Background: Myocardial bridging is a cardiac anomaly where a segment of epicardial coronary arteries runs through the myocardium and can rarely cause MI. Takotsubo syndrome is a stress-induced cardiomyopathy that can mimic MI. Catecholamine surge during stress can contribute to Takotsubo syndrome, but whether this surge can trigger an inconspicuous myocardial bridging to manifest symptomatically remains unclear, and alternately, whether a myocardial bridge might cause worsening of Takotsubo syndrome is also a matter that needs further research.

Introduction: The burden of multimorbidity is recognised increasingly in low- and middle-income countries (LMICs), creating a strong emphasis on the need for effective evidence-based interventions. Core outcome sets (COS) appropriate for the study of multimorbidity in LMICs do not presently exist. These are required to standardise reporting and contribute to a consistent and cohesive evidence-base to inform policy and practice.

Association between bronchial asthma and TSLP gene polymorphism: a systematic review and meta-analysis.

Ann Med Surg (Lond)

August 2024

Division of Pulmonary Disease and Critical Care Medicine, University of Kentucky, College of Medicine, Bowling Green, Kentucky.

Aims: This study entails an association between bronchial asthma and common single nucleotide polymorphisms (SNPs) in thymic stromal lymphopoietin (TSLP) gene; rs2289278, rs3806933, rs2289276, and rs1837253.

Methods: The databases of PubMed, Embase, Web of Science, and Google Scholar were searched for studies reporting TSLP polymorphisms and asthma from inception to January 2022. Hardy-Weinberg equilibriums (HWE) for each polymorphism of the control group were checked using the test.

Introduction: Methicillin-resistant Staphylococcus aureus (MRSA) expresses the Panton-Valentine leukocidin (PVL) virulence gene, which is associated with community and hospital-acquired severe MRSA infections. The objective of this study was to determine the prevalence and antibiotic susceptibility profile with a focus on the presence of the PVL gene among MRSA isolates in healthcare settings.

Methodology: A total of 1,207 clinical specimens and 304 hospital environment swabs were collected in a tertiary care hospital in Nepal, and investigated following basic microbiological techniques.

Background: Sodium glucose cotransporter 2 inhibitors are recommended for the treatment of heart failure due to their cardioprotective effects, despite primarily being used as antidiabetic medications. However, the comparative profile of two antidiabetic drugs, sodium glucose cotransporter 2 inhibitors with dipeptidyl peptidase 4 inhibitor remains unclear.

Study Hypothesis: This study aims to compare the safety and efficacy profiles of sodium glucose cotransporter 2 inhibitors versus dipeptidyl peptidase 4 inhibitor drugs.

Lateral medullary syndrome, resulting from cerebellar/brainstem infarction, can occur due to cardioembolic stroke from atrial fibrillation caused by rheumatic heart disease. This rare association highlights the importance of strict arrhythmia management, prophylactic anticoagulation, and timely diagnosis to prevent debilitating neurological outcomes.

Article Synopsis
  • A 43-year-old male with a history of Graves' disease developed neurological symptoms similar to a stroke, prompting an extensive evaluation for vasculitis and lymphoproliferative disease.
  • Imaging revealed narrowing of the left middle cerebral artery, leading to a diagnosis of probable cerebral vasculitis and treatment with methylprednisolone and cyclophosphamide.
  • Despite ongoing symptoms and further imaging showing additional narrowing, the treatment stabilized his condition, highlighting the diagnostic challenges of drug-induced ANCA-associated vasculitis.

Article Synopsis
  • A systematic review and meta-analysis were conducted to explore the risk of cardiac complications, like arrhythmias and heart failure, in patients with ischemic stroke (pwIS).
  • The analysis included data from 5 studies, involving over 106,000 participants, and found that pwIS are significantly more likely to experience arrhythmias (with a hazard ratio of 4.2) and heart failure (hazard ratio of 2.9).
  • The study concluded that pwIS have a higher risk of cardiac issues, but further research is needed to better understand the extent of these complications.

Introduction: Median arcuate ligament syndrome (MALS) is a rare condition caused by the compression of the celiac trunk by the median arcuate ligament, leading to a typical symptom triad: postprandial abdominal pain, weight loss, nausea, and vomiting.

Case Presentation: A 41-year-old female patient presented to our center with mild postprandial abdominal pain over the epigastric region, and bloating sensation. Ultrasonography of the abdomen showed multiple stones in the gall bladder lumen, and the computed tomography scan showed median arcuate ligament impingement along the proximal aspect of the celiac trunk causing moderate narrowing with post-stenotic dilation.
