In forensic pathology, identifying causes of death in traumatic brain injuries (TBIs) devoid of observable signs presents a significant challenge. Post-mortem biochemistry plays a crucial role in forensic medicine, particularly in determining causes of death in TBIs that lack macroscopic or histopathological evidence. This study aimed to evaluate the utility of Neuron Specific Enolase (NSE) and S100 Calcium Binding Protein B (S100B) in post-mortem serum and cerebrospinal fluid (CSF) as markers for TBI.

The Indian Society of Critical Care Medicine Position Statement on the Management of Sepsis in Resource-limited Settings.

Indian J Crit Care Med

August 2024

Department of Anesthesiology, Critical Care and Pain, Division of Critical Care Medicine, Tata Memorial Hospital, Homi Bhabha National Institute, Mumbai, Maharashtra, India.

Unlabelled: Sepsis poses a significant global health challenge in low- and middle-income countries (LMICs). Several aspects of sepsis management recommended in international guidelines are often difficult or impossible to implement in resource-limited settings (RLS) due to issues related to cost, infrastructure, or lack of trained healthcare workers. The Indian Society of Critical Care Medicine (ISCCM) drafted a position statement for the management of sepsis in RLS focusing on India, facilitated by a task force of 18 intensivists using a Delphi process, to achieve consensus on various aspects of sepsis management which are challenging to implement in RLS.

Gestational trophoblastic diseases (GTDs) encompass a spectrum of conditions characterized by abnormal trophoblastic cell growth, ranging from benign molar pregnancies to malignant trophoblastic neoplasms. This systematic review explores the molecular underpinnings of GTDs, focusing on genetic and epigenetic factors that influence disease progression and clinical outcomes. Based on 71 studies identified through systematic search and selection criteria, key findings include dysregulations in tumor suppressor genes such as , aberrant apoptotic pathways involving (B-cell lymphoma), and altered expression of growth factor receptors and microRNAs (micro-ribose nucleic acid).

  • * A recent study analyzed CPV genome sequences from clinical samples in India, showing the CPV-2c variant has replaced the previously dominant CPV-2a variant and revealed important mutations in the VP2 protein.
  • * The study suggests that new CPV-2c variants, including the 'CPV-2c 370Arg' variant, exhibit genetic changes that may affect vaccine efficacy, highlighting the need for ongoing research into virus evolution.

Discovering vaccines: the trial tale.

Naunyn Schmiedebergs Arch Pharmacol

August 2024

Department of Pharmacology, Jawaharlal Institute of Postgraduate Medical Education & Research, Puducherry, 605 006, India.

This paper reviews the importance of vaccine trials to public health. Vaccines are developed to build immunity to disease, which have helped to completely eliminate smallpox, and reduced incidence of polio and measles. A variety of breakthroughs made many years ago, such as the smallpox vaccine by Edward Jenner and the discoveries by Louis Pasteur, also set the stage for modern international immunization programs.

Cutaneous metastasis from head and neck cancers is rare, typically presenting as single or multiple nodules. This report presents a truly unique and intriguing case of squamous carcinoma of the tongue, in which the patient developed numerous metastatic nodules in the face and neck, a phenomenon rarely seen in clinical practice. The patient, a known case of carcinoma tongue, was treated radically with concurrent chemoradiation.

Insulinoma, a rare neuroendocrine tumor of the pancreas, often presents diagnostic challenges due to its diverse clinical manifestations. We present the case of a 25-year-old female with recurrent hypoglycemic seizures and neuroglycopenic symptoms, ultimately diagnosed with insulinoma. Despite an initial asymptomatic period, the patient experienced progressively worsening symptoms over three years, culminating in eight episodes of generalized tonic-clonic seizures per week.

Introduction: Mycobacterium tuberculosis has been extensively studied for mutations leading to drug resistance. Pyrazinamide is a drug acting on the semi-dormant bacteria that is responsible for relapse of tuberculosis. This drug helped reduce the treatment duration of tuberculosis from nine to six months.

Our objective was to study the proportion of children developing Catheter-related thrombosis (CRT) following central venous Catheter (CVC) insertion and the risk factors of CRT in pediatric patients with CVC. One hundred four children aged 29 days to 18 years who had a percutaneous non-tunneled CVC inserted were enrolled. Ultrasonogram (USG) with venous Doppler scan was performed within 48 hours of CVC removal to diagnose CRT.

The assessment of the transformation zone is a critical step toward diagnosis of cervical cancer. This work involves the development of a portable, label-free transvaginal multispectral diffuse optical imaging (MDOI) imaging probe to estimate the transformation zone. The images were acquired from N = 5 (N = 1 normal, N = 2 premalignant, and N = 2 malignant) patients.

Article Synopsis
  • The study focuses on the rising prevalence of lung cancer, particularly non-small cell lung cancer, and the challenges posed by drug resistance and toxicity in treatments involving gemcitabine.
  • It involved 184 healthy volunteers and 123 cancer patients in Southern India, using advanced techniques like RT-PCR to analyze genetic variants that may affect treatment outcomes.
  • Key findings revealed a minor allele frequency of a specific SNP (NRF2-617 C>A) among the healthy population, with no significant gender differences, suggesting genetic variants might influence the effectiveness and safety of gemcitabine therapy in lung cancer patients.

Background: The role of adjuvant therapy in resected periampullary adenocarcinomas is equivocal due to contrasting data and limited prospective trials.

Methods: The Multicentre Indian Pancreatic & Periampullary Adenocarcinoma Project (MIPPAP), included data from 8 institutions across India. Of the 1679 pancreatic resections, 736 patients with T3/T4 and/or Node positive adenocarcinomas (considered as high risk for recurrence) were included for analysis.

Article Synopsis
  • * The study analyzed 134 adults with nephrotic syndrome, focusing on how initial protein levels impacted long-term kidney health (eGFR decline) and disease remission/relapse over 18 months using specific statistical methods.
  • * Findings revealed that patients with lower (sub-nephrotic) protein levels at the start were less likely to progress to severe kidney disease and more likely to achieve remission, indicating that initial proteinuria levels have significant prognostic importance.

Cutaneous myiasis is an infestation of the tissues and organs of living vertebrates and humans by fly larvae, usually those belonging to the Calliphoridae family. The larvae feed on the host's necrotic or living tissue. Pre-existing dermatological conditions and poor hygiene are predisposing factors for cutaneous myiasis, which is especially common among neglected, dependent patients.

Plumbagin is a naphthoquinone from the roots of the Plumbago species and exhibits anticancer activity. Translational usage of plumbagin in biomedical sciences is restricted due to its poor solubility and bioavailability. Therefore, pH-responsive plumbagin-loaded vaginal nanoformulations with polylactic acid (PLA)-chitosan polymeric coat were fabricated by inotropic gelation technique.
