Global Challenges in the Access of Endovascular Treatment for Acute Ischemic Stroke (Global MT Access).

Background: Mechanical thrombectomy (MT) is the standard of care for eligible acute ischemic stroke (AIS) patients with large vessel occlusion (LVO) since 2015.

Aim: Our aim was to determine the key challenges for MT implementation and access worldwide.

Methods: We conducted an international online survey consisting of 37 questions, distributed through the World Stroke Organization network, and as invited by co-authors between December 2022 and March 2023.

COMPARING STAGES OF DIABETIC RETINOPATHY WITH SYSTEMIC VASCULAR STATUS USING FINGER PHOTOPLETHYSMOGRAPHY.

Purpose: To evaluate systemic vascular fitness measured by finger photoplethysmography in diabetic retinopathy (DR).

Methods: This was a cross-sectional observational study on patients with Type II diabetes mellitus from October 2020 to May 2021. Data collected include systolic/diastolic blood pressure, visual acuity, glycated hemoglobin, and central macular thickness.

Intraocular Pressure Fluctuation as Assessed by Water Drinking Test following iStent Inject Implantation in Open-Angle Glaucoma Patients.

Introduction: This study investigated the intraocular pressure (IOP) fluctuation as assessed by the water drinking test (WDT) in open-angle glaucoma (OAG) patients after combined cataract surgery with iStent implantation.

Methods: This was a prospective non-randomized comparative study. Eyes with OAG and cataracts that were planned for either combined phacoemulsification and iStent implantation (iStent+CS) or phacoemulsification alone (CS) were recruited.

Post-tracheostomy Bleeding: A Case Report on a Rare Variation of the Inferior Thyroid Artery.

This case report describes post-tracheostomy bleeding resulting from a rare occurrence of an aberrant course of the inferior thyroid artery (ITA), which arose directly from the common carotid artery (CCA). The patient, a 68-year-old male with severe COVID-19 complications, had undergone a tracheostomy due to prolonged intubation. Postoperatively, significant bleeding occurred from the tracheostomy site.

Comparing ergosterol identification by HPLC with fungal serology in human sera.

Background: Ergosterol, a predominant sterol in fungal cell membranes, holds promise as a specific marker for detecting fungal presence in human samples. This study investigated the performance of ergosterol detection compared to serological tests in identifying the presence of fungi in human sera.

Methods: Eighty-four non-duplicate human sera were analyzed by high performance liquid chromatography (HPLC) for ergosterol detection.

Family of juvenile X-linked retinoschisis with varied presentation: a case series with RS1 genetic analysis.

RS1 gene mutations are known to be a direct cause of the hereditary retinopathy known as retinoschisis. We describe a group of 3 siblings with the same RS1 gene mutation who presented with different retinopathy phenotypes. Genetic testing confirmed the RS1 genotypes.

A Case Series of Eye Removal Surgeries: Five Years of Experience at a Tertiary Eye Care Centre in Malaysia.

Background: Eye removal surgeries, also called anophthalmic surgeries, are usually performed for a painful blinded eye due to various underlying causes. In this case review, we intended to study the indications, the types of anophthalmic surgeries, and post-operative complications related to eye removal surgeries.

Method:  Five years of retrospective case review of surgical eye removals was conducted from 1st June 2018 to 31st May 2023 at Hospital Canselor Tuanku Muhriz (HCTM), University of Kebangsaan Malaysia (UKM).

Health-Related Quality of Life and Economic Analysis of Olanzapine Versus Aprepitant in Preventing Chemotherapy-Induced Nausea and Vomiting in Patients Receiving Highly Emetogenic Chemotherapy in Malaysia.

Objectives: Olanzapine has been shown to be effective in preventing chemotherapy-induced nausea and vomiting (CINV) after highly emetogenic chemotherapy (HEC); however, there is limited work on the impact of CINV on health-related quality of life (HRQoL) and the comparative cost-effectiveness of CINV prophylaxis in the Malaysian context. Therefore, this study was conducted to determine the HRQoL using EQ-5D-5L and the cost-effectiveness of olanzapine compared with aprepitant for CINV prophylaxis in Malaysia using data from a local study.

Methods: Fifty-nine chemo-naive patients receiving either olanzapine or aprepitant were randomly recruited and completed the EQ-5D-5L before and day 5 after HEC.

Negative Pressure Wound Therapy in Pharyngocutaneous Fistula.

Pharyngocutaneous fistula (PCF) is an abnormal connection between the pharynx and skin that can occur after laryngectomy surgery. It can have a significant negative impact on patient recovery, delaying wound healing, requiring prolonged nil-per-oral (NPO) status, and reducing quality of life. Traditionally, the management of PCF has relied on conservative measures or surgical intervention.

Magnetic Resonance Imaging after Nasopharyngeal Endoscopic Resection and Skull Base Reconstruction.

Postoperative imaging after nasopharyngeal endoscopic resection (NER) and skull base reconstruction is quite challenging due to the complexity of the post-surgical and regional anatomy. In this retrospective observational study, we included patients treated with NER from 2009 to 2019 and submitted to Magnetic Resonance Imaging (MRI) 6 and 12 months after surgery. A radiologist with 15 years of experience analyzed all MRI scans.

Aspirin for preeclampsia prevention in low- and middle-income countries: mind the gaps.

Preeclampsia is a syndrome that continues to be a major contributor to maternal and neonatal mortality, especially in low-income countries. The incidence of splenic abscess associated with infective endocarditis varies between 1-10% of cases, and its presence may indicate a severe form of the disease. We present a 24-year-old man diagnosed with infective endocarditis who was found to have a splenic abscess upon further evaluation.

Introduction: Workplace bullying (WPB) among trainee doctors is a concerning problem in Malaysia. However, there is still limited understanding regarding the influence of trainee doctors' personality traits on WPB. Furthermore, the impact of contract employment status on WPB among trainee doctors is not yet well-defined.

Tiptoe Walking in the Presence of Superimposed Aeromonas With Dengue Infection.

The rise in limb salvage surgeries has led to an increase in post-traumatic equinus contractures, resulting in secondary causes of tiptoe walking. There is notable evidence linking superimposed bacteremia to heightened risks of morbidity and mortality in dengue patients, particularly when the immune system is compromised, leading to documented cases of -induced necrotizing fasciitis with myonecrosis. A case report from August 2019 involves a five-year-old boy admitted for dengue fever, which progressed to severe bilateral leg necrotizing fasciitis caused by concurrent infection.

Synchronous primary malignancies of papillary thyroid carcinoma and Hodgkin lymphoma: Interventions and outcome.

Interprofessional collaborative care (IPCC) can improve the quality of care in patients with chronic diseases in primary care settings. In Malaysia, many medical and healthcare universities have adopted the concept of the interprofessional collaborative practice (IPCP) framework by the World Health Organization (WHO) and implemented interprofessional learning (IPL) in their curriculum to prepare fresh graduates for interprofessional collaboration (IPC) in the health workforce albeit in various degrees. However, there are potential challenges in putting what they have learned into practice, especially in managing chronic diseases due to the complexity of behavior changes required.

Magnetic resonance imaging T2 of the pancreas value using an online software tool and correlate with T2 value of myocardium and liver among patients with transfusion-dependent thalassemia major.

Objective: Patients with thalassemia major do require lifetime blood transfusions that eventually result in iron accumulation in different organs. We described the usefulness of using magnetic resonance imaging (MRI) T2imaging values for the evaluation of pancreatic iron load in these patients, and we correlated it with MRI T2 haemosiderosis of the myocardium and liver that has been recognized as a non-invasive assessment of iron overload among patients with thalassemia major.

Materials And Methods: We conducted a cross-sectional study on 39 patients with thalassemia major in one of the tertiary university hospitals for a 1-year period.
