Introduction: Debriefing after simulation facilitates reflective thinking and learning. Eye-tracking augmented debriefing (ETAD) may provide advantages over traditional debriefing (TD) by leveraging video replay with first-person perspective. This multisite randomized controlled trial compared the impact of ETAD with TD (without eye-tracking and without video) after simulation on 4 outcomes: (1) resident metacognitive awareness (the primary outcome), (2) cognitive load (CL) of residents and debriefers, (3) alignment of resident self-assessment and debriefer assessment scores, and (4) resident and debriefer perceptions of the debriefing experience.

Childhood Separation Anxiety Disorder and Panic Disorder: Differences Between Respiratory and Nonrespiratory Subtypes.

Prim Care Companion CNS Disord

September 2024

Laboratório de Pânico e Respiração, Instituto de Psiquiatria, Universidade Federal do Rio de Janeiro, Rio de Janeiro, Brazil.

Article Synopsis
  • A study investigated the link between childhood separation anxiety disorder (CSAD) and panic disorder (PD), finding a stronger prevalence of CSAD in patients with the respiratory subtype (RS) of PD compared to the nonrespiratory subtype (NRS) and control groups.
  • Among 180 participants, RS-PD patients exhibited a significantly higher history of CSAD at 55%, while NRS and control groups had 23% and 17%, respectively.
  • The findings suggest that RS-PD patients are 3 times more likely to have CSAD than NRS-PD patients and over 5 times more likely compared to controls, indicating the importance of screening RS-PD patients for separation anxiety symptoms to improve treatment outcomes.

Repetitive Transcranial Magnetic Stimulation (rTMS) in Major Depression.

Adv Exp Med Biol

September 2024

Department of Psychiatry and Centre for Neuroscience Studies, Queen's University, Kingston, ON, Canada.

Major depressive disorder (MDD) is a psychiatric disorder with several effective therapeutic approaches, being antidepressants and psychotherapies the first-line treatments. Nonetheless, due to side effects, limited efficacy, and contraindications for these treatments, alternative treatment options are required. Neurostimulation is a non-pharmacological and non-psychotherapeutic approach that has been under study for diverse neuropsychiatric conditions in the form of electrical or magnetic stimulation of the brain.

Introduction: Since 2022, all Canadian post-graduate medical programs have transitioned to a Competence by Design (CBD) model within a Competency-Based Medical Education (CBME) framework. The CBME model emphasized more frequent, formative assessment of residents to evaluate their progress towards predefined competencies in comparison to traditional medical education models. Faculty members therefore have increased responsibility for providing assessments to residents on a more regular basis, which has associated challenges.

J Bone Miner Res

April 2024

Division of Endocrinology, Department of Medicine, Centre for Menstrual Cycle and Ovulation Research, University of British Columbia, Vancouver, BC V5Z 1M9, Canada.

Article Synopsis
  • Sedentary behavior (SB), or prolonged sitting, has been linked to various health issues, particularly regarding bone health, but there is a lack of population-based data on its long-term impact on bone density and fracture risk.
  • The study assessed the relationship between daily sitting time, bone mineral density (BMD) measured via DXA, and the incidence of low-trauma fractures in a diverse group of over 8,000 participants in Canada aged 25 to 80+.
  • The findings indicated that while increased SB was associated with lower baseline BMD in certain groups, it did not correlate with significant changes in BMD or an increased risk of fractures over a 10-year period.

Antipsychotic agents in anxiety disorders: An umbrella review.

Acta Psychiatr Scand

April 2024

Laboratory of Panic and Respiration, Institute of Psychiatry, Federal University of Rio de Janeiro, Rio de Janeiro, Brazil.

Background: Although not approved for the treatment of anxiety disorders (except trifluoperazine) there is ongoing off-label, unapproved use of first-generation antipsychotics (FGAs) and second-generation antipsychotics (SGAs) for anxiety disorders. There have been systematic reviews and meta-analyses on the use of antipsychotics in anxiety disorders, most of which focused on SGAs.

Objective: The specific aims of this umbrella review are to: (1) Evaluate the evidence of efficacy of FGAs and SGAs in anxiety disorders as an adjunctive treatment to traditional antidepressant treatments and other nonantipsychotic medications; (2) Compare monotherapy with antipsychotics to first-line treatments for anxiety disorders in terms of effectiveness, risks, and side effects.

Background: Individuals with hypertensive disorders of pregnancy (HDP) have an elevated lifetime risk of chronic hypertension, metabolic syndrome, and premature cardiovascular disease. Because breastfeeding duration and exclusivity have been associated in observational studies with improved cardiovascular health, optimizing breastfeeding in those with HDP might be an unrealized cardio-prevention approach, in particular because individuals with HDP have more breastfeeding challenges. Breastfeeding supportive interventions targeting one's breastfeeding self-efficacy have been shown to improve breastfeeding rates.

Introduction: Liver resection patients may be at an increased risk of local anesthetic (LA) toxicity because the liver is essential for metabolizing LA and producing proteins (mainly α-acid glycoprotein (AAG)) that bind to it and reduce the free (and pharmacologically active/toxic) levels in circulation. The liver resection itself, manipulation during surgery, and pre-existing liver disease may all interfere with normal hepatic protein synthesis and result in an attenuation of the increased AAG (a positive acute-phase protein) that normally occurs postoperatively. The purpose of this study was to determine whether the AAG response is attenuated postoperatively following liver resection and whether patients approach toxicity thresholds with continuous postoperative epidural infusion of bupivacaine.
