Introduction: The widely employed Kidney Donor Profile Index (KDPI) scoring system, designed for assessing deceased donors (DD), plays a pivotal role in predicting graft function post kidney transplantation (KT). Given the dynamic nature of renal function, including serum creatinine (sCr), in managing DDs, it remains uncertain optimal timing to use KDPI for assessing postoperative graft function.

Methods: In this retrospective review, we assessed 246 DDs who were managed within a donor management program from January 2010 to December 2021.

Background: Accurate medical advice is paramount in ensuring optimal patient care, and misinformation can lead to misguided decisions with potentially detrimental health outcomes. The emergence of large language models (LLMs) such as OpenAI's GPT-4 has spurred interest in their potential health care applications, particularly in automated medical consultation. Yet, rigorous investigations comparing their performance to human experts remain sparse.

Single-cell transcriptomics by clinical course of Mycobacterium avium complex pulmonary disease.

Sci Rep

July 2024

Division of Pulmonary and Critical Care Medicine, Department of Medicine, Samsung Medical Center, Sungkyunkwan University School of Medicine, Seoul, South Korea.

Article Synopsis
  • Mycobacterium avium complex pulmonary disease (MAC-PD) shows varied clinical outcomes, which are linked to different immune profiles in patients.
  • A study of 21 MAC-PD patients identified three clinical groups based on disease progression, revealing that group C (progressive disease) had fewer natural killer (NK) cells and more classical monocytes compared to groups A (spontaneous culture conversion) and B (stable without treatment).
  • Specific genes and immune markers were found to be up-regulated or down-regulated in different groups, providing insights into how immune responses may affect the progression of MAC-PD, potentially leading to new treatment strategies.

Article Synopsis
  • Current guidelines advise against routine imaging for asymptomatic breast cancer patients, yet advancements in treatments prompt a reevaluation of intensive imaging's role in survival outcomes.
  • A study of 4,130 patients from 11 Korean hospitals found that high-intensity imaging was linked to shorter distant metastasis-free survival, particularly for bone or lung metastases, while overall 5-year breast cancer-specific survival rates were high.
  • Despite earlier metastasis detection from frequent imaging, there was no significant correlation between screening intensity and breast cancer-specific survival, indicating that current treatments may diminish the benefits of intensive surveillance.

Purpose: The aim of this study was to generate deep learning-based regions of interest (ROIs) from equilibrium radionuclide angiography datasets for left ventricular ejection fraction (LVEF) measurement.

Patients And Methods: Manually drawn ROIs (mROIs) on end-systolic and end-diastolic images were extracted from reports in a Picture Archiving and Communications System. To reduce observer variability, preprocessed ROIs (pROIs) were delineated using a 41% threshold of the maximal pixel counts of the extracted mROIs and were labeled as ground-truth.

Background And Objectives: Lipid lowering therapy is essential to reduce the risk of major cardiovascular events; however, limited evidence exists regarding the use of statin with ezetimibe as primary prevention strategy for middle-aged adults. We aimed to investigate the impact of single pill combination therapy on clinical outcomes in relatively healthy middle-aged patients when compared with statin monotherapy.

Methods: Using the Korean National Health Insurance Service database, a propensity score match analysis was performed for baseline characteristics of 92,156 patients categorized into combination therapy (n=46,078) and statin monotherapy (n=46,078) groups.

Analysis of hemodynamics and impedance using bioelectrical impedance analysis in hypovolemic shock-induced swine model.

Sci Rep

July 2024

Division of Acute Care Surgery, Department of Surgery, Korea University Anam Hospital, Korea University Medical Center, Seoul, Republic of Korea.

To treat hypovolemic shock, fluid infusion or blood transfusion is essential to address insufficient volume. Much controversy surrounds resuscitation in hypovolemic shock. We aimed to identify the ideal fluid combination for treating hypovolemic shock-induced swine model, analyzing bioelectrical impedance and hemodynamics.

Aims: Protein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) are novel lipid-lowering agents used in patients with cardiovascular disease. Despite reassuring safety data from pivotal trials, increasing evidence from real-world studies suggests that PCSK9i increase the risk of bacterial and viral infections. Therefore, this study aimed to identify signals of infection-related adverse events (AEs) associated with PCSK9i.

Background: The increasing number of vaccines and the complexity of immunization programs, along with continuous changes in the epidemiology of infectious diseases, necessitate a systematic approach to vaccine effectiveness (VE) evaluation. This study presents a preliminary survey to establish a VE evaluation framework in Korea, focusing on the National Immunization Program.

Methods: Experts' opinions were collected through a two-round online survey targeting key stakeholders.

Neurological disorders such as Parkinson's disease (PD) often adversely affect the vascular system, leading to alterations in blood flow patterns. Functional near-infrared spectroscopy (fNIRS) is used to monitor hemodynamic changes via signal measurement. This study investigated the potential of using resting-state fNIRS data through a convolutional neural network (CNN) to evaluate PD with orthostatic hypotension.

Behavioral Activation and Brain Network Changes in Depression.

J Clin Neurol

July 2024

Department of Psychiatry, Korea University Anam Hospital, Korea University College of Medicine, Seoul, Korea.

Behavioral activation (BA) is a well-established method of evidence-based treatment for depression. There are clear links between the neural mechanisms underlying reward processing and BA treatment for depressive symptoms, including anhedonia; however, integrated interpretations of these two domains are lacking. Here we examine brain imaging studies involving BA treatments to investigate how changes in brain networks, including the reward networks, mediate the therapeutic effects of BA, and whether brain circuits are predictors of BA treatment responses.

Image-based deep learning model using DNA methylation data predicts the origin of cancer of unknown primary.

Neoplasia

September 2024

Department of Biomedical Sciences, Seoul National University Graduate School, Seoul, the Republic of Korea; Genomic Medicine Institute, Medical Research Center, Seoul National University, Seoul, the Republic of Korea. Electronic address:

Cancer of unknown primary (CUP) is a rare type of metastatic cancer in which the origin of the tumor is unknown. Since the treatment strategy for patients with metastatic tumors depends on knowing the primary site, accurate identification of the origin site is important. Here, we developed an image-based deep-learning model that utilizes a vision transformer algorithm for predicting the origin of CUP.

Purpose: The molecular classification of breast cancer is crucial for effective treatment. The emergence of digital pathology has ushered in a new era in which weakly supervised learning leveraging whole-slide images has gained prominence in developing deep learning models because this approach alleviates the need for extensive manual annotation. Weakly supervised learning was employed to classify the molecular subtypes of breast cancer.

Kidney Res Clin Pract

June 2024

Division of Nephrology, Department of Internal Medicine, Yonsei University College of Medicine, Seoul, Republic of Korea.

Background: Patients with sensitization and blood type O experience increased waiting times for deceased-donor kidney transplantation (DDKT). While allocation benefits are needed to resolve inequity in DDKT opportunity, whether DDKT has comparable outcomes in this disadvantaged population requires further study. This study assessed these outcomes and developed a new allocation system that balances equity and utility.

This study aimed to identify the clinical manifestation and implications according to the grading of tumor spread through air spaces in early-stage small (≤2 cm) pathological stage I non-mucinous lung adenocarcinomas. Medical records of patients with pathological stage I tumors sized ≤2 cm were retrospectively reviewed and analyzed. The furthest distance of the spread through air spaces from the tumor margin was measured on a standard-length scale (mm).
