Background: Guideline-directed medical therapy has become an important component of heart failure (HF) therapy, with sacubitril/valsartan as one of the recommended drugs; however, the real-world prognostic implications of sacubitril/valsartan uptitration are unclear.

Methods And Results: Patients with HF newly initiated on sacubitril/valsartan were registered in a retrospective multicenter study (REVIEW-HF). In all, 995 patients were divided into 3 groups according to the maximum dose achieved: high dose, sacubitril/valsartan 400 mg; intermediate dose, sacubitril/valsartan 200-<400 mg; and low dose, sacubitril/valsartan <200 mg.

Article Synopsis
  • Evolocumab, a monoclonal antibody approved for treating familial hypercholesterolemia (FH) and hypercholesterolemia, was evaluated in a real-world study in Japan, as data on its effectiveness and safety were previously limited.
  • The study involved 3724 patients with various forms of FH and hypercholesterolemia over 104 weeks, measuring both safety (incidences of adverse reactions) and effectiveness (changes in LDL cholesterol levels).
  • Results indicated that Evolocumab was well tolerated, showing significant reductions in LDL cholesterol levels, with low rates of serious adverse events, suggesting it is a viable treatment option for high-risk patients in Japan.

  • - Migraine affects many people aged 20-50 and has serious social and economic consequences, but its underlying causes are still largely unknown; this review focuses on the role of cortical spreading depolarization (CSD) related to migraine aura and potential therapies.
  • - The article outlines how CSD, which leads to significant neuronal changes, is linked to the visual symptoms of migraines and is also found in other neurological disorders, emphasizing its cellular characteristics and effects on the trigeminal nervous system.
  • - Current treatments include prevention strategies and drugs targeting the calcitonin gene-related peptide (CGRP) involved in migraine pain, while new therapies like intranasal insulin-like growth factor 1 and vagus nerve stimulation show promise in reducing

This study aimed to evaluate the osteogenic potential of mesenchymal stromal cell (MSC) spheroids combined with the basic fibroblast growth factor (bFGF) in a mouse femur fracture model. To begin, MSC spheroids were generated, and the expression of key trophic factors (, and ) was assessed using quantitative PCR (qPCR). A binding assay confirmed the interaction between the bFGF and the spheroids' extracellular matrix.

  • The study aimed to enhance the accuracy of predicting glaucomatous visual field (VF) impairment by using pointwise total deviation (TD) values with a machine-learning method called random forest model (RFM) and examining the role of ganglion cell-inner plexiform layer (GCIPL) thickness.
  • Researchers analyzed data from 89 eyes with open-angle glaucoma that had cataract surgery, using various predictive models to calculate mean TD.
  • Results showed that RFM provided more accurate predictions than traditional methods, and incorporating GCIPL measurements did not significantly improve the model’s accuracy.

We successfully treated a 4-year-old girl with short bowel syndrome and eosinophilic enterocolitis with teduglutide, a glucagon-like peptide-2 analog. Her eosinophilic enterocolitis was cured without relapse, and we were able to increase enteral nutrition. We found that teduglutide had an anti-inflammatory effect in this patient with eosinophilic gastrointestinal disease associated with short bowel syndrome.

Article Synopsis
  • - A rare case of blunt abdominal aortic injury (BAAI) in a patient over 80 was reported, following a motor vehicle accident, which presented with both thoracic and abdominal aortic injuries.
  • - The patient underwent endovascular repair for the abdominal aortic injury on the day of admission and a thoracic repair 11 days later, resulting in a successful recovery.
  • - The findings suggest that the timing of intervention for aortic injuries should consider the urgency of treatment and any potential damage to other organs.

Objective: To investigate the incidence of male sexual dysfunction (SD) including erectile dysfunction (ErD) and ejaculatory dysfunction (EjD) after minimally invasive rectal cancer surgery.

Background: Male SD significantly affects post-rectal cancer surgery quality of life (QOL). Current assessments using the International Index of Erectile Function-5 are unsuitable for patients with reduced postoperative sexual activity, because it assumes sexual intercourse.

Background: Patients with heart failure exhibiting low systolic blood pressure (SBP) have a poor prognosis. Sacubitril/valsartan reduces cardiovascular events; however, its use in patients with low SBP has not been fully examined. Therefore, in this study, we aimed to investigate the association between baseline SBP and adverse events (AEs) in patients starting sacubitril/valsartan therapy using data from a real-world registry in Japan.

Article Synopsis
  • Older patients (65+) with heart failure (HF) often experience physical frailty and malnutrition, which can worsen each other and lead to poor health outcomes.
  • A study involving 862 hospitalized patients found that those with both frailty and malnutrition had the highest risk of death within a year of discharge compared to those with neither condition.
  • Identifying and addressing both frailty and malnutrition in these patients is essential for improving their overall health and survival rates.

Fournier's gangrene is a severe type of necrotizing fasciitis that affects the perineal and genital regions. Because of its rapid progression, Fournier's gangrene is associated with high mortality and morbidity rates. Surgical treatment of Fournier's gangrene requires leaving the wound open and performing multiple debridement procedures.

CD163 Tumor-Associated Macrophage Recruitment Predicts Papillary Thyroid Cancer Recurrence.

J Surg Res

Division of Advanced Surgical Oncology, Research and Development Center for New Medical Frontiers, Kitasato University School of Medicine, Sagamihara, Kanagawa, Japan.

Introduction: Skewed immune response plays a pivotal role in tumor progression. Systemic inflammatory responses represented by combined peripheral leukocyte fractions are prognostic predictors of multiple cancers, including thyroid cancer. We previously reported the prognostic significance of lymphocyte-to-monocyte ratio (LMR) in curatively resected papillary thyroid cancer (PTC).

Purpose: Neoadjuvant chemoradiotherapy (nCRT) is employed for the local control of locally advanced rectal cancer; however, its prognostic impact is limited and often impairs pelvic organ function. Therefore, careful patient selection is essential. This study aimed to investigate the impact of nCRT on relapse-free survival (RFS) by stratifying patients according to MRI detected circumferential resection margin (mrCRM) or extramural vascular invasion (mrEMVI), as the ability of MRI findings to identify patients who will have beneficial outcomes from nCRT is uncertain.

  • - The study investigated the significance of residual hyperintense areas on T2-weighted MRI in glioblastoma patients after initial treatment, involving surgery, radiotherapy, and temozolomide.
  • - Out of 150 cases, 77 were analyzed, revealing that 71.4% had residual hyperintense areas post-treatment, with a median progression-free survival of 12.4 months and overall survival of 27.4 months; however, these areas were not indicative of prognosis.
  • - Results showed that local recurrences aligned with residual hyperintense areas, while the T2D group (without these areas) experienced more distant recurrences, highlighting the need for targeted monitoring of both groups based on their

  • Osimertinib is effective as a first-line treatment for patients with EGFR mutation-positive NSCLC, with a median progression-free survival of 20 months and overall survival of 41 months in a multicenter study of 583 patients.
  • Adverse events occurred in 23.3% of patients, and different progression patterns were identified, with many patients remaining asymptomatic upon progression.
  • Continuing osimertinib post-progression may not be beneficial, as those who discontinued treated had better survival rates compared to those who persisted on the drug, suggesting a need for a shift to alternative therapies like platinum plus pemetrexed for improved outcomes.
