Tc-99m glucoheptonate is poor man's fluorodeoxyglucose.

Indian J Nucl Med

October 2011

Department of Nuclear Medicine and Radioimmunoassay, Lilavati Hospital and Research Centre, Department of Nuclear Medicine and PET-CT, Jaslok Hospital and Research Centre, Mumbai, India.

While fluoro-deoxy-glucose (FDG) has emerged as an important radiotracer for imaging tumors, myocardial viability and infection, the role of other glucose analogues should also be explored. Tc-99m Glucoheptonate (GHA) has been used for imaging brain tumors and lung tumors. The uptake mechanism may be linked to GLUT-1 (Glucose transporter) and GLUT-4 expression similar to FDG.

Organ donation: victory after death.

J Assoc Physicians India

August 2011

Jaslok Hospital and Research Centre, Mumbai 400 026.

Despite the Transplantation of Human Organ Act passed in Indian Parliament in 1994, cadaver liver and/or kidney transplant are infrequently performed (in a few private hospitals) in our country compared to living donor liver or kidney transplant. The need for performing more cadaver liver and/or kidney transplants in private and public hospitals is obvious. Immediate measures which should be taken to facilitate more cadaver organ transplant both in private and public hospitals are suggested.

J Assoc Physicians India

August 2011

'The Headache and Migraine Clinic', Jaslok Hospital and Research Centre, 15, G. Deshmukh Marg, Mumbai 400026, India.

Objective: The aim of the present study was to compare the analgesic efficacy of a single dose of sublingual piroxicam to that of a placebo during acute attacks of migraine without aura.

Methods: The drug (N = 30) or a placebo (N = 30) was administered, on randomisation and double-blind basis, to 60 patients between 18 and 50 years of age suffering from migraine without aura. The patients were instructed to take a single tablet sublingually [corresponding to piroxicam 40 mg or placebo] and the severity of the painful symptomatology and associated symptoms were evaluated by this study.

Anterior segment infiltration in acute myeloid leukemia (AML) presenting as hypopyon uveitis is very rare. We report this case as an uncommon presentation in a patient on remission after bone marrow transplant for AML. In addition to the hypopyon, the patient presented with "red eye" caused by ocular surface disease due to concurrent graft-versus-host disease and glaucoma.

Does model for end-stage liver disease (MELD) require modification?

J Assoc Physicians India

June 2011

Jaslok Hospital and Research Centre, Dr G Deshmukh Marg, Mumbai 400 026.

Model for End-stage Liver Disease (MELD) measures serum bilirubin (mg/dL), serum albumin (g/dL), International Normalised Ratio (INR) for prothrombin time. MELD score accurately predicts survival in 3 months for cirrhotic patients on the waiting list of liver transplantation. In about 15% of patients, it does not accurately predict survival and hence MELD-Na is recommended.

Targeting mTOR pathway: A new concept in cancer therapy.

Indian J Med Paediatr Oncol

October 2010

Jaslok Hospital and Research Centre, Mumbai, Maharashtra, India.

This article highlights the current knowledge of mTOR biology and provides new insights into the role of mTOR in different cancers. An active mTOR coordinates a response in cell growth directly through its effects on cell cycle regulators and indirectly by sustaining nutrient supply into the cell through the production of nutrient transporters and also through the promotion of angiogenesis. A primary way that mTOR exerts its regulatory effects on cell proliferation is by controlling the production of cyclin D1.

Antifibrinolytics in liver surgery.

Indian J Anaesth

November 2010

Department of Anaesthesia, Jaslok Hospital and Research Centre, Mumbai, India.

Hyperfibrinolysis, a known complication of liver surgery and orthotopic liver transplantation (OLT), plays a significant role in blood loss. This fact justifies the use of antifibrinolytic drugs during these procedures. Two groups of drug namely lysine analogues [epsilon aminocaproic acid (EACA) and tranexamic acid (TA)] and serine-protease-inhibitors (aprotinin) are frequently used for this purpose.

Objective: The centrosome is the microtubule organizing center (MTOC) paternally inherited by the zygote during fertilization. As the centrosome is located in the midpiece of the sperm tail, we presume that oligoasthenozoospermic sperm samples should also have abnormal concentrations of centrosomal proteins. This study therefore aims to determine if there is any correlation between sperm centrosomal proteins, centrin, α and γ-tubulin, in sperm samples from normozoospermic and oligoasthenozoospermic men.

Renal transplantation in patients with malignancy is controversial. Renal transplantation is generally not considered for patients with multiple myeloma (MM) because of their extremely poor prognosis. There are few reports of MM recurrence among kidney transplant recipients.

Background: The use of granulocyte colony-stimulating factors to treat patients with chemotherapy-induced neutropenia is well accepted. To assess whether administration of filgrastim along with standard empiric antibiotic therapy is beneficial for patients with chemotherapy-induced febrile neutropenia (FN), we conducted an open, non-randomized clinical trial.

Materials And Methods: This was a prospective, open, Phase IV clinical trial in patients receiving chemotherapy for histologically confirmed cancer, with an oral temperature of >38.

Acute Lymphocytic Leukemia (ALL) is a clonal myeloid disorder affecting all age groups, characterized by accumulation of immature blast cells in bone marrow and in peripheral blood. Autologous Bone Marrow Transplantation is a present treatment for cure of ALL patients, which is very expensive, invasive process and may have possibility of transplantation of malignant stem cells to patients. In the present study, we hypothesized to isolate large number of normal Mesenchymal & Hematopoietic stem cells from peripheral blood of ALL patients, which will be further characterized for their normal phenotypes by using specific molecular stem cell markers.

The first 'International Classification of Headache Disorders' by The International Headache Society (1988) was a major landmark document. It helped develop a common language for understanding headaches and advanced research in this neglected field. It was found to be useful, but because it was complex and cumbersome, it remained underutilised in practice.

Recurrent cholangitis in the tropics: worm or cast?

J Postgrad Med

November 2010

Department of Surgical Gastroenterology, Jaslok Hospital and Research Centre, Mumbai, India.

The development of biliary casts is very rare, especially in non-liver transplant patients. The etiology of these casts is uncertain but several factors have been proposed which lead to bile stasis and/or gallbladder hypo-contractility and promote cast formation. Here, we report a 54-year-old male, with diabetes and ischemic heart disease, who presented with recurrent attacks of cholangitis.
