, hosted by hogs, is the most prevalent gastric non- species found in humans. Recent studies have suggested that infection has caused many cases of gastric disease, but the transmission route from hogs remains unclear. Diagnostic methods based on urease activity often yield negative results, and there is no reliable method for diagnosing infection in clinical practice without gastric biopsy specimens.

Rationale: Chronic eosinophilic pneumonia (CEP) presents eosinophil infiltrations in the lung due to allergic reactions. Most CEP patients continue to take glucocorticoids, and their prolonged use induces various side effects. In this case report, based on the efficacy of baricitinib in patients with rheumatoid arthritis (RA) and CEP, we aimed to show that the administration of Janus kinase (JAK) inhibitors, when RA is complicated by an allergic disease, can stabilize the disease state and help avoid the adverse effects of long-term systemic glucocorticoid administration.

Article Synopsis
  • The Japanese diagnostic criteria for autoimmune gastritis (AIG) were created by a workshop affiliated with JGES and CARP to standardize diagnosis based on endoscopic and histological findings, along with the presence of gastric autoantibodies.
  • In diagnosing early-stage AIG, only histological confirmation and positivity for gastric autoantibodies are required, as endoscopic data is still being collected.
  • The new criteria aim to improve diagnosis rates, allow better patient stratification for risks like gastric tumors, and establish a surveillance system, but challenges remain in validating endoscopic findings and securing insurance for antibody testing.*

Objectives: To validate the correlation between laboratory markers reflecting disease activity of macrophage activation syndrome (MAS) and serum cytokine levels and identify the valuable laboratory markers that change over time for a prompt MAS diagnosis.

Methods: Serum cytokine levels were determined by enzyme-linked immunosorbent assay and compared with laboratory markers reflecting MAS disease activity. The changes in values were evaluated from the acute phase of systemic juvenile idiopathic arthritis (s-JIA) to MAS diagnosis.

Approximately 50% of autosomal dominant polycystic kidney disease (ADPKD) patients have gross hematuria, but few cases of bladder cancer complications are known. We report a case of a 49-year-old female ADPKD patient with bladder cancer, who was presented to our hospital 4 months after the onset of gross hematuria. A computed tomography (CT) scan showed a bladder mass, enlarged pelvic and left inguinal lymph nodes, multiple liver cysts, and a polycystic kidney.

Background/aim: Most paratesticular liposarcomas (PLPSs) are well-differentiated liposarcomas (WDLPSs) with favourable prognoses. As such, the rare occurrence of PLPS often leads to its misdiagnosis as a hernia or hydrocele on physical examination. Curative resection of the tumour may not be possible in cases where PLPSs have transformed into dedifferentiated liposarcomas (DDLPSs) owing to a delay in diagnosis.

Article Synopsis
  • Early diagnosis of systemic juvenile idiopathic arthritis (s-JIA) is crucial for effective treatment, but it's often difficult as patients may not present with arthritis initially.
  • Serum ferritin levels have been traditionally used to help diagnose s-JIA, but they aren't definitive, complicating clinical diagnosis.
  • This report suggests that serum interleukin (IL)-18 levels rise before ferritin in early s-JIA cases, proposing IL-18 as a potentially better biomarker for early diagnosis.

Background: Severe odontogenic infections in the head and neck region, especially necrotizing soft tissue infection (NSTI) and deep neck abscess, are potentially fatal due to their delayed diagnosis and treatment. Clinically, it is often difficult to distinguish NSTI and deep neck abscess in its early stage from cellulitis, and the decision to perform contrast-enhanced computed tomography imaging for detection is often a challenge. This retrospective case-control study aimed to examine the utility of routine blood tests as an adjunctive diagnostic tool for NSTI in the head and neck region and deep neck abscesses.

Tertiary Emergency Medical Center (Trauma and Critical Center), Tokyo Metropolitan Bokutoh Hospital, 4-23-15, Kotobashi, Sumida-ku, Tokyo, 130-8575, Japan.

Article Synopsis
  • Streptococcus suis is a gram-positive bacterium that can cause serious infections like bacterial meningitis and sepsis in humans, often transmitted through raw pork or exposure to pigs.
  • A case study highlights a 38-year-old woman who developed septic shock after consuming raw pork, and although medical interventions were aggressive, her condition worsened rapidly.
  • The report emphasizes the importance of timely recognition of such zoonotic pathogens and suggests that peripheral blood smears can help diagnose infections even before blood culture results are available.

Objectives: Chemoradiotherapy (CRT) or radiotherapy (RT) alone is often the treatment of choice for elderly patients with esophageal cancer with the expectation of organ preservation. However, salvage treatment remains a problem when endoscopic resection is not indicated for local failure after CRT/RT. Photodynamic therapy (PDT) is indicated for local failure after CRT/RT, but there are few reports on its efficacy and safety in elderly patients.

Article Synopsis
  • Lower third molar extraction is a common dental surgery, and while the procedures are routine, their difficulty and associated complications can vary based on individual patient factors.
  • A study involving 1,826 extractions in Japan found that the overall prevalence of postoperative complications was 10.0%.
  • Key risk factors for complications included older age (over 32 years), radiographic relationships of tooth roots with the mandibular canal, and the depth of tooth impaction, indicating that older patients with deeply impacted teeth may face higher risks after surgery.
