Objective: The management of cervical spine injuries in the elderly is often complicated by the presence of multiple medical comorbidities, and it is not uncommon for preoperative testing to reveal other conditions that require the postponement of surgery. However, the factors that affect the waiting time from injury to surgery have not been clarified. The purpose of this multicenter database study was to analyze the clinical features and identify the factors affecting the number of days waited between injury and surgery in elderly patients with a cervical spine injury.

Purpose: Identifying patients at high risk of immune-related adverse events (irAEs) that impede the achievement of durable efficacy of programmed cell death 1 (PD-1)/programmed death ligand 1 (PD-L1) blockade therapy is important in improving their management. Identification of a novel predictive factor of therapeutic benefit is also important in improving patient selection for treatment with PD-1/PD-L1 inhibitors. Further determinants driving response and linking with irAEs are urgently required.

Introduction: Although recent advances in chemotherapy for lung cancer are remarkable, most clinical trials have excluded patients with interstitial lung disease (ILD) due to the concern of developing acute exacerbation (AE) of ILD. Hence, accumulating original evidence of cancer treatment for this population is important.

Methods: Between 2016 and 2020, a prospective observational study was conducted across 11 Japanese hospitals.

Article Synopsis
  • Cervical esophageal cancer (CEC) has a low incidence and poor prognosis, prompting a study in Japan to determine effective treatment strategies from 2009 to 2014.
  • A review of 302 CEC cases revealed that various treatments, including endoscopic resection, surgery, induction chemotherapy, and chemoradiotherapy, showed no significant differences in 5-year survival rates; advanced stage and nerve invasion were linked to poorer outcomes.
  • Although advanced CEC patients may preserve their laryngeal function with induction chemotherapy or chemoradiotherapy, careful treatment selection is crucial to avoid complications like aspiration pneumonia if laryngeal preservation isn’t possible.

Article Synopsis
  • The study analyzed the effectiveness of the inactivated influenza vaccine in children from 6 months to 15 years old over multiple seasons from 2013/14 to 2020/21.
  • Results indicated that the vaccine was effective against influenza A and B, with notable effectiveness in children aged 1 to 2 years old, while younger children (6-11 months) showed lower effectiveness against influenza B.
  • The findings suggest that the vaccine can be recommended for children across different age groups for future influenza seasons, even during and after the COVID-19 pandemic.

Background: Vonoprazan, a potassium-competitive acid blocker, inhibits gastric acid secretion and attenuates the antiplatelet function of clopidogrel more potently than esomeprazole. We investigated whether alternate-day dosing of vonoprazan might avoid this interaction with clopidogrel while providing sufficient gastric acid inhibition.

Methods: Following 24 h of pH monitoring (control regimen), 12 healthy volunteers received three regimens (clopidogrel-only regimen: clopidogrel 75 mg daily [q.

Purpose: In two 24-week migraine prevention studies in Japan, erenumab was associated with significantly greater reductions in migraine frequency versus placebo over Weeks 13-24 (primary endpoint). This post hoc analysis evaluated the onset of efficacy within the first 4 weeks after the initiation of erenumab from the 24-week double-blind periods of these studies.

Methods: Placebo-adjusted differences in least squares mean (LSM) change from baseline in weekly migraine days (WMD) were assessed weekly in each study and by migraine type (episodic (EM]/chronic [CM]) (Study 20170609).

Lupus anticoagulant (LA), an antiphospholipid antibody, prolongs activated partial thromboplastin time (APTT) despite the presence of a hypercoagulable state . Irrespective of whether they receive antithrombotic therapy, meticulous anesthetic management is imperative in patients with LA positivity to prevent thrombotic complication. Additionally, emergency surgery in such patients can be challenging, as the time to devise perioperative strategies is limited.
