Background: As aging was found to be associated with increased D-dimer levels, the question arose whether D-dimer measurement was useful in the diagnostic strategy of venous thromboembolism (VTE) in elderly patients.

Aim Of The Study: To compare retrospectively the performance of six diagnostic strategies based on the three-level Wells scores and various cut-off levels for D-dimer, evaluated using the HemosIL D-Dimer HS 500 assay, in a derivation cohort of 644 outpatients with non-high pretest probability (PTP) of VTE. The clinical usefulness of the best-performing strategy was then confirmed in a multicenter validation study involving 1255 consecutive outpatients with non-high PTP.

SGLT2 inhibitors and the autonomic nervous system in diabetes: A promising challenge to better understand multiple target improvement.

Diabetes Metab

July 2021

Unit of Endocrinology, Diabetology, Nutrition, AP-HP, Jean Verdier Hospital, Paris 13 University, Sorbonne Paris Cité, CINFO, CRNH-IdF, Bondy, France.

The puzzle of the multifaceted actions of sodium-glucose cotransporter type-2 inhibitors (SGLT2is) is still incomplete. For diabetologists (who are accustomed to dealing with the burden of cardiovascular autonomic neuropathy), the relationship between SGLT2is and the autonomic nervous system (ANS) represents one of the most intriguing aspects of the multiple effects of this family of drugs, especially given the poor availability of disease-modifying treatments for such complication. Therefore, the present review has considered both preclinical and clinical studies of this topic with autonomic perspectives by exploring the pathophysiological background of the interactions between the ANS and SGLT2, including sympathetic control of kidney function and the role of renal afferent nerves, and also by providing insights into the effects of SGLT2is on 24-h blood pressure (BP) and heart rate variability (HRV), with particular attention focused on the EMBODY trial.

Article Synopsis
  • The study examines how laparoscopic liver resection (LLR) affects the quality of surgical care and long-term outcomes for patients with early-stage hepatocellular carcinoma (HCC).
  • Out of 425 patients studied, those who underwent LLR were more likely to achieve "textbook outcomes," indicating a higher quality of care compared to those who had open liver resections (OLR).
  • The results suggest that better surgical quality, associated with the laparoscopic method, leads to improved disease-free survival rates in HCC patients.
