  • This study compared lower-dose melphalan (80 mg/m) and higher-dose melphalan (140 mg/m) for patients with myelodysplastic syndrome (MDS) receiving stem cell transplants.
  • Ninety-two patients were analyzed, revealing that those on the higher-dose FM140 had a significantly better overall survival rate at three years (63.9%) compared to the lower-dose FM80 group (54.2%).
  • The results indicate that higher-dose melphalan may be more effective, particularly for patients with poor cytogenetic risk, although the difference in relapse rates and nonrelapse mortality between the two groups was not statistically significant.

Chronic graft-versus-host disease (cGVHD) is a complication of allogeneic hematopoietic stem cell transplantation (HSCT), negatively impacting quality of life (QoL) and increasing the risk of death. Complexity in cGVHD diagnosis and treatment causes significant variations in cGVHD management strategies across medical centers and physicians despite the existence of published guidelines. Thus, we hypothesized that center volume is associated with cGVHD incidence and outcomes after cGVHD develops.

  • Monoclonal gammopathy of undetermined significance (MGUS) is a precancerous condition that can lead to multiple myeloma and may cause renal insufficiency in some patients, leading to a related condition called monoclonal gammopathy of renal significance (MGRS).
  • A unique case involving a 66-year-old Asian male with MGUS showed renal issues not due to M-protein but rather direct infiltration of clonal cells in the kidneys.
  • The study emphasizes the need for reconsidering the understanding of MGUS and its complications, as this specific infiltration has been confirmed through advanced techniques like fluorescence in situ hybridization.

Background: The association between meat, fish, or fatty acid intake and acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS) has been investigated in a few studies, and the results were inconsistent. In addition, most studies are mainly based on the United States and European countries, in which the dietary patterns differ from that in Asia. Therefore, the risk of AML/MDS from meat, fish, or fatty acid intake in Asia requires further exploration.

Allogeneic hematopoietic cell transplantation (HCT) offers a possible cure for patients with relapsed and refractory non-Hodgkin lymphoma (NHL) through potentially beneficial graft versus lymphoma effects. However, allogeneic HCT is associated with high nonrelapse mortality (NRM). Fludarabine with reduced-intensity busulfan (Flu/Bu2) and myeloablative busulfan (Flu/Bu4) are commonly used in conditioning regimens for allogeneic HCT; however, data on their use in patients with NHL is limited.

Article Synopsis
  • - The COVID-19 pandemic impacted healthcare globally, raising concerns about the quality and access to allogeneic hematopoietic stem cell transplantation (HSCT) in Japan, but the number of transplants actually increased in 2020 compared to previous years.
  • - Changes in HSCT practices during the pandemic included older patient demographics and a decrease in certain types of transplants, like those from unrelated and matched donors, alongside a rise in reduced-intensity conditioning.
  • - Despite these changes, the 1-year survival rates for patients remained stable across the years, indicating that it's possible to adapt transplant care during the pandemic while ensuring patient outcomes are preserved.

  • A conditioning regimen is crucial for allogeneic hematopoietic stem cell transplantation in MDS patients, but determining the best intensity (reduced-intensity vs. myeloablative) can be challenging.
  • This study analyzed 2,567 MDS patients who underwent HCT from 2009 to 2019, creating a machine learning model to calculate a benefit score for reduced-intensity conditioning (RIC).
  • In the validation phase, high-score patients showed improved progression-free survival with RIC compared to myeloablative conditioning (MAC), while low-score patients had worse outcomes with RIC, indicating that machine learning can help personalize treatment plans for MDS.
