Cardiac strangulation is a rare phenomenon in children following epicardial pacemaker implantation, caused by compression of the heart and great vessels by the epicardial pacemaker wires. We report a rare case of cardiac strangulation presenting after 8 years of epicardial pacemaker implantation. On routine follow-up, computed tomography angiography showed significant extrinsic compression of the mid-left anterior descending (LAD) artery by the epicardial pacing wire.

Extramedullary infiltration of acute lymphoblastic leukemia/lymphoma (ALL) to genital organs is extremely rare. Here, we present a case report of an asymptomatic 49-year-old female, known case of precursor B-cell ALL, who was incidentally detected with thickened and heterogeneously hyperechoic endometrium on sonography. Contrast magnetic resonance imaging detected large polypoidal enhancing lesions showing intense diffusion restriction occupying the endometrial cavity and similar lesions in the left adnexa, left ovary, and fallopian tube which were suspicious for leukemic infiltration because of the clinical history and atypical appearance of the lesions.

Malignancies are increasing worldwide with changing lifestyle, pollution, increasing life expectancy, and diagnostic advancements. However, multiple primary malignancies (MPMs) detected simultaneously are very rare. Here, we present a rare case of three primary malignancies (sigmoid colon, prostate, and thyroid) detected simultaneously in a 77-year-old male patient, who initially presented with bleeding per rectum and was then found to have a large pedunculated mass in the sigmoid colon on colonoscopy, which further turned out to be adenocarcinoma.

Progressive supranuclear palsy (PSP) has emerged as a key area of interest among researchers worldwide, including those in India, who have actively studied the disorder over the past several decades. This review meticulously explores the extensive range of Indian research on PSP up to the present and offers insights into both current initiatives and potential future directions for managing PSP within the region. Historical research contributions have spanned 80 publications from 1974 to 2023, encompassing diverse themes from clinical phenotyping and historical analysis to isolated investigative studies and therapeutic trials.

This real-life observational study from a diabetes centre in Western India reports the status of A1C (glycated hemoglobin), blood pressure, and cholesterol levels (ABC goals) currently achieved in type 2 diabetic patients. A cohort of 497 patients of type 2 diabetes first seen at the Diabetes Endocrine Nutrition Management and Research Centre from the years 2014 to 2017 were followed for a median [interquartile range (IQR)] duration of 21.5 (7, 33) months.

Balancing the risks of ischemic and bleeding events in patients who have undergone coronary angioplasty with drug-eluting stents (DES) is a delicate task. Individuals recovering from myocardial infarction are at increased risk of recurrent ischemic events, highlighting the potential benefits of rigorous secondary prevention measures. Dual antiplatelet therapy (DAPT), combining aspirin with P2Y12 inhibitors such as clopidogrel or ticagrelor, forms the cornerstone of post-percutaneous coronary intervention (PCI) care aimed at preventing stent thrombosis and lowering the risk of ischemic events.

Background: Parkinson disease (PD) is a progressive neurodegenerative disorder characterized by motor dysfunctions and nonmotor symptoms. Current treatments do not alter disease progression, highlighting the need for alternative therapies. Music, dance, and mindfulness meditation have shown the potential to improve symptoms and quality of life in patients with PD.

Ubiquitin-specific protease 53 (USP53) is essential for formation of cellular tight junctions and variations in this gene disrupt the tight junctions, resulting in cholestasis. We describe the clinical manifestations and outcomes of patients with USP53 mutations from the Indian progressive familial intrahepatic cholestasis registry. All 29 patients who harbored mutations in the USP53 gene either in the homozygous, compound heterozygous, or heterozygous state and presented with cholestasis were included.

A strategic vision toward global convergence in transplantation must encourage and remove barriers to living organ donation and transplantation. Here, we discuss deliberations of a working group of the 2023 Santander Summit charged with formulating recommendations for the safe expansion of living donor kidney transplantation and living donor liver transplantation worldwide. Living donor kidney transplantation has grown to be the preferred treatment for advanced kidney failure.

Establishing transparency and oversight of organ transplantation by regulatory agencies is of paramount importance to assure ethical, legal, and clinically robust transplantation practices. Registries reporting activity and outcome data of the donor and recipient, including donor source (living or deceased), must be developed for each transplant and should be a mandatory requirement to achieve accreditation to perform transplant surgeries. Collected data for the living organ donor must include the nationality, the nature of their relationship with the recipient, and the complications encountered by living donors that result in prolonged morbidity or mortality.

Polymorphous adenocarcinoma (PAC) of head and neck tumors is a rare salivary gland neoplasm of indolent course. We reported a 63-year-old female who presented as an asymptomatic mass in buccal space. The patient, after metastatic workup, underwent complete excision of the lesion with a negative margin.

Purpose: The blur introduced by breathing motion degrades the diagnostic accuracy of whole-body F-18 fluorodeoxyglucose positron emission tomography-computed tomography (F-FDG PET-CT) in lesions adjacent to the diaphragm by increasing the apparent size and by decreasing their metabolic activity. This study aims to evaluate the efficacy of motion correction by four-dimensional phase-based respiratory-gated (RG) F-FDG PET-CT in improving metabolic parameters of lesions adjacent to the diaphragm (especially in the lungs or liver).
Materials And Methods: Eighteen patients with known lung or liver lesions underwent conventional F-FDG PET-CT and respiratory-gated PET-CT acquisition of the desired region using a pressure-sensing, phase-based respiratory-gating system.

The history of ventral hernia repair techniques has seen substantial evolution, from primary suture repair to the introduction of mesh-based procedures, aiming to reduce recurrence rates. Notable advancements include the anterior and posterior component separation techniques. The Transversus Abdominis Release (TAR) technique, a refinement of the posterior approach, emerged to address challenges associated with complex ventral hernias.

Background: Current guidelines recommend the perioperative continuation of aspirin in patients with coronary drug-eluting stents (DES) undergoing noncardiac surgery. However, supporting evidence is limited.
Objectives: This study aimed to compare continuing aspirin monotherapy vs temporarily holding all antiplatelet therapy before noncardiac surgery in patients with previous DES implantation.

Timely diagnosis and management of pediatric acute liver failure (PALF) is of paramount importance to improve survival. The Indian Society of Pediatric Gastroenterology, Hepatology, and Nutrition invited national and international experts to identify and review important management and research questions. These covered the definition, age appropriate stepwise workup for the etiology, non-invasive diagnosis and management of cerebral edema, prognostic scores, criteria for listing for liver transplantation (LT) and bridging therapies in PALF.

Immune checkpoint inhibitors (ICI) have transformed the management of cancer, particularly for older adults, who constitute a majority of the global cancer patient population. This study aimed to assess the inclusion, characteristics, and reporting of older adults enrolled in Food and Drug Administration (FDA) registration clinical trials of ICI between 2018 and 2022. Clinical trials of ICI leading to an FDA approval in solid tumor oncology between 2018 and 2022 were included.

Background: Ataxia telangiectasia is a multisystem disorder with progressive neurodegeneration. Corticosteroids can improve neurological functioning in patients with the disorder but adrenal suppression and symptom recurrence on treatment discontinuation has limited their use, prompting the development of novel steroid delivery systems. The aim of the ATTeST study was to evaluate the efficacy and safety of intra-erythrocyte delivery of dexamethasone sodium phosphate compared with placebo in children with ataxia telangiectasia.
