Article Synopsis
  • The study explores a broader range of neuropsychiatric symptoms in patients with systemic autoimmune rheumatic diseases (SARDs), finding that self-reported symptoms are significantly more common in these patients compared to controls.
  • Researchers conducted comparisons between patients, clinicians, and a control group, using mixed methods to analyze 30 different neuropsychiatric symptoms and their prevalence in a large patient sample.
  • The findings reveal a substantial gap between patients’ self-reports of mental health issues and clinicians’ assessments, highlighting barriers to symptom identification such as lack of communication and misunderstanding of symptom visibility.

Aims: Specialist cardiology care is associated with a prognostic benefit in patients with heart failure (HF) with reduced ejection fraction (HFrEF) admitted with decompensated HF. However, up to one third of patients admitted with HF and normal ejection fraction (HFnEF) do not receive specialist cardiology input. Whether this has prognostic implications is unknown.

Treatment of Iron Deficiency in Heart Failure.

Curr Cardiol Rep

July 2023

Department of Cardiology, King's College Hospital London, Denmark Hill, Brixton, London, SE5 9RS, UK.

Purpose Of Review: Heart failure (HF) is commonly associated with iron deficiency (ID), defined as insufficient levels of iron to meet physiological demands. ID's association with anaemia is well understood but it is increasingly recognised as an important comorbidity in HF, even in the absence of anaemia. This review summarises contemporary evidence for the measurement and treatment of ID, in both HFrEF and HFpEF, and specific HF aetiologies, and highlights important gaps in the evidence-base.

Article Synopsis
  • Acute heart failure (AHF) is a complex condition influenced by both sudden triggers and existing heart issues, often linked to valvular heart disease (VHD).
  • Patients with AHF may show varying symptoms, ranging from mild heart failure to severe cases like cardiogenic shock, making it tough to assess their condition accurately.
  • Current treatment guidelines for AHF patients with severe VHD are unclear due to limited evidence from clinical trials, prompting a need for more research on this specific group.

Bilateral selective laryngeal reinnervation in patients with bilateral vocal cord palsy.

Acta Otorhinolaryngol Ital

June 2023

Department of Surgery, Unit of Otorhinolaryngology, Azienda Ospedaliero-Universitaria di Cagliari, University of Cagliari, Cagliari, Italy.

Objective: Bilateral selective reinnervation of the larynx aims to restore both vocal cord tone and abductor movements in patients with bilateral vocal cord palsy.

Methods: Four females and one male treated by bilateral selective reinnervation of the larynx were included in the present study. In all cases, both posterior cricoarytenoid muscles were reinnervated using the C3 right phrenic nerve root through the great auricular nerve graft, while adductor muscle tone was bilaterally restored using the thyrohyoid branches of the hypoglossal nerve through transverse cervical nerve grafts.

Aims: To evaluate the prevalence and associations of non-cardiac comorbidities (NCCs) with in-hospital and post-discharge outcomes in acute heart failure (AHF) across the ejection fraction (EF) spectrum.

Methods And Results: The 9326 AHF patients from European Society of Cardiology (ESC)-Heart Failure Association (HFA)-EURObservational Research Programme Heart Failure Long-Term Registry had complete information for the following 12 NCCs: anaemia, chronic obstructive pulmonary disease (COPD), diabetes, depression, hepatic dysfunction, renal dysfunction, malignancy, Parkinson's disease, peripheral vascular disease (PVD), rheumatoid arthritis, sleep apnoea, and stroke/transient ischaemic attack (TIA). Patients were classified by number of NCCs (0, 1, 2, 3, and ≥4).

Uterine rupture is a life-threatening peripartum complication. Spontaneous uterine rupture in early pregnancy is very rare. The diagnosis of uterine rupture should be considered when a pregnant patient presents with an acute abdomen because its clinical signs in early pregnancy are non-specific and the differentiation with other acute abdominal emergencies is challenging.

Background Growth differentiation factor 15 (GDF-15) is elevated in heart failure with preserved ejection fraction and is associated with adverse outcome, but its relationship with myocardial fibrosis and other characteristics remains unclear. We sought to evaluate the effect of pirfenidone, a novel antifibrotic agent, on GDF-15 in heart failure with preserved ejection fraction and identify characteristics that associate with GDF-15 and with change in GDF-15 over 1 year. Methods and Results Among patients enrolled (n=107) in the PIROUETTE (Pirfenidone in Patients With Heart Failure and Preserved Left Ventricular Ejection Fraction) trial, GDF-15 was measured at baseline and at prespecified time points in patients randomized (n=94) to pirfenidone or placebo.
