Fatty acid binding protein 4 (FABP4) is highly expressed in adipocytes. Lipolysis, caused by an elevated adrenergic input, has been suggested to contribute to elevated serum FABP4 levels in patients with cardiovascular diseases. However, the relationship between the serum FABP4 and efferent sympathetic nerve activity remains poorly understood.

Background: Allergic bronchopulmonary mycosis (ABPM) is an allergic disease caused by type I and type III hypersensitivity to environmental fungi. Schizophyllum commune, a basidiomycete fungus, is one of the most common fungi that causes non-Aspergillus ABPM.

Objective: Herein, we attempted to clarify the clinical characteristics of ABPM caused by S.

Olfactory and taste dysfunctions caused by COVID-19: a nationwide study.

Rhinology

December 2023

Department of Respiratory Medicine and Allergology, Kochi Medical School, Kochi University, Kochi, Japan.

Background: Olfactory dysfunctions (OD) and taste dysfunctions (TD) are widely recognized as characteristic symptoms of COVID-19; however, the frequency and mode of occurrence has varied depending on the viral mutation. The prevalence and characteristics of OD/TD in Japan have not been definitively investigated. The purpose of this study is to assess the prevalence of OD/TD in Japan during the Alpha variant epidemic, and measure symptom prolongation at 6 months and 1 year later following initial infection.

We report a rare case of adenosquamous carcinoma of the gallbladder which simultaneously produces granulocyte-colony-stimulating factor (G-CSF) and parathyroid hormone-related protein (PTHrP), confirmed serologically and histologically. A 71-year-old man was examined for a gallbladder tumor with multiple lymph nodes and liver metastases. Histopathological evaluation by endoscopic ultrasound fine-needle aspiration revealed adenosquamous carcinoma of the gallbladder.

Endobronchial tuberculosis often causes bronchial stenosis. Balloon dilation is a minimally invasive and effective bronchoscopic intervention for bronchial stenosis; however, reports on balloon dilation in older individuals are limited. We present a case of a 77-year-old woman with endobronchial tuberculosis and clarify the efficacy and safety of balloon dilation.

, hosted by hogs, is the most prevalent gastric non- species found in humans. Recent studies have suggested that infection has caused many cases of gastric disease, but the transmission route from hogs remains unclear. Diagnostic methods based on urease activity often yield negative results, and there is no reliable method for diagnosing infection in clinical practice without gastric biopsy specimens.

Background: Endoscopic gastroduodenal stent (GDS) placement is widely used as a safe and effective method to rapidly improve gastrointestinal symptoms of malignant gastric outlet obstruction (MGOO). While previous studies reported the utility of chemotherapy after GDS placement for prognosis improvement, they did not fully address the issue of immortal time bias.

Objectives: To examine the association between prognosis and clinical course following endoscopic GDS placement, using a time-dependent analysis.

Background: Coronavirus disease (COVID-19), an infectious disease caused by the novel coronavirus severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has spread worldwide since early 2020, and there are still no signs of resolution. The Japanese Clinical Practice Guidelines for the Management of Sepsis and Septic Shock (J-SSCG) 2020 Special Committee created the Japanese Rapid/Living recommendations on drug management for COVID-19 using the experience of creating the J-SSCG.

Methods: The Grades of Recommendation, Assessment, Development, and Evaluation (GRADE) approach was used to determine the certainty of the evidence and strength of recommendations.

Purpose: Catheter ablation (CA) to isolate the pulmonary vein, which is an established treatment for atrial fibrillation (AF), is associated with left atrium reverse remodeling (LARR). The intrinsic cardiac autonomic nervous system includes the ganglion plexi adjacent to the pulmonary vein in the left atrium (LA). However, little is known about the effect of CA on the relationship between LARR and sympathetic nerve activity in patients with AF.

The arterial velocity pulse index (AVI) and arterial pressure-volume index (API) have been proposed as new arterial stiffness indices that can be measured using an oscillometric cuff. Sympathetic nerve activity (SNA) contributes to arterial stiffness via increasing vascular smooth muscle tone. However, the associations between SNA and the AVI or API are not understood.

Article Synopsis
  • The study investigates the relationship between serum KL-6 levels and the progression of systemic sclerosis-associated interstitial lung disease (SSc-ILD) in 77 patients over a 5-year period.
  • Results showed that 51% of patients experienced disease progression, with higher KL-6 levels correlating with this progression.
  • Changes in KL-6 levels were identified as significant independent predictors of disease progression, providing a potential biomarker for assessing patient outcomes in SSc-ILD.

Article Synopsis
  • The text discusses the ongoing global impact of COVID-19 since early 2020 and details the Japanese Clinical Practice Guidelines for managing the disease, specifically focusing on drug recommendations.
  • It outlines the methodology of creating these guidelines using the GRADE approach to assess evidence and establish recommendations, with the first edition released in September 2020 and a revised version in September 2021.
  • Key recommendations include the usage of remdesivir and corticosteroids for moderate to severe COVID-19 cases, while favipiravir is not recommended for all patients and specific guidelines are provided for other drugs like tocilizumab and anticoagulants.
