Electronic address:

Patients with dihydropyrimidine dehydrogenase (DPD) deficiency in peripheral mononuclear cells are at higher risk of severe toxicity due to the improper dose of fluorouracil-based chemotherapy drugs, which has become an essential aspect for consideration in clinical studies. 5-fluorouracil (5-FU) is a first-line and second-line chemotherapy drug in adjuvant, neoadjuvant, or palliative therapy settings to treat solid tumors and cancers. In this work, a novel in-syringe-based fast drug extraction (IS-FaDEx) technique followed by UHPLC-MS/MS detection was developed for rapid biomonitoring of 5-FU and its biometabolites in human blood samples.

Article Synopsis
  • The study investigates how the cytotoxin-associated gene A (CagA) influences cell cycle progression in B lymphoma cells by activating nuclear factor of activated T cells (NFAT), which may affect the response to Helicobacter pylori eradication in gastric MALT lymphoma.
  • Researchers used three B-lymphoma cell lines and evaluated their response to H. pylori strains, measuring CagA expression, NFATc1 signaling, and cell cycle inhibitors.
  • Results show a significant link between CagA presence, nuclear NFATc1 localization, and responsiveness to treatment, suggesting that both CagA and NFATc1 may play roles in improving therapy outcomes for patients with gastric MALT lymphoma.

Background: The evidence for adjuvant chemoradiotherapy (CRT) of oral cavity squamous cell carcinoma (OCSCC) with extra-nodal extension (ENE) in National Comprehensive Cancer Network (NCCN) guidelines is derived from patients with head and neck cancer. The guidelines further suggest a radiation dose ranging from 6000 to 6600 cGy. In this nationwide study, we sought to evaluate the prognostic significance of adjuvant therapy and the specific radiation dosage in Taiwanese patients with pure OCSCC and ENE.

Article Synopsis
  • Palbociclib combined with tamoxifen was tested in a phase 3 study involving 184 women with HR+/HER2- advanced breast cancer to evaluate its efficacy and safety compared to tamoxifen alone.
  • The results showed a significant improvement in progression-free survival (PFS), with a median of 24.4 months for the palbociclib-tamoxifen group versus 11.1 months for the placebo-tamoxifen group.
  • While overall survival (OS) data is still being gathered, there is a trend suggesting palbociclib-tamoxifen may also reduce mortality risk, though neutropenia was a common severe side effect.

Article Synopsis
  • Occam's razor suggests finding a single diagnosis from various symptoms, while Hickam's dictum encourages considering multiple diagnoses, especially in complex cases.
  • The case of a 72-year-old man shows the importance of Hickam's dictum, as initial diagnoses missed co-existing metastatic cancers linked to his chronic fatigue and hyponatremia.
  • This emphasizes the need for healthcare providers to consider multiple potential conditions and remain cautious about liver cancer risks in patients with a history of hepatitis B, even after a functional cure.

Article Synopsis
  • Elective tracheotomy is often performed on patients with resected oral squamous cell carcinoma (OCSCC) to help keep their airways open, but its necessity varies among surgeons.
  • A large study including over 18,000 OCSCC patients found that those who had tracheotomies had lower 5-year disease-specific and overall survival rates compared to those who didn't, even after adjusting for other factors.
  • Despite the survival rates being similar after matching groups for other variables, patients with tracheotomies generally experienced longer hospital stays, indicating the procedure may be associated with more complications or extended recovery times.
