Current Situation of Non-cleft Craniofacial Anomalies in Thailand: A Multicenter Study.

J Craniofac Surg

January 2025

Division of Plastic Surgery, Department of Surgery, Faculty of Medicine, Prince of Songkla University, Hat Yai, Songkhla, Thailand.

Article Synopsis
  • - The study investigates the impact of non-cleft craniofacial anomalies on patients and their caregivers, focusing on experiences and challenges faced by caregivers across four major hospitals in Thailand over three years, involving 224 participants.
  • - Findings indicate a regional prevalence of anomalies, with craniosynostosis and clefts being the most common, and reveal a lack of family history for these conditions. Many patients faced multiple hospital stays, and despite frequent visits, a notable portion had not received necessary surgeries.
  • - Caregivers reported significant stress but largely did not view it as a burden, highlighting issues such as inadequate access to specialized care and a lack of support networks, indicating the need for improved healthcare strategies and resources tailored to regional

Four-Dimensional Impedance Manometry in Esophageal Motility Disorders.

Am J Gastroenterol

October 2024

Kenneth C. Griffin Esophageal Center of Northwestern Medicine, Department of Medicine, Division of Gastroenterology and Hepatology, Feinberg School of Medicine, Northwestern University, Chicago, Illinois, USA.

Introduction: Four-dimensional high-resolution impedance manometry (4D HRM) uses impedance to estimate esophageal luminal cross-sectional area and track nadir impedance to measure intrabolus pressure (IBP). We aimed to determine whether 4D HRM metrics could define abnormal esophagogastric junction (EGJ) opening between Chicago Classification version 4.0 motility disorders and compare 4D HRM with functional lumen imaging probe (FLIP) metrics.

Background: The prevalence of emergency department (ED) visits among the elderly is high and increasing. While emergency services for the elderly involve many factors, drug-related problems (DRPs) that can worsen patient conditions are less frequently discussed. This study investigates the prevalence of preventable drug-related ED visits (DRED) and the characteristics of DRPs in elderly ED patients through a comprehensive medication review.

Background: Previous house dust mite subcutaneous immunotherapy (HDM SCIT) placebo-controlled trials have small sample sizes and lack a consensus on baseline treatment.

Objective: To determine the efficacy of HDM SCIT in moderate-to-severe allergic rhinitis (AR) patients treated with an intranasal corticosteroid at baseline.

Methods: We conducted a randomized, placebo-controlled trial comparing HDM SCIT against placebo in Dermatophagoides pteronyssinus (Der p) sensitized.

The effect of corticosteroids in developing active pulmonary tuberculosis among patients with COVID-19.

PLoS One

October 2024

Department of Medicine, Faculty of Medicine, King Chulalongkorn Memorial Hospital, Thai Red Cross Society, Chulalongkorn University, Bangkok, Thailand.

Article Synopsis
  • * A study conducted at King Chulalongkorn Memorial Hospital analyzed 243 COVID-19 patients on high-dose corticosteroids, revealing that 1.6% developed TB within a year, with higher incidences among those with chronic kidney and lung diseases.
  • * The mortality rate for patients with TB was significantly higher (50%) compared to those without TB (0.4%), suggesting a need for routine screening for latent TB in COVID-19 patients receiving high-dose corticosteroids to prevent severe outcomes.

The histone acylation reader ENL/AF9 regulates aging in Drosophila melanogaster.

Neurobiol Aging

December 2024

Department of Pharmacology, Faculty of Medicine, Chiang Mai University, Chiang Mai, Thailand; Drosophila Centre for Human Diseases and Drug Discovery (DHD), Faculty of Medicine, Chiang Mai University, Chiang Mai, Thailand. Electronic address:

Histone acylation plays a pivotal role in modulating gene expression, ensuring proper neurogenesis and responsiveness to various signals. Recently, the evolutionary conserved YAF9, ENL, AF9, TAF41, SAS5 (YEATS) domain found in four human paralogs, has emerged as a new class of histone acylation reader with a preference for the bulkier crotonyl group lysine over acetylation. Despite advancements, the role of either histone crotonylation or its readers in neurons remains unclear.

  • Mutations in the Dynactin 1 (DCTN1) gene, particularly the p.Gly59Ser variant, are linked to neurodegenerative disorders, first identified in a family with specific motor issues.
  • * This study focuses on six Thai patients with the p.Gly59Ser mutation, revealing a mean onset age of 32.6 years and atypical symptoms such as tongue fasciculations and bilateral split-hands.
  • * The findings suggest that this mutation leads to more severe lower motor neuron complications, including vocal cord paralysis, and introduces the split-hand phenomenon as a significant clinical feature.*

A Rare Noncoding Enhancer Variant in Contributes to the High Prevalence of Brugada Syndrome in Thailand.

Circulation

January 2025

Department of Medicine, Center of Excellence in Arrhythmia Research (J.M., W.W., B.S., P.W., N.C., R.C., S.P., K.N., A.K.), Faculty of Medicine, Chulalongkorn University, Bangkok, Thailand.

Article Synopsis
  • Brugada syndrome (BrS) is a hereditary cardiac disorder linked to sudden death in young adults, particularly prevalent in Southeast Asia, with certain genetic variants associated with the condition.
  • Researchers conducted genome sequencing on individuals with BrS and matched controls in Thailand to find rare noncoding variants that are more common in BrS patients.
  • A specific rare variant was identified that disrupts a transcription factor binding site, causing reduced gene expression and reduced sodium current in heart cells, contributing to the high prevalence of BrS in the region and identifying at-risk individuals.

Background And Aims: Inhalation anesthesia suppresses the immune system and stimulates the growth of tumor cells, contrary to intravenous anesthesia. However, no consensus exists on which anesthetic technique is better for preventing cancer recurrence. Therefore, this study compared the effects of two different anesthetic techniques on natural killer cell cytotoxicity (NKCC) in hepatocellular carcinoma (HCC) patients undergoing open hepatic resection.
