lobe.

Int J Stroke

January 2025

Indian Council of Medical Research, Non-Communicable Diseases Section, New Delhi, India.

Rationale: Early mortality in intracerebral hemorrhage (ICH) is due to hematoma volume (HV) expansion, and there are no effective treatments available other than reduction in blood pressure. Tranexamic acid (TXA) a hemostatic drug that is widely available and safe can be a cost-effective treatment for ICH, if proven efficacious.

Hypothesis: Administration of TXA in ICH patients when given within 4.

  • Aluminum phosphide (AlP) is a toxic substance readily available in unregulated markets in developing countries, posing serious health risks due to lack of antidotes.
  • The text describes three unique cases where AlP was detected in abdominal subcutaneous fat postmortem, highlighting a previously unreported method for detection amid challenges like decomposition and other interfering substances.
  • Gas chromatography-mass spectrometry (GC-MS) analysis confirmed AlP presence in both routine viscera and abdominal fat, suggesting that subcutaneous fat can effectively be used for toxicological testing, even in decomposed bodies.

Loop Diuretics Unique Mechanism of Action.

J Assoc Physicians India

September 2024

Country Head, Medical Affairs, Cipla Ltd., Mumbai, Maharashtra, India.

Loop diuretics, including torsemide, furosemide, bumetanide, and piretanide, act by inhibiting the sodium-potassium-chloride (Na/K/2Cl) cotransporter in the thick ascending limb of the loop of Henle within the nephron. This mechanism is pivotal in managing fluid retention associated with conditions such as heart failure, cirrhosis, chronic kidney disease, and hypertension. A comprehensive understanding of how these diuretics uniquely target this transporter provides crucial insights into effectively addressing fluid overload across diverse clinical conditions.

Purpose: Perception that extra-medullary fixation of these fractures are fraught with difficulties and deficiencies is becoming inappropriate. This device provides angular stable fixation retaining fracture biology with minimum interference to osseous and soft-tissue vascularity and it does not require reaming which destroys 80% of endosteal vasculature for 6-12 weeks. PFLCP averts iatrogenic fracture in lateral trochanteric wall (LTW) which is frequent with DHS, protects LTW from secondary fracture in post-operative period.

Pediatric lung abscess is a rare and poorly studied disease entity. In the past, prolonged courses of intravenous (IV) antibiotics have been successfully used; however, with the advent of interventional radiology, the main therapeutic approach is through percutaneous placement of pigtail catheters with ultrasound and computed tomography (CT) direction, where available. The pathogen yield identified from fluid samples of the abscess has dramatically increased owing to the greater invasive measures, such as aspiration and drainage, as well as enhanced microbiological diagnostic methods, which also include polymerase chain reaction testing.
