Background: The short-term and midterm impact of gender differences on transcatheter aortic valve implantation (TAVI) has been studied. However, the impact on long-term clinical outcomes remains unclear. The objective of the study was to investigate the impact of gender differences after TAVI on long-term clinical outcomes and structural valve deterioration (SVD).

Association of Preoperative Physical Fitness With Post-Esophagectomy Pneumonia in Older With Locally Advanced Esophageal Cancer: An Exploratory Prospective Study.

Backgrounds: Currently, there is a lack of evidence of prehabilitation during neoadjuvant chemotherapy (NAC) to prevent pneumonia of older patients. This study aimed to investigate the association of preoperative physical fitness after NAC with post-esophagectomy pneumonia in older patients with locally advanced esophageal cancer (LAEC).

Methods: This single-center exploratory prospective cohort study included 80 patients aged ≥ 65 years with LAEC scheduled for curative esophagectomy after NAC between 2021 and 2023.

Chronic complete spinal cord injury (SCI) is difficult to treat because of scar formation and cavitary lesions. While human iPS cell-derived neural stem/progenitor cell (hNS/PC) therapy shows promise, its efficacy is limited without the structural support needed to address cavitary lesions. Our study investigated a combined approach involving surgical scar resection, decellularized extracellular matrix (dECM) hydrogel as a scaffold, and hNS/PC transplantation.

A 33-year-old Japanese man with a history of atopic dermatitis and asthma had never been diagnosed with any apparent glucose intolerance but had been aware of palpitations for >10 years. A 75g oral glucose tolerance test (OGTT) at his physical examination in March 2021 revealed fasting hyperglycemia and post-load hypoglycemia. An OGTT recheck was performed in May 2021 and was normal.

Evaluation of the Efficacy and Accuracy of Super-Flexible Three-Dimensional Heart Models of Congenital Heart Disease Made via Stereolithography Printing and Vacuum Casting: A Multicenter Clinical Trial.

Three-dimensional (3D) printing is an advanced technology for accurately understanding anatomy and supporting the successful surgical management of complex congenital heart disease (CHD). We aimed to evaluate whether our super-flexible 3D heart models could facilitate preoperative decision-making and surgical simulation for complex CHD. The super-flexible heart models were fabricated by stereolithography 3D printing of the internal and external contours of the heart from cardiac computed tomography (CT) data, followed by vacuum casting with a polyurethane material similar in elasticity to a child's heart.

Photodynamic Therapy Using IR-783 Liposomes for Advanced Tongue and Breast Cancers in Humans.

Photodynamic therapy (PDT) is a minimally invasive treatment that elicits tumor apoptosis using laser light exclusively applied to the tumor site. IR-783, a heptamethine cyanine (HMC) dye, impedes the proliferation of breast cancer cells, even without light. Although studies have investigated the efficacy of IR-783 in cell and animal studies, its efficacy in clinical settings remains unknown.

Background: A geographical analysis could be employed to uncover social risk factors and interventions linked to chronic pain. Nonetheless, geographical variation in chronic pain across different regions of Japan have not been well explored. This study aims to investigate geographical variation in high-impact chronic pain (HICP), defined as moderate to severe chronic pain, and examine the associated psychological factors at the prefecture level.

Development of a quantitative assessment for abnormal flexor synergy index in patients with stroke: a validity and responsiveness study.

Background: Arm-lifting movements (shoulder flexion) are essential for upper extremity rehabilitation after a stroke. Abnormal flexor synergy (elbow flexion) is frequently observed during shoulder flexion, impeding functional improvement. However, no quantitative method exists for assessing abnormal flexor synergy.

Impact of COVID-19 on hospital visit behaviour in cancer patients in Japan: a nationwide study.

Effectiveness of sequential bilateral repetitive transcranial stimulation versus bilateral theta burst stimulation for patients with treatment-resistant depression (BEAT-D): A randomized non-inferiority clinical trial.

Background: Bilateral repetitive transcranial magnetic stimulation (BL-rTMS) over the dorsolateral prefrontal cortex is effective for treatment-resistant depression (TRD). Owing to a shorter treatment time, bilateral theta burst stimulation (BL-TBS) can be more efficient protocol. The non-inferiority of BL-TBS to BL-rTMS was established in late-life TRD; however, this has not been determined in adults of other age groups.

Japanese clinical practice guidelines for oral cancer, 2023.

The Japanese Society of Oral Oncology and Japanese Society of Oral and Maxillofacial Surgeons have jointly developed clinical practice guidelines for oral cancer (oral squamous cell carcinoma) to improve and standardize the quality of oral cancer treatment in Japan. The first, second, and third editions were published in 2009, 2013, and 2019, respectively, and the 2023 edition was recently developed. In the development of the 2023 edition, 60 clinical questions (CQs) were listed.

A 50-year-old man presented to our hospital with a fever, edema, and a rash. The clinical diagnosis was renal dysfunction, nephrotic syndrome, and syphilis. The patient was treated with benzylpenicillin, and his symptoms improved.

Upper gastrointestinal triple stenosis in a patient with trisomy 17p syndrome: Case report and literature review.

Upper gastrointestinal stenosis, which can be congenital or acquired, can lead to dysphagia. The association between trisomy 17p syndrome, a rare chromosomal abnormality, and upper gastrointestinal stenosis is unclear. A 20-year-old man diagnosed with trisomy 17p syndrome was referred to our department due to recurrent vomiting.

Silver-Russell syndrome (SRS) is a syndrome characterized by prenatal and postnatal growth retardation, facial features, and body asymmetry. SRS is often complicated with hypoglycemia, whose etiology is unclear. We describe the clinical course of 25-year-old man with hypoglycemia.

The relationship between long COVID, labor productivity, and socioeconomic losses in Japan: A cohort study.

Objectives: We examined shifts in labor productivity and their economic ramifications among adult patients with long COVID in Japan.

Methods: A total of 396 patients were categorized into three groups based on symptom progression: non-long COVID, long COVID recovered, and long COVID persistent. Patient-reported outcomes were assessed at three time intervals: 3, 6, and 12 months after COVID-19 diagnosis.

Could video game-based physical rehabilitation substitute for conventional physiotherapy in patients with glioma? A proof-of-concept study.

Purpose: The present study investigated the clinical feasibility of replacing a part of conventional physiotherapy (PT) with Nintendo Wii® for the recovery of motor function and activities of daily living (ADL) in patients with glioma.

Methods: This study included 10 patients with first-episode gliomas who were admitted to the neurosurgery department of a tertiary hospital. According to the patients' preferences, they were allocated to conventional PT or Wii® rehabilitation groups in which a part of the conventional PT sessions were replaced with Wii® training.

Long-term efficacy of sarilumab on the progression of interstitial lung disease in rheumatoid arthritis: the KEIO-RA cohort and literature review.

Objectives: To clarify the impact of sarilumab (SAR) on the progression of interstitial lung disease (ILD) in patients with rheumatoid arthritis (RA).

Methods: We conducted a retrospective review of all consecutive RA patients from the KEIO-RA cohort who visited our institution between 2018 and 2024 and received SAR treatment. Patients were followed for 24 months from the initiation of SAR.
