J Foot Ankle Surg

August 2024

Division of Orthopedic Surgery, Department of Surgery, McMaster University, Hamilton ON, Canada.

Prescription opioids, particularly for treating musculoskeletal pain, are a significant contributor to the opioid epidemic in North America. An updated review on pathogenic coronaviruses (CoVs) amid the emergence of SARS-CoV-2 variants: A look into the repercussions and possible solutions.

J Infect Public Health

November 2023

Department of Microbiology, Punjab Agricultural University, Ludhiana 141004, India; Trafford College, Altrincham, Manchester WA14 5PQ, UK. Electronic address:

SARS-CoV-2, responsible for COVID-19, shares 79% and 50% of its identity with SARS-CoV-1 and MERS-CoV, respectively. It uses the same main cell attachment and entry receptor as SARS-CoV-1, which is the ACE-2 receptor. However, key residues in the receptor-binding domain of its S-protein seem to give it a stronger affinity for the receptor and a better ability to hide from the host immune system.

Article Synopsis
  • DBR1 is vital for lariat debranching and its deficiency leads to a severe immune disorder resulting in pediatric brainstem viral-induced encephalitis.
  • A specific variant in DBR1 has been linked to a new allelic disorder observed in four families, characterized by prematurity, severe growth issues, skin abnormalities, and high infant mortality.
  • Research on patient fibroblast cells revealed unique RNA changes and decreased DBR1 protein levels, suggesting this new disorder differs from the known encephalitis susceptibility associated with DBR1 deficiency.

  • Very long-chain fatty acids (VLCFAs) are crucial for cell membrane formation in the brain, skin, and retina, with the ELOVL4 enzyme playing a key role in their elongation beyond 28 carbon atoms.
  • The study involved clinical exome sequencing of individuals from four unrelated families with neuro-ichthyosis, leading to the discovery of three new ELOVL4 variants, including a significant exonic deletion found in two families from the same Saudi tribe.
  • Findings indicated that most affected individuals experienced severe developmental issues and neurological symptoms, highlighting the genetic diversity related to ELOVL4 and suggesting the presence of a tribal founder mutation among the families studied.

Background: Anti-N-methyl-d-aspartate "anti-NMDA" receptor encephalitis is one of the most common autoimmune encephalitis for which first- and second-line therapies have been recommended following international consensus. However, some refractory cases do not respond to the first- and second-line therapy and require further immune-modulatory therapies such as intra-thecal methotrexate. In this study, we reviewed six confirmed cases of refractory anti-NMDA receptor encephalitis from two tertiary centers in Saudi Arabia that required escalation of treatment and received a six-month course of intra-thecal methotrexate.
