Dasatinib, a second-generation tyrosine kinase inhibitor, has been reported to have immunomodulatory effects. Epstein-Barr virus (EBV)-associated lymphoproliferative disorders (EBV-LPD) occur in immunocompromised patients, such as those receiving methotrexate or other immunosuppressive drugs or after allogenic transplantation. EBV-LPD is also reported to be a rare side effect in patients receiving long-term dasatinib or imatinib.

ETV6::LYN fusion gene is recognized as one of the genetic alterations responsible for myeloid/lymphoid neoplasms with eosinophilia and tyrosine kinase gene fusions (MLN-TK) according to the 2022 WHO classification. However, the clinical features and pathogenesis of MLN-TK with ETV6::LYN are not well defined because of the rarity of the disease. Here, we report an MLN-TK patient with ETV6::LYN that manifested as myeloproliferative neoplasms (MPN) with eosinophilia, myelofibrosis, and T-lymphoblastic lymphoma (T-LBL), which eventually led to acute myeloid leukemia.

Background: Despite the availability of advanced imaging technologies, it remains difficult to achieve sufficient staging accuracy to ensure a tailored treatment strategy for patients with upper tract urothelial carcinoma (UTUC). The aim of the study was to identify preoperative risk factors for tumor upstaging in patients with UTUC initially staged as clinical T2 or lower and to analyze these factors separately for renal pelvic cancer and ureteral cancer.

Methods: This retrospective study included data from patients with UTUC who underwent nephroureterectomy.

Successful Kidney Transplantation for Bilateral Renal Hypoplasia With Ebstein Disease.

Pediatr Transplant

February 2025

Department of Pediatrics, Graduate School of Medical Science, Kyushu University Hospital, Fukuoka, Japan.

Background: Patients with renal hypoplasia are often associated with congenital heart disease (CHD). During the perioperative period of kidney transplantation (KTx), sufficient circulation volume is required to maintain renal blood flow in the donor kidney. However, little is known about the indication and management of KTx in patients with CHD who require precise hemodynamic assessment during transplantation.

Background: Complex segmentectomy for uniportal video-assisted thoracoscopic surgery (VATS) remains controversial due to procedural complexity and the risk of increased complications, unlike multiportal VATS. Demonstrating the perioperative results and proficiency of individual surgeon is believed to influence future dissemination for uniportal VATS. In this study, we aimed to compare the perioperative outcomes and learning curves of complex versus simple segmentectomy for uniportal VATS.

Aims: In repaired tetralogy of Fallot (rTOF), the septal anatomical isthmuses (AI), AI 3, between the ventricular septal defect (VSD) and pulmonary annulus, and AI 4, between the VSD and tricuspid annulus, are important ventricular tachycardia (VT) substrates when slow conducting. Our aim was to assess the influence of VSD characteristics, specifically the presence of muscular or fibrous tissue at its border, on the presence or absence of septal AIs, potentially related to VT.

Methods And Results: All consecutive rTOF patients who underwent electroanatomical mapping between January 2005 and March 2023 with an available surgical report providing VSD details (n = 91) were included.

Objective: Loss-of-function mutations in the GIRDIN/CCDC88A gene cause developmental epileptic encephalopathy (DEE) in humans. However, its pathogenesis is largely unknown. Global knockout mice of the corresponding orthologous gene (gKOs) have a preweaning lethal phenotype with growth failure, preventing longitudinal analysis.

We aimed to elucidate the dynamic changes in short-chain fatty acids (SCFA) produced by the gut microbiota following smoking exposure and their role in chronic obstructive pulmonary disease (COPD) pathogenesis. SCFA concentrations were measured in human plasma, comparing non-smokers (n = 6) and smokers (n = 12). Using a mouse COPD model induced by cigarette smoke exposure or elastase-induced emphysema, we modulated SCFA levels through dietary interventions and antibiotics to evaluate their effects on inflammation and alveolar destruction.

Oxidative stress and chronic cerebral hypoperfusion: An overview from preclinical rodent models.

J Cereb Blood Flow Metab

December 2024

Neuroprotection Research Laboratories, Departments of Radiology and Neurology, Massachusetts General Hospital and Harvard Medical School, Boston, MA, USA.

Article Synopsis
  • - Chronic cerebral hypoperfusion (CCH) causes long-term reduced blood flow to the brain, leading to neurodegenerative diseases like Alzheimer's and vascular dementia, with rodent models helping to study its effects.
  • - CCH induces oxidative stress through cellular disruptions, resulting in damage to neurons and worsened cognitive function due to the buildup of reactive oxygen species (ROS).
  • - Edaravone, an antioxidant initially used for ischemic stroke, shows promise in rodent studies for protecting against neuronal damage by reducing oxidative stress in CCH conditions.

Introduction: Adult spinal Langerhans cell histiocytosis (LCH) presents a treatment challenge due to ongoing controversies. Traditional approaches such as curettage with bone grafting and internal fixation are preferred for severe cases involving mechanical instability, neurological deficits, or deformity. This study aimed to explore the efficacy of a customized approach involving simple posterior instrumentation without curettage or bone grafting in treating adult spinal LCH.

Prostate cancer (PC) growth is hormone-dependent and it frequently develops distant metastases as disease progresses. Patients with metastatic castration-sensitive prostate cancer (mCSPC) initially respond to androgen deprivation therapy (ADT) but eventually become refractory and develop metastatic castration-resistant prostate cancer (mCRPC). Castration-resistance is associated with high lethality and metastases confer poor prognosis, therefore unmet needs in treatment for mCSPC remain high.

Article Synopsis
  • Osteosarcopenia, a condition combining sarcopenia and osteopenia, is linked to poor survival rates in patients who have undergone biliary tract cancer (BTC) resection.
  • A study of 109 patients revealed that 28% had osteosarcopenia, which was associated with higher rates of lymph node metastasis and worse disease-free and overall survival outcomes.
  • The findings indicate that osteosarcopenia is an important prognostic factor for predicting adverse outcomes following BTC surgery.
