Background: Although intravenous tranexamic acid is used in cardiac surgery to reduce bleeding and transfusion, topical tranexamic acid results in lower plasma concentrations compared with intravenous tranexamic acid, which may lower the risk of seizures. We aimed to determine whether topical tranexamic acid reduces the risk of in-hospital seizure without increasing the risk of transfusion among cardiac surgery patients.

Methods: We conducted a multicenter, double dummy, blinded, randomized controlled trial of patients recruited by convenience sampling in academic hospitals undergoing cardiac surgery with cardiopulmonary bypass.

May 2024

BC Cancer-Sindi Ahluwalia Hawkins Centre, Dept. of Surgical Oncology, Kelowna, BC, Canada; Department of Surgery, Kelowna General Hospital, Kelowna, BC, Canada.

Background: Regional lymphadenectomy (RL) has traditionally been recommended in patients with melanoma found to have clinical lymphadenopathy or a positive sentinel lymph node biopsy (SLNB). Regional control of disease is still a relevant issue for patients, even after undergoing lymphadenectomy. The goal of this study was to identify the clinicopathologic characteristics that predict locoregional recurrence in patients who have undergone either therapeutic lymph node dissection (TLND) or completion lymph node dissection (CLND) following SLNB.

Article Synopsis
  • Female stress urinary incontinence (SUI) is common among women and significantly affects their quality of life, with suburethral slings being the most prevalent treatment option.
  • A survey revealed that while 68% of practitioners use midurethral slings, only 60.6% would consider removing the sling if complications arose, with various types of sling techniques being utilized.
  • The findings highlight the need for surgeons to discuss surgical risks and other treatment alternatives thoroughly with patients during the consent process, promoting a more diverse range of management options for female SUI.

Article Synopsis
  • In the AcT trial, researchers examined how effective and safe intravenous tenecteplase is compared to alteplase for treating patients with minor ischaemic strokes, specifically those with low NIHSS scores (≤5).
  • The study included 378 patients, showing that 51.8% of those treated with tenecteplase achieved good functional outcomes compared to 47.5% with alteplase, but the differences in outcomes were not statistically significant.
  • Safety results indicated no major differences between the two treatments in terms of severe complications like symptomatic intracranial hemorrhage or mortality within 90 days.

Quality of Life After Intravenous Thrombolysis for Acute Ischemic Stroke: Results From the AcT Randomized Controlled Trial.

Stroke

March 2024

Department of Community Health Sciences, University of Calgary, AB, Canada (T.T.S., O.I.A., A.A., M.A.A., S.B.C., A.M.D., M.D.H., B.L., B.K.M.).

Background: Recent evidence from thrombolysis trials indicates the noninferiority of intravenous tenecteplase to intravenous alteplase with respect to good functional outcomes in patients with acute stroke. We examined whether the health-related quality of life (HRQOL) of patients with acute stroke differs by the type of thrombolysis treatment received. In addition, we examined the association between the modified Rankin Scale score 0 to 1 and HRQOL and patient-reported return to prebaseline stroke functioning at 90 days.
