Introduction: In patients undergoing transcatheter aortic valve replacement (TAVR), cerebral embolic protection devices (CEPD) are used to possibly diminish the risk of periprocedural stroke. Trends and outcomes of CEPD usage in TAVR are not well characterized.

Methods: National readmission databases (NRD) 2017-2019 was used to identify hospital admissions for TAVR using ICD-10 codes, with versus without Sentinel CEPD.

Layered plaque, a signature of previous plaque destabilization and healing, is a known predictor for rapid plaque progression; however, the mechanism of which is unknown. The aim of the current study was to compare the level of vascular inflammation and plaque vulnerability in layered plaques to investigate possible mechanisms of rapid plaque progression. This is a retrospective, observational, single-center cohort study.

Background: Lead dwell time is the single strongest predictor of failure and complications in transvenous lead extraction.

Objectives: To report the success rate and complications of transvenous lead extractions with implant dwell time of at least 15 years.

Methods: Procedural and patient data were prospectively collected into a database.

Genetically and clinically confirmed atypical cerebrotendinous xanthomatosis with normal cholestanol and marked elevations of bile acid precursors and bile alcohols.

J Clin Lipidol

June 2024

Center for Preventive Cardiology, Knight Cardiovascular Institute & Division of Endocrinology, Diabetes & Clinical Nutrition, OHSU, Portland, OR, United States of America (Dr Duell).

Background: Cerebrotendinous xanthomatosis (CTX) is a rare autosomal recessive lipid disorder. Affected patients often remain undiagnosed until the age of 20-30 years, when they have already developed significant neurologic disease that may not be reversible. An elevated plasma cholestanol concentration has been accepted as a diagnostic criterion for CTX for decades.

Article Synopsis
  • - The study focused on evaluating the safety and effectiveness of the APTURE transcatheter shunt system in patients with heart failure and left ventricular ejection fraction (LVEF) greater than 40%.
  • - Results showed that the APTURE shunt had a low incidence of serious complications (2.6%) and helped improve heart function, as indicated by reduced pulmonary capillary wedge pressure and improved quality of life scores over a year.
  • - Overall, the findings suggest that the APTURE shunt is a promising option for managing heart failure in patients with mildly reduced or preserved ejection fraction.

  • Nonobstructive hypertrophic cardiomyopathy (nHCM) currently lacks approved treatments, but a new drug, ninerafaxstat, aims to improve heart energy and alleviate symptoms.
  • A clinical trial with 67 patients showed that ninerafaxstat led to better ventilatory efficiency after 12 weeks compared to a placebo, despite similar peak exercise levels.
  • Overall, ninerafaxstat was safe, well tolerated, and particularly beneficial for patients with more severe symptoms, indicating potential for improving their health and exercise capacity.

Background: Giant cell myocarditis (GCM) is an inflammatory form of acute heart failure with high rates of cardiac transplantation or death. Standard acute treatment includes multi-drug immunosuppressive regimens. There is a small but growing number of case reports utilizing rabbit anti-thymocyte globulin in severe cases.

  • World Rugby requires ECGs for players entering international competitions, but they are not mandatory for all players; this study aimed to establish ECG norms for male and female rugby players.
  • Researchers analyzed the ECGs of 356 professional rugby players, focusing on differences by sex and playing position, using guidelines from 2017 to categorize findings.
  • Results showed that 84% of players had normal ECGs, with a higher percentage in males (91%) compared to females (60%), and only a small fraction (2.2%) had positive ECGs, indicating the potential feasibility of ECG screenings in rugby.

Informational letters or postcards to initiate remote monitoring among veterans with pacemakers and implantable cardioverter-defibrillators: A randomized, controlled trial.

Pacing Clin Electrophysiol

May 2024

Section of Cardiology, Department of Medicine, San Francisco Veterans Affairs Health Care System, San Francisco, California, USA.

Background: Remote monitoring (RM) of pacemakers and implantable cardioverter-defibrillators (ICDs) is a Class 1, Level of Evidence A recommendation because of its multitude of clinical benefits. However, RM adherence rates are suboptimal, precluding patients from achieving these benefits. There is a need for direct-to-patient efforts to improve adherence.

Background: Insurance companies have adopted variable and inconsistent approval criteria for chronic venous disease (CVD) treatment. Although vein ablation (VA) is accepted as the standard of care for venous ulcers, the treatment criteria for patients with milder forms of CVD remain controversial. This study aims to identify factors associated with a lack of clinical improvement (LCI) in patients with less severe CVD without ulceration undergoing VA to improve patient selection for treatment.

Nicotine is the primary addictive component of tobacco products. Through its actions on the heart and autonomic nervous system, nicotine exposure is associated with electrophysiological changes and increased arrhythmia susceptibility. To assess the underlying mechanisms, we treated rabbits with transdermal nicotine (NIC, 21 mg/day) or control (CT) patches for 28 days before performing dual optical mapping of transmembrane potential (RH237) and intracellular Ca (Rhod-2 AM) in isolated hearts with intact sympathetic innervation.

Barriers to implementing good nutrition in pregnancy and early childhood: Creating equitable national solutions.

Ann N Y Acad Sci

April 2024

Knight Cardiovascular Institute, Center for Developmental Health, and Moore Institute for Nutrition & Wellness, Oregon Health & Science University, Portland, Oregon, USA.

Exposure to deleterious stressors in early life, such as poor nutrition, underlies most adult-onset chronic diseases. As rates of chronic disease continue to climb in the United States, a focus on good nutrition before and during pregnancy, lactation, and early childhood provides a potential opportunity to reverse this trend. This report provides an overview of nutrition investigations in pregnancy and early childhood and addresses racial disparities and health outcomes, current national guidelines, and barriers to achieving adequate nutrition in pregnant individuals and children.

We develop a large-scale single-cell ATAC-seq method by combining Tn5-based pre-indexing with 10× Genomics barcoding, enabling the indexing of up to 200,000 nuclei across multiple samples in a single reaction. We profile 449,953 nuclei across diverse tissues, including the human cortex, mouse brain, human lung, mouse lung, mouse liver, and lung tissue from a club cell secretory protein knockout (CC16) model. Our study of CC16 nuclei uncovers previously underappreciated technical artifacts derived from remnant 129 mouse strain genetic material, which cause profound cell-type-specific changes in regulatory elements near many genes, thereby confounding the interpretation of this commonly referenced mouse model.

Aims: Despite increased temporary mechanical circulatory support (tMCS) utilization for acute myocardial infarction complicated by cardiogenic shock (AMI-CS), data regarding efficacy and optimal timing for tMCS support are limited. This study aimed to describe outcomes based on tMCS timing in AMI-CS and to identify predictors of 30-day mortality and readmission.

Methods And Results: Patients with AMI-CS identified in the National Readmissions Database were grouped according to the use of tMCS and early (<24 h) vs.

Article Synopsis
  • This phase 2 trial assessed the safety and effectiveness of a medication called aficamten in patients suffering from nonobstructive hypertrophic cardiomyopathy (nHCM).
  • 41 patients participated, and after 10 weeks of treatment, over half showed improvement in heart failure symptoms, with many reaching a better functional class.
  • While some patients experienced a slight decrease in heart function (LVEF), the overall results indicated that aficamten is generally safe and effective in improving symptoms and relevant heart health markers.
