Article Synopsis
  • Haematology patients experiencing high-risk neutropenia are susceptible to bloodstream infections linked to mucosal barrier injuries, particularly during episodes of fever.
  • A study involving 416 neutropenic haematology patients identified risk factors for these infections, noting that certain conditions like low MASCC scores and specific fungal colonizations significantly increased risk.
  • Findings revealed that while low citrulline levels at fever onset correlated with candidaemia, they did not indicate the likelihood of bacterial infections; moreover, quinolone antibiotics appeared beneficial.

Background & Aims: Although primary biliary cholangitis (PBC) is considered a rare disorder, accurate determination of its incidence and prevalence remains challenging due to limited comprehensive population-based registries. We aimed to assess the incidence and prevalence of PBC in the Netherlands over time through the nationwide Dutch PBC Cohort Study (DPCS).

Methods: DPCS retrospectively included every identifiable patient with PBC in the Netherlands from 1990 onwards in all 71 Dutch hospitals.

Article Synopsis
  • The study investigates the challenges of diagnosing and treating leptomeningeal metastases (LM) in patients with EGFR mutation positive non-small cell lung cancer (NSCLC) who are being treated with osimertinib, focusing on identifying resistance mechanisms in cerebrospinal fluid (CSF) and plasma.
  • A group of 28 patients was analyzed, with a high detection rate of the driver mutation in CSF, but resistance mechanisms were found in 27%, indicating limitations in treatment options available at this time.
  • After four weeks on an escalated osimertinib dosage, many patients showed stabilization or worsening of symptoms, suggesting that while some had radiological improvement, overall clinical efficacy remains

The association of inflammatory markers with frailty and in-hospital mortality in older COVID-19 patients.

Exp Gerontol

October 2024

Department of Internal Medicine, Section Geriatric Medicine, Amsterdam UMC, Amsterdam, the Netherlands.

Introduction: During the COVID19 pandemic, older patients hospitalized for COVID-19 exhibited an increased mortality risk compared to younger patients. While ageing is associated with compromised immune responses and frailty, their contributions and interplay remain understudied. This study investigated the association between inflammatory markers and mortality and potential modification by frailty among older patients hospitalized for COVID-19.

Use and Waste of Reconstituted Whole Blood Exchange Transfusions: An 11-year National Observational Study.

J Pediatr

December 2024

Department of Immunohematology Diagnostic Services, Sanquin Diagnostic Services, Amsterdam, the Netherlands; Department of Hematology, Leiden University Medical Center, Leiden, the Netherlands.

Article Synopsis
  • - The study aimed to evaluate the need for exchange transfusions in neonatal care, track the use and waste of transfusion products, and understand their prevalence across the Netherlands.
  • - Data from all 9 neonatal intensive care units and 15 other hospitals were analyzed over an 11-year period, revealing that severe immune hemolysis was the most common reason for transfusions while around 50% of products ordered were not used.
  • - The findings showed that only 14.6 out of every 100,000 liveborn neonates required an exchange transfusion, indicating that these procedures are quite rare in the Netherlands and a significant number of products go unused.

Background: With survival rates of critical illness increasing, quality of life measures are becoming an important outcome of ICU treatment. Therefore, to study the impact of critical illness on quality of life, we explored quality of life before and 1 year after ICU admission in different subgroups of ICU survivors.

Methods: Data from an ongoing prospective multicenter cohort study, the MONITOR-IC, were used.

Background: Older patients with type 2 diabetes mellitus (T2D) have an increased risk of hypoglycaemic episodes when using sulphonylureas or insulin. In the Netherlands, guidelines exist for reducing glucose-lowering medication in older patients. However, evidence is lacking that a medication reduction in older patients can be safely pursued.

Objective: Acute vestibular syndrome (AVS) is a common symptom presented by emergency department (ED) patients. Differentiating peripheral from central etiology poses a challenge and clinical practice lacks a uniform diagnostic approach. This study aims to provide insight on incidence and diagnostics in ED patients presenting with AVS in the Netherlands.

Purpose: In older patients with kidney failure (KF) starting dialysis, there is a high rate of functional decline. Not much is known about the functional trajectory of patients receiving conservative kidney management (CKM). Therefore, the aim of this study is to assess this functional trajectory and explore clinical parameters associated with functional decline.

Article Synopsis
  • This study assessed the effectiveness of magnetic resonance dispersion imaging (MRDI) in detecting clinically significant prostate cancer (csPCa) compared to standard multiparametric MRI (mpMRI).
  • It involved 76 patients, with two radiologists evaluating MRI results and comparing findings to actual prostate cancer histopathology after surgery.
  • Results indicated that MRDI potentially enhances sensitivity in detecting csPCa and reduces variability between observers, with one radiologist finding a 20% increase in detected cases using MRDI.

Background: Intracellular methotrexate polyglutamates (MTX-PGs) concentrations are measurable in red blood cells (RBCs) during MTX treatment. MTX-PG concentrations correlate with efficacy in patients with Crohn's disease (CD). Since RBCs are not involved in pathogenesis of CD and lack extended MTX metabolism, we determined MTX-PGs accumulation in peripheral blood mononuclear cells (PBMCs: effector cells) and intestinal mucosa (target cells) and compared those with RBCs as a potential more precise biomarker.

Background: Distinguishing postoperative fibrosis from isolated local recurrence (ILR) after resection of pancreatic ductal adenocarcinoma (PDAC) is challenging. A prognostic model that helps to identify patients at risk of ILR can assist clinicians when evaluating patients' postoperative imaging. This nationwide study aimed to develop a clinically applicable prognostic model for ILR after PDAC resection.

Management of suspected early-onset sepsis (EOS) is undergoing continuous evolution aiming to limit antibiotic overtreatment, yet current data on the level of overtreatment are only available for a select number of countries. This study aimed to determine antibiotic initiation and continuation rates for suspected EOS, along with the incidence of culture-proven EOS in The Netherlands. In this retrospective study from 2019 to 2021, data were collected from 15 Dutch hospitals, comprising 13 regional hospitals equipped with Level I-II facilities and 2 academic hospitals equipped with Level IV facilities.
