Quality Indicators for the Detection and Removal of Colorectal Polyps and Interventions to Improve Them.

Gastrointest Endosc Clin N Am

April 2022

Division of Gastroenterology and Hepatology, Northwestern University Feinberg School of Medicine, 676 North Street, Clair, Suite 1400, Chicago, IL 60611, USA. Electronic address:

Modifiable risk factors for postcolonoscopy colorectal cancer include suboptimal lesion detection (missed neoplasms) and inadequate lesion removal (incomplete polypectomy) during colonoscopy. Competent detection and removal of colorectal polyps are thus fundamental to ensuring adequate colonoscopy quality. Several well-researched quality metrics for polyp detection have been implemented into clinical practice, chief among these the adenoma detection rate.

Introduction: Despite studies showing improved safety, efficacy, and cost-effectiveness of endoscopic resection for nonmalignant colorectal polyps, colectomy rates for nonmalignant colorectal polyps have been increasing in the United States and Europe. Given this alarming trend, we aimed to investigate whether colectomy rates for nonmalignant colorectal polyps are increasing or declining in a large, integrated, community-based healthcare system with access to advanced endoscopic resection procedures.

Methods: We identified all individuals aged 50-85 years who underwent a colonoscopy between 2008 and 2018 and were diagnosed with a nonmalignant colorectal polyp(s) at the Kaiser Permanente Northern California integrated healthcare system.

Article Synopsis
  • This study evaluates the accuracy of a mortality prediction model for patients undergoing revascularization due to chronic limb-threatening ischemia (CLTI), comparing expected outcomes with those observed in a specific medical center.
  • Analyzing 195 procedures on 169 patients, the research found significant differences in comorbidities and patient demographics compared to the Vascular Quality Initiative (VQI) database, including higher rates of unstable angina and dialysis dependence.
  • While the observed 30-day mortality rates were higher than the VQI estimates across all risk groups, the study concluded that the VQI model still effectively stratified patients based on mortality risk.

Division of Research, Kaiser Permanente Northern California, Oakland, CA USA; Department of Health Systems Science, Kaiser Permanente Bernard J. Tyson School of Medicine, Pasadena, CA USA; Departments of Epidemiology, Biostatistics and Medicine, University of California, San Francisco, CA USA; Department of Medicine, Stanford University, Palo Alto, CA USA.

Background: Adults with chronic kidney disease (CKD) are at increased risk of heart failure (HF) morbidity and mortality. Despite well-characterized abnormalities in cardiac structure in CKD, it remains unclear how to optimally leverage echocardiography to risk stratify CKD patients.

Methods: We evaluated associations between echocardiographic parameters and risk of HF hospitalization and death using Cox proportional hazard models and forward selection with integrated discrimination improvement (IDI).

Objective: Patients with risk factors for or established atherosclerotic cardiovascular disease (ASCVD) remain at high risk for subsequent ischemic events despite statin therapy. Triglyceride (TG) levels may contribute to residual ASCVD risk, and the performance of global risk assessment calculators across a broad range of TG levels is unknown.

Methods: We performed a retrospective cohort study of Kaiser Permanente Northern California members aged ≥45 years with ≥1 ASCVD risk factor (primary prevention cohort) or established ASCVD (secondary prevention cohort) between 2010 and 2017 who were receiving statin therapy and had a low-density lipoprotein cholesterol between 41-100 mg/dL.

Article Synopsis
  • RAAS inhibitors (RAASi) are commonly used in COVID-19 patients, and this study aimed to assess their impact on short-term outcomes when continued versus discontinued during hospitalization.
  • The trial involved 46 participants, revealing that the difference in clinical outcomes between continuing and stopping RAASi was not significant, but stopping them showed a higher risk of adverse effects and increased biomarkers indicating heart stress.
  • The study concluded that continuing RAASi in hospitalized COVID-19 patients appears to be safe and may help prevent complications like acute heart failure.
