Traditionally, patients with a fracture of the distal radius are treated in a cast if they do not require surgery. If the fracture requires manipulation, the cast is moulded to hold the reduction and maintain normal anatomical alignment during healing. However, is a cast necessary for patients whose fracture does not require manipulation? Removable splints are an alternative treatment option.

Objectives: The objective of this study was to develop and test a patient decision aid for end-of-life care to be used when some or all life-sustaining treatments have been withheld or withdrawn.

Methods: A multi-professional, multi-sectorial group together with patients and relatives used a systematic process to develop and test the patient decision aid, including alpha and beta testing.

Results: Healthcare professionals, patients and relatives were involved in the development and testing.

The Clinical Frailty Scale to assess patients referred for intensive care therapy.

Dan Med J

October 2024

Department of Anesthesiology and Intensive Care, University Hospital of Southern Denmark - Lillebaelt Hospital, Kolding Hospital.

Introduction: Decisions to admit patients to intensive care units are often complex, and assessing patients is essential but difficult. In recent years, the Clinical Frailty Scale has been highlighted as a potential assessment tool for triaging patients for admission to the intensive care unit. This study aimed to investigate the clinical differences and Clinical Frailty Scale scores between patients who are refused and those who are admitted to the intensive care unit.

Background: Critically ill patients experience cognitive impairment throughout their intensive care unit trajectory, in the acute phase and the long-term alike. Cognitive impairment may negatively impact patients' quality of life and rehabilitation outcomes.

Aim: To provide an overall examination of literature concerning non-pharmacological interventions that can enhance cognitive functioning in critically ill patients or facilitate their rehabilitation pathway during and after their intensive care unit stay.

Introduction: Anaesthetic choices in cancer surgery, including the use of epidural analgesia, may affect immune function during the perioperative period and might play an important role in subsequent cancer spread and recurrence.

Methods: This was a prospective, randomised, controlled, double-blinded, single-centre study allocating patients scheduled for video-assisted thoracoscopic surgery (VATS) lobectomy to post-operative pain management using either thoracic epidural analgesia or oral morphine. We compared pre-, per-, and post-operative plasma levels of interleukin (IL)-6, IL-10, IL-12, and interferon (IFN)-γ using regression analysis, and conducted a two-year survival follow-up.

  • Biomarker guided therapy can enhance treatment for COVID-19 patients, with a focus on evaluating the effectiveness of point-of-care (POC) antibody tests in this context.* -
  • Two POC tests, LumiraDX and RightSign, were assessed for their reliability and ease of use against a standard test, with both showing moderate agreement but differing in accuracy and usability.* -
  • The RightSign test was found to be easier to use and more effective at identifying patients without antibodies compared to the LumiraDX test, despite both tests being associated with clinical outcomes.*

Introduction: Traumatic injury of a vascular prosthesis-to-prosthesis anastomosis leading to an extravasation and pseudoaneurysm is rare. If not identified the complications associated with this condition can lead to high morbidity and mortality and require surgical treatment.

Case: We describe a patient who presented with a tear in prosthesis-prosthesis anastomosis eight years after implantation resulting in a pseudoaneurysm.

Article Synopsis
  • - The study compared one-year outcomes of lower (8 kPa) and higher (12 kPa) oxygenation targets in ICU patients with COVID-19 and severe hypoxaemia, involving 726 randomly assigned patients.
  • - Findings showed that 33.6% of patients in the lower-oxygenation group died, compared to 39.1% in the higher-oxygenation group, suggesting a potential benefit of the lower target, though results were not statistically significant (p = 0.11).
  • - Health-related quality of life assessments indicated no significant differences between the two groups, with the lower-oxygenation group having a median EQ-VAS score of 50 vs 40 for the higher group (p

Article Synopsis
  • Functional neurological disorder (FND) is a significant cause of neurological disability in Europe, but it receives inadequate attention in medical training compared to conditions like multiple sclerosis (MS).
  • A survey of 25 countries revealed that FND is often not included in mandatory neurology training and has limited representation in examinations and specialized services.
  • There is a notable disparity in access to care and disability benefits for FND patients, highlighting the need for improvements in education, healthcare policies, and patient advocacy.

Triglyceride-glucose index improves risk prediction beyond traditional risk factors and hypertension mediated organ damage in healthy adults.

Nutr Metab Cardiovasc Dis

November 2024

Steno Diabetes Center Odense, Odense University Hospital, Odense, Denmark; Department of Internal Medicine, Kolding Hospital, Kolding, Denmark.

Article Synopsis
  • The study explored the relationship between the triglyceride-glucose (TyG) index, a marker for insulin resistance, and major adverse cardiovascular events (MACE) among healthy individuals over a follow-up duration of approximately 15.4 years.
  • Results showed that higher TyG index levels were linked to an increased risk of MACE, even after accounting for traditional cardiovascular risk factors and signs of heart damage.
  • The findings suggest that incorporating TyG index measurements may enhance the accuracy of cardiovascular risk predictions, especially in middle-aged individuals.

Diagnosing patients in the medical emergency department is complex and this is expected to increase in many countries due to an ageing population. In this study we investigate the feasibility of training machine learning algorithms to assist physicians handling the complex situation in the medical emergency departments. This is expected to reduce diagnostic errors and improve patient logistics and outcome.

Background: Prevalence of pulmonary embolism (PE) in patients referred to diagnostic imaging is decreasing, indicating a need for improving patient selection. The aim of this study was to assess reduction in referral to diagnostic imaging by integrating a bespoke ultrasound protocol and describe associated failure rate and adverse events in patients with suspected PE.

Methods: In a randomized open-label multicentre trial spanning June 18, 2021, through Feb 1, 2023, adult patients with suspected PE and 1) a Wells score of 0-6 and elevated age-adjusted D-dimer or 2) Wells score >6 were randomly assigned 1:1 to direct diagnostic imaging (controls) or focused lung, cardiac, and deep venous ultrasound by unblinded investigators.

Unlabelled: Coinfections with human pegivirus 1 (HPgV-1) are common in chronic hepatitis C virus (HCV) patients. However, little is known about whether HPgV-1 is affected by direct-acting antivirals during HCV treatment. Metagenomic analysis and reverse transcriptase-quantitative PCR (RT-qPCR) were performed on RNA from the plasma of 88 selected chronic HCV patients undergoing medical treatment.
